    Hi I have watched this video again and again for months and I just had the courage now to comment on your post. I hope I get a respond from you. I wanna have an eyelid surgery because ever since I was young my family and I have noticed that my eyelids are not even. And as I grew older it got worse. My left eyelid is so droopy and my right eyelid doesn’t have a lid at all lol. I am from the Philippines so here treatments like this are rare and they can never really addressed the issue I have over my eyelids. As I grew older I started having insecurities because people would make fun of me wearing one eyelid tape on my right eye so that it looks even. At one point an ex of mine made fun of my eyes and said that I am ugly. My uncle on my dad side had the exact same eyes as me and he had an upper blepharoplasty surgery here and after just a couple of months it went back to the way it looks before he had the surgery and you can really see the stitches which made it look worse so I am so scared to have it done with a wrong doctor. Do you happen to have any clinics here in Asia? Or someone you can refer to me to really help me with my issue? I think what I have is ptosis and I wanna have this corrected. I hope you will respond on this and help me.

    Hi ms mownique, Sorry to hear about your problem. There aren't many oculoplastic/oculofacial surgeons worldwide, and some ophthalmologists in other countries claim to perform oculoplastic surgery, so I can't make any recommendations due to varying expertise in ptosis correction surgery in other countries. I only practice medicine in the US. Having ptosis treated with blepharoplasty is a common mistake as cosmetic and plastic surgeons are not trained to operate on the muscle that lifts the eyelid. The only way I could really help you is if you had eyelid ptosis diagnosed and operated on in our offices in the US. I hope this helps you.

    Hi Kendall mclean, Sorry to hear about your problem. You should follow up with your surgeon as any surgeon with experience with eyelid ptosis correction knows that additional enhancement is sometimes needed because of the precise nature of the surgery,, so they should have a revision policy in place. Also, your surgeon knows details of your condition, and your surgery that I'm unaware of. Your surgeon can also physically examine you properly, while I could only speculate a number of things if I don't examine the issue. You can refer to this video about why ptosis can recur after previous surgery, but this focuses on recurrence years after the previous surgery - kzread.info/dash/bejne/Znp8p7mqgK7Afbg.html

    Hi Dr. Prasad, I had ptosis surgery on one eyelid 18 days ago. It was an acquired ptosis. The operated eye lid is now 1 to 2 mm higher than the other one. My surgeon told me that I have to do massage to lower the eye lid and didn't want to do a correction surgery. Does massage really work? And how long will it take? Another ophthalmologist told me that there is still some swelling (which is not visible for me) which causes a second crease and tighten tissue, after swelling gets back eye lid should lower a little when tissue is relaxed. So massage is relaxing muscle and tissue. Would you agree? Thanks for your time.

  • @AmiyaPrasadMD

    @AmiyaPrasadMD

    Hi Ahmet Topaloglu, I generally don't advise massage after delicate eyelid surgery as you can undo the surgical work you just paid a lot for. I would have to examine your eyelids and know details of your surgery to see if immediate enhancement can be done, or if it's best to wait for more healing before considering a revision. I hope this helps you.

    Hello Dr Prasad, I have mild/moderate acquired aponeurotic ptosis in my left eye and I am planning to get a ptosis surgery, however I have a concern. I have seen that post ptosis surgery, in case of unilateral correction there is difference in eye lid heights (the eyelid with surgery is little higher than the normal eye) while looking down. This makes the operated eye look odd with bigger white region visible than the other eye. My doctor has suggested to incorporate behavioral change to look down with my head lowered and not just to look down with my eyes. Could you share whether every patient will face this issue ? Are there techniques(surgical/exercises) with which we can avoid / lower this occurrence ? TIA

    Hello! I’ve had ptosis surgery on both of my eyes, but one of them seems over-corrected. The surgeon have tried to re-do it for 4 times during 3 years and it still goes back to the original state where it was over-corrected. He recommended stretching of the eyelid to see if it helps. But I don’t feel it makes any difference. It can be that one of my eyes have a stronger levator muscle. But it seems too difficult for the surgeon to calculate this. If I’m gonna stretch the eyelid, how many days do I need to wait after the surgery and how do I do it? Like for how long and how often? Is there something else I can do to make it weaker or lengthen it? It’s just 1-2 mm difference. But the eye aches, becomes red (probably due to exposure of air). If I stretch it just 1-2 mm down so it’s symmetrical with the other eye, the pain goes away.

    Hi Millie Lu, A difference of 1-2 mm in height between the two eyelids is normal, and unnoticeable to others. The redness and pain you are feeling are things I would have to diagnose properly before making recommendations. I hope this clarifies things for you.

    Hi Izzy, While there are risks to any surgery, those risks are reduced by the experience and expertise of the doctor performing them. I've personally not had ptosis patients experience the extreme complications you mention, but sometimes further enhancement is needed for over or under correction of eyelid height as it is a very precise procedure. I hope this clarifies things for you.

