Cauda Equina Syndrome
Cauda Equina Syndrome is a medical emergency, and it must be diagnosed quickly and treated urgently to avoid long term, serious complications. It occurs at the level of the roots causing lower motor neuron injury. The cauda equine is multiple nerve root within the lower end of the spinal canal (beginning at the level of L1). L1-S5 peripheral nerve roots are inside the lumbar canal. Cauda equina syndrome symptoms are usually bilateral and symmetrical. It will be a sudden onset of more back pain than radicular pain, and the sensation over the perianal area is affected. Cauda equina syndrome usually occurs due to a space occupying lesion such as disc herniation, tumor, fracture, epidural hemorrhage or an abscess. Central disc herniation is the most common cause of cauda equina syndrome. Disc herniation can be posterolateral disc herniation, central disc herniation, or foraminal disc herniation. The posterolateral disc herniation is the most common type of disc herniation. With posterolateral disc herniation, there will be a nerve root injury, and there will be changes in the sensory distribution, and there could also be motor and reflex changes. Posterolateral disc herniation usually affects the foot and the ankle. This type of disc herniation is usually treated conservatively initially. Posterolateral disc herniation usually causes unilateral leg pain and weakness with positive straight leg raising test. Central disc herniation will affect the cauda equina. In the cervical spine and in the thoracic spine, compression causes involvement of the spinal cord and an upper motor neuron lesion, but in the lower lumbar spine, compression causes injury to the nerve roots, and it causes a lower motor neuron lesion. In cauda equina syndrome, the patient will have back pain, buttock pain, saddle anesthesia, bilateral leg pain, weakness in the lower extremity, urinary retention followed by urinary incontinence, and bowel dysfunction. Saddle anesthesia occurs from involvement of S2-S5 nerve roots. These nerve roots provide sensory innervation to the anal area, perineum, and inner thigh. Reduced or absent sensation to pin prick in the perianal region from S2-S5 dermatomes. Decreased rectal tone or voluntary contraction. Red flags include lower back pain, sciatica (unilateral or bilateral), sudden sensory disturbances, bladder and bowel dysfunction, and variable lower extremity motor and sensory changes. Early diagnosis is challenging. The initial signal and symptoms usually are subtle. It is important to suspect cauda equina syndrome, and when you do, you should do digital rectal exam and test the perianal sensation. Get an emergency MRI. If you suspect cauda equina syndrome clinically, the MRI should be ordered and read urgently. The patient will probably need to be admitted for completion of the workup. If the patient has a pace maker, then you need to do CT myelogram. If you suspect it, get a study. Identify the source of compression in the lumbar spine. Cauda equina syndrome is a surgical emergency with maximum chance of recovery of the urinary and bowel function with adequate early surgery. The timing of the surgery decides the outcome. The bladder dysfunction symptoms are the least likely to improve, so you want to do the surgery early. You will do early decompression with removal of the disc or bony fragments that is compressing on the nerve roots (very important). The patient should be taken to surgery urgently to provide the best chance of symptoms resolution. Prognosis for cauda equina syndrome, there is improvement in pain and weakness. The result is less predictable for recovery of the urinary bladder and bowel function. If the patient presents more than 48 hours, there will be less than 40% of the patients that have improvement in bladder and bowel function. 60% of the patients will have chronic bowel and bladder dysfunction if surgery was delayed more than 48 hours.

    Demand an MRI if u haven’t already. I just had emergency spinal surgery for this condition after 3 ER visits back to back. By the 3rd ER visit my left leg had gone numb…sat in hospital for 3 days while it spread and when it did, they finally sprung into action with emergency surgery.

    This helped to answer a lot of questions my doctor never took the time to answer. Sadly, this is years after the fact. At least now I know why I’m suffering from urinary incontinence. Had to wait almost 5 months before my surgery because doctors kept screwing up paperwork, and now I can’t really walk. Pro tip: always take care of your body, especially your back, and especially after 30 yrs of age.

