Definitive Test! MCT Oil in Coffee When Intermittent Fasting -2 Fit Docs Run The Tests

We tested our ketone and blood glucose levels at multiple times after drinking coffee with MCT Oil, Cream, and Butter added, so we could bring you a definitive answer to the question...
What can I add to my coffee when I am intermittent fasting.
Over the course of four mornings, we (2 Fit Docs) stuck our fingers 16 times with our Precision Xtra Blood Glucose and Ketone meter.
Each morning we tested one ingredient:
Day 1 - Black Coffee (the control)
Day 2 - Heavy Cream
Day 3 - MCT Oil (most interesting result!)
Day 4 - Butter
As you may know, heavy cream, MCT oil, and butter are often used to create a form of keto coffee or bulletproof coffee.
We have often speculated that the ingredients were almost entirely fat, so they should not raise our blood glucose readings or kick us out of ketosis, but until now, this was just an educated guess.
Watch the video for the definitive answer on what does or does not knock you out of ketosis when you're intermittent fasting.

This video discusses fasting in the context of a ketogenic diet for the purpose of controlling insulin as it relates to obesity. "Some people will do a classic water-only fast, others a modified fat fast, and still others a bone broth fast" Page 202 'The Diabetes Code' Jason Fung, M.D.

What about autophagy and HGH, the two other benefits of post 12-hour fasting? I think Dr Fung said autophagy is not affected by fat but he didn't specify if HGH is affected. I think autophagy would necessarily mean HGH is occurring but I am a lay person and have not investigated the studies.

Some in the autophagy world believe that anything other than water during a fasting period will halt the process and others are a little more liberal in their opinions. Our guess is that HGH, autophagy, gene expression, etc. probably require a more rigorous fast to get the wheels rolling.

Almost 6 months ago to the date I had a small heart attack (98% blockage in my LAD). I'm only 38 and had not been to the Dr in 6 years. When I was in the ER I was told that I was diabetic and later was told my A1C was 10.1. A month after the hospital I lost very little weight was (was about 250) and my glucose numbers were only down a small amount but were very unstable. My meds killed the thought of avoiding food because I would be hypoglycemic often. I decided to try ketosis because I had lost weight doing low carb in the past so keto sounded even better. Today I am 213 lbs and a month ago my A1c was 5.2. I'm not on diabetes meds anymore and my cardiologist couldn't believe my numbers. Just thought I would share. As a small update. Weight is down to 203 and A1c is 5.1. Cholesterol is like 20.

This was heartening to me, as I want to stay in ketosis, which has not only reduced my excess weight slowly, but also reduced the ache in all my joints from the hips down to almost nothing since the end of last summer. However, I really, really love to start my day whenever I wake up and finish meditating with a couple of cups of coffee with cream, even though I don’t usually eat any food until around noon. I am a retired 70-year-old, so I’m usually free to start my day slowly. It’s good to know that the coffee and cream are likely to at worst have no effect on my fasting period. Good job of testing your hypotheses! Thanks for sharing the results!

Ruth Clarke yes, I agree about the profiles. I believe the more correct interpretation of the data is it is showing insulin spikes of varying amounts. Was the previous evening meal the same every test? Perhaps Dr. Panda’s circadian theories are in play here. The 78@120 for black coffee would seem that way. But, please! Bravo! Thank you both for this great test, it gives much food for thought to all of us who have adopted the fasting and keto lifestyles!

Technicaly fats boost ketones yet if one is doing IF for autophagy....it seems that fats break the fast from this point of view. I just finished a 68 hours of fast , no fats at all, my ketone began with 1,5 and finised above 5.6. And feeling great. Keith ...that ketone level so low must make it harder for you to fast as hunger may appear more often. I know...took me 4 months to get rid of insulin resistance but only ketogenic diet was not enough. I began fasting by 18 hours and now even 3 days but easiest is 48 hours once a week. Loved your video. Very interesting 😊

Alina Cretu yes, my concern is stopping autophagy. I think I will consider adding an occasional fatty coffee while doing IF on hectic days when I need the energy boost. But on extended fasts of 24 hrs+, I will just stick to water.

lonesumtee autophagy increases almost 300%in first 24 h and another 30 % in the next 24....so if you do OMAD you will get the best of authophagy without raising your cortizone for stress for not eating. At leat 24 h anybody can skip fats and drink only tea and coffee 😘. With time fasting comes naturally. ...this means cortisone will stay low when doing extended fasting. It helps checking blood sugars too 😊.