  • @AmiyaPrasadMD

    @AmiyaPrasadMD

    3 жыл бұрын

    Hi Diogooo Afonso, Sorry to hear about your problem. If your unsatisfactory results from eyelid ptosis correction were soon after the surgery, then any doctor experienced in this surgery does know that further enhancements may be needed since it's such a precise and delicate surgery, so it should have been addressed by your surgeon at the time. However, if it drooped further years later, then another surgery may be needed as the eyelid muscle can stretch or detach years after surgery. I hope this helps you.

Is this normal and how "open" should I expect my eyes to be post-surgery?

  • @AmiyaPrasadMD

    @AmiyaPrasadMD

    4 жыл бұрын

    Hi Langa's House, Not having complete eyelid closure is a normal risk with a frontalis sling as it is a very precise surgery, so even a little variation can lead to incomplete closure. I would have to assess your situation in a physical exam to determine the risk myself. Eyelid closure is not as much of an issue if your eyeball rolls up when you sleep called Bell's phenomena, but is more of an issue if it doesn't. If you can't come for a consultation with me, you may want another opinion from an oculoplastic/oculofacial surgeon who can examine your eyes. I hope this helps you.

    Hi Magnolia14, Unfortunately, use of contact lenses can contribute to ptosis because of the added friction between the contact lens and the upper eyelid, creating additional strain on the levator muscle. I hope this clarifies things for you.

    I had congenital ptosis .I had my surgery at the age of 13 , the rapid growth period of life. muscles taken from thigh side were implanted on my forehead .As the outcome, I could open my eyes a bit wider , but by applying force with the muscles which were implanted ,it wasn't quite helpful at all. Instead i am suffering from unexpected side effects.The most problematic side effect is , my forehead,for some reason hasn't grown as much as it should have in the last 4 years , since the surgery . It is awfully small , seems like hasn't grown since then .As I said it was done at the time of my rapid bone growth, could it be that the muscles hampered the growth of my frontal coroti ? And will not also put bad effect on my brain development and intelligence quality?

    Hi Tauhid Bhuiyan, Sorry to hear about your problem, but an examination is needed to determine what can be done for you. I have operated on children with congenital ptosis using the fascia lata for the frontalis sling procedure, and saw them when they grew up and the muscle grew with their forehead. I hope this clarifies things for you.

    Hi Abu Luqman, If your surgery was performed well, you should be able to close your eyes fully. With the precise nature of ptosis surgery, sometimes enhancements are needed, even with the best oculoplastic surgeons. Frontalis sling does take time to get used to for the patient as a different muscle is used to raise and close the eyelids. I hope this clarifies things for you.

    Hi mac smith, It is unlikely that your sleeping position had an effect on your results (unless you are sleeping face down, and directly on the operated area). Most of the healing takes place in the first 2 days after surgery, and after that it's more about resolving swelling. If you do have concerns, you can follow up with your surgeon who can physically examine where you are it in the recovery process. I hope this helps you.

    Hi Michel'le Feaster, It has only been a day since your son's surgery, and temporary swelling is expected. I don't know the details of your son's surgery, but it may take about a month before disorganized collagen fibers flatten, for more significant swelling to go down. Temporary swelling from eyelid surgery usually takes several months to dissipate, and full surgical healing takes about a year, but your son may experience faster healing since he is very young. I suggest you follow up with your surgeon, but temporary swelling this soon after surgery is definitely normal. I hope this clarifies things for you.

    Hi ramunniie, There is no success rate that applies to everyone as it depends on the severity of the ptosis, and the doctor that performs the surgery. Any surgeon experienced in eyelid ptosis knows that enhancement is sometimes needed as it is such a precise surgery. People suffering from eyelid ptosis usually require more than one surgery in their lifetime, especially if they were born with it. While surgery can be successful, the levator muscle is engaged thousands of times a day, every day, so surgery is usually needed again years down the road. Hello! I did ptosis surgery 3,5 weeks ago. They made the insicion on the outside of the eyelid, so its visible (i hope you understand my english). The eyelid is slightly peaking upwards in the centre of the eye. The scarring is pretty thick and creating a second crease. Im worried its hypertrofic and wont go down. Because of the peaking of the eyelid i can barely see my eyelid at the centre of the eye when looking straight forward (hooded by the peaking). I have started eyelid massage and the peaking is not as sharp anymore. Its still one millimeter higher than my non-operated eyelid thought. Do you think my issue possibly can resolve by itself or will i need revision surgery? Thank you!

  • @AmiyaPrasadMD

    @AmiyaPrasadMD

    Hi Nadia, It depends on how Botox treatment is performed. In the Botox brow lift, for example, Botox is injected into muscles that push the forehead down, so the frontalis muscle works unopposed, and raises the eyebrows. However, forehead wrinkles develop sooner in people with a frontalis sling procedure because they use their forehead to open their eyelid(s), so if you have forehead wrinkles treated with Botox, it can affect the ability to raise your forehead, and open your eye with the frontalis sling. I hope this clarifies things for you.