    I hope your doing well and remain strong. I also been experiencing some of the symptoms for the last couple of years now, it started when I lay down and exercise I would do the straight leg rise, and on me left leg I notice a pop every time, on my lower back. I also had light weight pain on my lower back, it feel like my muscles or bone is weak. Sorta pain. And in these last 2/3 moths I lightweight been experiencing leg weakness on my left leg, I step on leg foot n feel like I’m fall cuz my leg foot can’t support me. N been feeling my nerves. I had a back/spinal injury lifting heavy, n I’m thinking that the root cause n when it started. When home lay down for a cool while, couldn’t get back out for like 10 mins, my nerves felt pain n my back spine didn’t have the strength to Lift me up, I literally roll on my back n reach for the couch as support, I also had trouble getting up in the morning, this only happened when laying down. So I know it was really serious now, back then I didn’t think much of it, n like you said didn’t take care of my body, I’m only 25 now, still young for shit like this to be happening. But yes take care of especially your back. That’s one thing that cannot be replaced as a chiropractor told me, I will be making and appointment to get tested for this now that I know what it’s called n positive about my symptoms, I can finally stop visiting the chiropractor for the pain n more of for the problem n solution.

    @@lorrainegilmore9075 Tbh it took me almost about 3 or fours years for me to realize there was someone wrong with my back that wasn’t normal before. But I had symptoms for about 4/5 years, back problems, lightweight lower back pain. Over the years it’s been more noticeable n new symptoms are felt, in these last 3 months or so I been feeling my left leg get weak, back bothering, n others symptoms that hard to explain but it’s in the pelvic area. That kinda why I ended up here today I been researching cause for my symptoms n this is the match I ended up with so I now know the name for this cause. I plan to visit my usual chiropractor n let them know of my suspicion n get and request and MRI to confirm if this is the cause of for my backs problems. 😤I gotta be strong from here on out, my path only get worst from here on.

    I had classical symptoms of CES but doctors screwed up and surgery was done after 22 days. That left a profound impact on me. Thank god i recovered to a great extent after some years. Evrn though the recovery was not complete, i am able to lead a fully independent life

    I so wish you were my doctor or could find one like you. There is NOTHING normal in my spine. I have stenosis, scoliosis, spondylitis and sclerosis. I've had sciatica since 1979 when I fell roller skating. No dr. will help me with any of my problems. Both arms and right leg are severely affected along with other bodily functions. Last year I fell twice trying to get out of the tub and also fell on my back porch. I think that I may have fractured my coccyx, but the dr. doesn't even want to do an MRI on it. My pain is unbearable. He did give me a script to see a pain mgmt. dr. though. I am thinking about seeing a chiropractor who does acupuncture however. I am finding it very difficult to live like this. Thank you for all you do Doctor Ebraheim.

    Hope all is well. Perhaps try a different doctor if possible? Try other therapies like chiro etc is a good idea I feel! Just make sure u give them all the info regarding ur history

    JUST ME ... I too have broken my coccyx twice. The first time it happened during childbirth, Didn't know why so painful back there when I woke from delivery, nurse said she heard my tailbone break, and although not common, it does happen. Second time was roller skating...just like you! Whenever I've mentioned or read about Sciatica....it says how it comes and goes...but I think with some people like us...it's constant. I also have spondelolythesis, and severe sacroiliac joint pain. I also get shingles repeatedly on my coccyx area and the last time was Internal Shingles (shingles also attack internal organs). My legs are in constant nerve pain from it. I have been going to Pain Mngmnt. for several years. I seriously hope you find relief!!

    I diagnosed myself after being sent home from the ER with steroids and pain meds. The second ER trip a week later, I had to be sedated for an MRI bc I couldn’t lay down flat. They only took me seriously due to leaking urine when I vomited after pain meds😏My discharge paperwork listed possible cauda equina as a diagnosis and my surgeon couldn’t say if it was or not after performing a decompression, durotomy repair and fusion. I’m 6 months out and have had minimal lingering effects. My biggest problem is tightness in the hamstring and calf muscles, which I just learned through another google search is related to impingement in that level. Thank god for google and KZread or I’d never have answers 😂😂 Thank you for this simple but very in depth explanation of what’s going on 🙏🙏

    I'm 5 years since the onset of my CES. I'm trying to find yoga and stretching poses I can do without aggravating my pain. Unfortunately, CES doesn't give pain warnings like a broken or strained bone or joint, so the only way I know if I've aggravated my CES is when I wake up in screaming agony some hours later. Thank you for your video, it helps me to have something to send to friends and family to explain what I have.