LitoGeorge ...there are days it is easy to fast...some not. Actually one hour or 2 doesn't make a difference. For autophagy it is important no calorie to be consumed...only water...coffe and tea...no sweetners. I find more important than the hours that pass into fast state the feeling wellness and not being stressed. Very important to break the fast when into a calm state of mind...not angry or tired... Hope this helps 😊

Thanks Alina - to finish my understanding of what you're passing on: you said it took four months to get rid of insulin resistance. At what point did you count your 4 months? After starting OMAD or doing the 48hr once a week regime? What metrics do you use to state that you are now free of insulin resistance? Do you measure by blood or urine ketones? If you had to go an 18:6 day or a 20:4 day now would you ketones be at 3+ in the morning? Some questions to gain a greater understanding of your experience in healing insulin resistance which is something I believe I have.

Ask yourself why/how a zero calorie, non digested carbohydrate could kick your body out of ketosis. Suppose you were in deep ketosis, and you went on a water and stevia fast? What would happen? Would your liver go apeshit creating glucose to stop you from passing out?

Clearly the results show that coffee plus fat does not knock you two out of ketosis and tends to lower BG. However it seems quite a stretch to state that this means that these manipulations do not break ones "fast". They are just plane not the same though i wish it were true. I do not in fact know (nor any expert) of any test that can be performed to determine if even black coffee metabolically breaks a fast in humans. Does anyone have such information? Dr Longo even will not really answer that question and Rhonda Patrick flat out states that even coffee breaks the circadian rhythm and a fast.

The real benefits of fasting is stimulating autophagy...but coffee and fat is a very small affect on fasting and I doubt you automatically stop all autophagy in your body as soon a a drop of coffee touches your tongue lol In the end you have to decide how you want to run your fast...what you feel comfortable with.... no sense in going into a complete dry fast if it always makes you sad and hungry and miserable right? You are after all trying to improve your life not make it miserable. ;-)

Scientific studies have shown that black coffee combined with Irish whiskey has a synergistic effect that can prevent strokes and even reverse the effects of a stroke if taken in time. The alcohol opens the blood vessels while the coffee increases blood flow and was found to be as good or better than any medications in preventing and treating stroke victims. The added benefit is that it won't break your fast or knock you out of Ketosis.

What I've always heard to be the best benefit of fasting is that it increases (and prolongs) the bodies release of HGH (human growth hormone), which is our natural youth hormone. It always increases at night because we're fasting as we sleep, but the first thing we put into our bodies halts our production of HGH. Ketones and insulin aren't really a factor in what breaks a fast in that case, your HGH production is. I doubt there is a method available to the public where you could easily determine if these fats (or even the coffee itself) breaks this fast, but if you can test these things it would give a much more accurate test result. I'd certainly love to know, because drinking nothing but water for 16 hrs a day would be tedious. Morning coffee with a little cream is what tides me over for the first 5 hrs after I wake up, but I'm not sure it's worth the daily fast if either the coffee or the fat reduces my HGH production. ):

Terrific experiment guys, I applaud your effort. 2 comments: 1. I don’t agree with the fundamental hypothesis that “not kicking you out of ketosis” and “not elevating glucose” equate to “not breaking a fast.” I think there’s more to it than that. I wonder what would happen if you ate one egg, and then checked measurements. If they were similar ketone and glucose findings, would you conclude an egg doesn’t break the fast? Not sure. 2. The low ketones and higher glucose numbers for the gentleman are indicative of chronic ketosis and adaptive glucose sparing, not insulin resistance. It’s actually a really good thing, and it will take exogenous ketones or MCT to really push up his ketone levels. Again, great work. Much respect for your efforts.

Theoretically, the protein in the egg would boost glucose and would throw a person out of a fasting state. But I agree, I think fasting 0 calorie probably has more nuanced benefits than keeping glucose down/steady, and ketones high. When you consume any type of calorie your body has to do work to process that energy and break it down. The body is a beautiful machine. If your trying to lose weight and using IF to do that then I probably wouldn't recommend the keto-coffee during a fasting period. At that point, I would think your body would use the fat your consuming instead of the fat you have stored to provide the bodies energy. It would be interesting to see over a period of time if a person lost more weight with water fasting vs keto-coffee fasting, as it were.