I can only give the guidance in the video about healing, but your father's surgeon who knows the details of his eyelids, and how the surgery was performed can tell you about where he is in the healing process, and how effective the surgery was. Any surgeon who is experienced in ptosis surgery knows that additional enhancement is sometimes needed because it is such a precise surgery. Your surgeon should have a revision policy in place for eyelid ptosis surgery. I hope this helps you.

    Hi Stanzin Chuskit, It is possible to treat, but I would have to perform an eyelid ptosis exam, as well as assess your brow and forehead for possible contribution to the droop. If you want to have Lasik and ptosis correction, you may want to consider Lasik first as the process does retract your eyelids, which may be more delicate soon after ptosis correction surgery. Hi @lanceoneail7603, You are still very early on in your healing, so swelling will still continue to go down. I have my patients use cold compresses for the first two days after surgery, then warm compresses for two days after that. Collagen fibers start to flatten about a month after surgery, so swelling will go down further from there, as detailed in the video. I hope this clarifies things for you.

    Hi FAYESZ AHAMED, The fluid in the face after frontalis sling surgery can cause swelling. You can either be patient until the fluid absorbs, or apply warm compresses to help clear the fluid. Is upper blepharoplasty the same as a ptosis correction in one eye?

  • @AmiyaPrasadMD

    @AmiyaPrasadMD

    Жыл бұрын

    Hi R. K., They are completely different surgeries as upper blepharoplasty treats the skin, and possible fat behind the eyelid; while ptosis correction treats the muscle. Your eyelid will still droop after a blepharoplasty, but you may end up with a skin shortage. Also, general plastic and cosmetic surgeons are not trained to treat eyelid ptosis, you need to see an oculoplastic/oculofacial surgeon for ptosis correction. Hi PACE.ROCKS, While ptosis may require more than one surgery in a lifetime, it usually lasts longer than two years, but may done sooner due injury, or the weakened condition of the levator muscle. The number of repeat ptosis surgery depends on if the levator muscle can still be resected or shortened, and if it still has some strength. If the levator muscle can no longer be shortened, or if it's too weak, a frontalis sling is performed, where the muscle that lift the brow called the frontalis is used to lift the eyelids. I will make a video about it soon, although I do have an older video of people born with ptosis requiring more than one surgery - kzread.info/dash/bejne/naWltNKDmL24erw.html

    @@AmiyaPrasadMD Thank you very muc for your reply. It´s not actually a condition, it appeared right after I woke up from my neck surgery because during that my sympathetic nerve got damaged. I got a schwannoma removed, a benign tumour of a nerve, and that nerve seems to have been the sympathetic nerve, hence the Horner´s syndrome. I have already seen doctors about it and they all agree it´s caused by damage during surgery, so the cause of the syndrome is clear. My question is, how can you treat a congenital ptosis combined with Horner´s syndrome? Do bith affect the same muscle or do I have two muscles now which are not functioning properly? After using them, my eye goes to completely normal (save a the minimal congenital ptosis) for about 1-2 hours. Would using this agent instead of surgery be an option? Sorry for all the questions!

    Hi@@Punki80, While it is possible to treat your concurrent conditions, you do have a lot going on, so I would have to properly assess you in consultation before making a specific recommendation for treatment. I hope you understand.

    @@AmiyaPrasadMD Thank you very much, I understand. It´s enough for me to know that you think it is possible to treat this, this is a help already. Sadly I live in Germany, so I can´t come over for assessment. But I think we have at least one specialist eye surgeon whose good reputation seems to be known nationwide over here, and he is not even far away from me, so I will see him about this if the nerve doesn´t recover. As yet I have been told I have to wait until next year to be sure whether the nerve has been permanently damaged in my neck surgery or not. Only then can a surgery be taken into consideration.

    Hi Sanusi michelle, It's important to have congenital ptosis treated before age 5 for the child's visual development. You can consult a pediatric ophthalmologist about it. I hope this clarifies things for you.

    Hi Raj monga, Sorry to hear about your problem. While I would have to examine you before making a definitive recommendation, it may be possible to revise and reconstruct your affected eyelid. You can refer to this video about it - kzread.info/dash/bejne/k6erl6qQYLy_Z5c.html

    Hi Atta ullah, Sorry to hear about your problem. You should follow up with the doctor who did your surgery, as any surgeon experienced with ptosis correction knows that further enhancement is sometimes needed. I can't assess your situation as I have not seen your results, nor do I know details of your surgery, but your surgeon does. I hope this helps you.

    Hi Kajal Verma, It is explained in the video that temporary swelling depends on the type of surgery being done, but in general in our practice, most swelling dissipates in the first month, with residual swelling depending on how individual patients heal. I hope this clarifies things for you.