    @@goodlifetony Unfortunately surgery was not an option for me. Apart from the lack of a physical injury, I presented absolutely classically for CES. It started with weakness and burning pain in my legs, pain and numbness in my saddle area. It progressed to loss of bladder and bowel control, with increasing neuropathic pain and lack of sensation and mobility from the waist down. At first I had drop foot but it ultimately became complete lack of leg strength. 5 years on I have excruciating pain, an inability to sit, stand or walk, and ongoing bladder and bowel issues. The best guess among my numerous specialists for what caused my CES is influenza B, which appears to have crossed my blood brain barrier, ran up and down my spinal cord for a while, (I have some other non CES related issues), and settled in to damage my Cauda Equina.

    @@revenantsage Unfortunately as my CES wasn't caused by an actual spinal cord injury there was nothing on which to actually operate. I have tried to go back to yoga a handful of times, but the pain ends up defeating me. Neuropathic pain is a constant weight on my back, and thus far nothing has worked or helped unfortunately.

    @@michellestratford9753 are they sure there isn't an injury? The reason I'm asking is I have something similar. The difference is I already had spinal fusion L4-S1 in 2019. In January 2021, I twisted sideways while sitting to grab something. I felt the bone rubbing in the area of my fusion, with severe pain. I've been complaining to my neurosurgeon since. He kept telling me the bone is fused and can't move. In August 2021 pain management did an MRI which showed I had a slipped disk in the area of the fusion. I went to physical therapy and he manipulated my legs in a way that aligned my back releasing the acute pain. My surgeon ordered a CT scan in October. That showed the bone is healed. I don't have saddle anesthesia. I have worsening bladder incontinence. My left foot inner side of my big toe, 2nd and 3rd toes are completely numb. There is an area of numbness rhat runs up my ankle to outer calf. My feet hurt so bad I can barely walk. My surgeon insists this is due to my L3-L4 disk herniation. I have a aching in that area. But not sharp pain. I don't have constant pain in L4-S1. But if I move wrong I can feel movement there. My pain management doctor spoke with my surgeon about the problem. Pain Management told me Tuesday he suspects CES, and asked me to ask Neurosurgery for a second opinion. They have no openings until July. My neurosurgeon retired. My pain management doctor said sometimes the injury is very hard to see until you are actually in there. I'm guessing my surgeon didn't want to deal with it because my 1st surgery was very bad. Is it possible you have a small injury or bone spur that they are missing? I have another MRI scheduled at the end of the month.

    wow, wow oh wow. September 2018 I was seen about my sciatia, low back pain, immobilized, severe pain, CRIPPLED! for over a year. not one test was carried out using a machine, pelvic floor dysfunction, pelvic floor dysfunction. I had an MRI behind the hospitals back on my right hip and not the spine! what the hell has the spine to do with it? nobody told me to and I was forced to diagnose myself. As of now, problems with passing true back passage and leaking, pins and needles numbness when pooping too long and too many times, 4 times yesterday alone and had a small food day, upper legs are bloober yet i train enough and my right inner quad thigh is losing contour, walking on egg shells with every movement I take so as not to pinch the nerve and be in hospital, why, seen in hospital after taken by ambulance with paralyses after lifting something in september 2019, september 2019, an mri is only booked for march 2020. Well at least I did not die and the damage is done, my god what part of the world am I living in? I am actually waking up some morning asking myself am I dead, is this actually real, they have destroyed my education, my brain, my wrist, my chest, and now my lower body! but I am a scammer they say with delusions of grandeur. so infuriated if I get diagnosed with this.

    Damm bro. I’m sorry to hear about this. How are you doing now? Hope all is well and you remain strong. I also feel like I had some of the symptoms, and went I do the leg lift lying down a hear a pop in my lower back. I had a horrible incident lifted heavy where I felt I injured my spine. I went home and started laying on the floor. And then I couldn’t get up. My back was in so much pain, my nerves restrain me from trying to lift my upper body up. I literally had to roll on my back and to get up, I also reach my arms out to grab a couch for support. I just rested for a few days. Never seen any doctor or anything. I didn’t think too much about it. I’m thinking of making an appointment to a chiropractor this week and tell them my thoughts and that I want to be tested for this to confirm if it is or not and to get treated for my back problems. I can only imagine what you must be going thru, I would not wish this on anyone. When I was in high school I would have dreams, so often that they became my nightmares. It was of me sitting in a wheel chair unable to move. Looking out tru those eyes, as I realize it was me. They occurred so often during my 10/11 grade. I always feel that those dream meant something and as I now fear they may have. My brain or body was somehow trying to tell me this long ago. And this is legit. Thats why I can relate a lot, n can only imagine how hard this must have been. Feeling like you lost hope, like the world was out to punish you in the most severe way possible. Like nothin matters anymore. And your in the worse position they could possibly be. That not even death phases you anymore. N it becomes clear that death and being paralyzed are not to different at all. Except when ones dead ones at peace’s, feel nothing or knows nothing. When you paralyzed your still immobilized, except you still feel, and suffered. You feel like your being punish to suffer in this life, and that this is your hell. Like I said I hope you still strong, and doing well I just want you to know your not alone. I’m soo certain I’m on the boat with you mate. I know my body. How I feel. I just want you to mainly know, your not alone my friend, unfortunately many have/ and are and will walk this path of pain and suffering, but what’s knew. Pain n suffering comes in many forms. But I know its just really sucks that it had to be like this. And if this is the path waiting ahead for me as well then I too will suffer along side you my comrade. Endured the hardships of life until our final moments. Just know I’m behind you supporting you in any decision/way. We’re already in the worst position what could phase us now, nothin.

    @@keephiscommandmentsandlive9860 alot of pain in my right leg , i could bearly walk and when i walked i was bent forward i couldn't stand straight ,numbness too also and also was suffering from passing pee ,i would have to force it out,as soon as i had my mri results and i told them what my symptoms where they operated me the very next day.     I feel like I am sitting on a lump kind of like the size and shape of a russet potato. I have had difficulty urinating since MVA 5 years ago. Now, my saddle area is numb like the result of a dentist shooting me up with novocaine. My legs get weaker every day. I see a new neurologist in two days. Wish me luck!

    Oh, so glad to hear from you. After years of this, I finally got a MRI with contrast dye. I have a tumor growing out of my sacrum spine. Get a MRI that is looking for something unusual. When I first started having this sensation, I thought it might be a cyst. I have had them before. I waited for that sinus to appear. Nothing happened. So I Googled “Why do I feel like I am sitting on balled up sock?” The choices were a. A cyst. B. A prolapsed bowel. C. A tumor. There was-no indication of the first two choices and the third wasn’t looked in to until a month or so ago. Even then, my new and improved neurologist thought I had extra fat in my nerve sheath or something. I do a pre-operative tele appointment with a neurosurgeon on Wednesday. In the meantime, this tumor has been wreaking havoc on my Cauda Equina, My legs are getting weaker, and it has gotten bigger. The radiologist said it was a lipoma, but according to my research, it may be a chardoma, a quite cancerous type of tumor. Unfortunately, we have to be our own best advocates. Get as much information as you can. Read what John Hopkins, The NIH, and various legitimate sources have to say before you go to Dr.Oz. I am not saying this to freak you out, just to help you in your quest for answers. Keep pushing. I had to change health insurances to get what I needed. After 3 1/2 years of fooling around with a HMO, I went to a different insurance and in less then 6 months, finally got an answer. I did 2 MRI's, one with contrast (expensive) and one spine neurosurgeon saw nothing bad, some little protrusion that couldn't cause the symptoms (shortness of breath, numbness, diarrhea, weak legs and weird gait [propioceptive]). I felt like my legs were unattached to the body, weak and stiff. Other drs told me is a spike in distress, that caused IBS (bowel disfunction), the weird gait, insomnia, etc. All CES symptom-like, but not medically justified they said. MY GAD is being treated after years of battling it with failing miserably self intents of deep breathing and positive thinking. The meds are kicking and I see that I was into a constant distress for decades. This amplifies any nervous symptom the neurosurgeon said, and that even an EMG would bring nothing bad. Somehow, I'm not sure, because my stress is much lesser, the numbness ''should'' have become lesser, right? Or, I'm dealing with the peripheral damage that huge general anxiety spike did to my body. I fear that's the answer now :( Just waiting for having money to get a EMG, but I feel like better and it's totally nonsense doing that NOW that symptoms are fading (except genital numbness). They shloud be done the first day/week but NO DOCTOR HEARD ME. Even friends weren't believing I had this syndrome or the incomplete version (CES-I). I talked about CESI with last neurosurgeon and he told me that it could be, or not. He also wasn't helpful and maybe he dismissed it because I came to consultation with some pre-diagnose but WTH, this is not something you just ''leave it to the doctor''. They can fail no matter how much experience thay have. Wish I could have money to do all necessary to rule out, instead of abandone myself to fate.... Hugs 💛 PS: the MRI should bring something like tumor? Or you have to ask for it? There wasn't anything important enough to cause worries, and I was left with this.

    @@a0yama1694 Still numbness, not sure what could be. No spinal compression. It came from other unknown condition, but I fear is peripheric damage done because not a single sign of recovery of feeling in the saddle area. Lots of stress may got something to do? Couldn't find any pelvic floor specialist to check that, but only one accepted one call and then trolled me (idiot person). I've seen many people are took by lunatics when afraid of CES, and there are a lot of them having CES-I (CES Incomplete) without doctors wanting to help them. I guess that begging for good luck is the only hope I have for us all.

    If the MRI results show Cauda Equina root impingement and "tortuosity" does it mean Cauda Equina Syndrome? Wondering why these results aren't treated as a medical emergency. Have a month wait to consult with surgeon. Thank You

    I was just diagnosed with CES. I demanded an MRI after going to the emergency room twice. By the time I went in the 3rd time I had saddle anesthesia. I was operated on the next day after being treated with heavy amounts of steroids. It’s been 5 weeks and I’m recovering.

    I believe this is what I have. My drs are not listening to what I'm telling them and want to blame it on other things. I know my body well. I can barely stand for more than 5 minutes due to the severe pain in my lower back radiatimg down my legs. I had a bad fall March 28th, but already had severe lumbar stenosis and fractured my L1 in 2016.

    I hurt my back a week ago tomorrow on a fun ride, I felt a sharp pain across my lower back, I’ve had L4L5 Microdiscectomy and decompression 1.5yrs ago and had already got herniation of L5S1 but not bad enough for surgery, my Gp sent me to Hosp for MRI and I was told because I have not got CES they wouldn’t operate and treat me just for the several impinged nerves. Today I have experienced my bowels go out of control with them opening 6 times in less than 3 hours and the urge to go all the time, should I be worried?

    Hi not sure if you reply or not? I have had a fall fractured my L2 in 3 places the hospital had a very big and long meeting to decide what to with my back. The hospita gave me a back brace i have to wear for 3 months. Every month a ct scan. With the fracture at the back gets very painful the pain in the groin abdomen area especially when bladder is full becomes unbearable After using bathroom I have relief of all systems. However I am told to walk sit only when i have the brace on. What is the cause of this all neurosurgeon said was immediate operation rods and fusion..is this a ecuada syndrome? Its been a month no.operations.

    Surgical decompression is urgent and must by as fast is possible. Every hour is important. Best results are until 6 hours. Your opinion about 48 hours is from year 2000 and during years was refuted.

    Cauda equina syndrome is unilateral and asymmetric anesthesia , the Conus medullaris is bilateral and symmetric anesthesia.

    What if you had a MRI in 2009 that showed disk protrusions at l4l5 and l5s1 as well as 2 protrusion in the c spine and 4 in the t spine with bladder, and bowel, symptoms, balance problems, severe debilitation neck, back, and all extremity pain (including penile) with weakness and falling, migraines that start in the neck and would instantly pass out with certain head movement and the onset was rapid. Now skip ahead to 2023 and after seeing dozens of doctors over the years you finally have radiculopathy diagnosis of l spine and both legs, c spine and both arms and are awaiting a new scan of the t spine and further testing. But the kicker is that NOTHING other than pain management and therapy attempts that ALL FAILED multiple times in the 14 + years and the CT, X Ray, And MRI scans from 2009 to present ALL showed deterioration of the disks, pedicels, lamina, and facets. On top of that you have had long term sensation and feeling loss of ALL extremities (including penile) Migraines have stopped but you now have a constant dull headache, vision, and hearing problems, trouble thinking, and memory problems that both get worse with pain and constantly loose balance, and constantly drop things, have trouble urination and emptying and have bowel control problems. After 14+ years. Now what?

    Has anyone had this but opted for no surgery and to try steroid injection first? Currently on month 3 after steroid injection and all numbness and most of pain has gone away. afraid it will come back though

    No, doctors here don't believe anything I say, and won't risk to say anything to know, so I'm at my own fate. They say IBS, stress, etc, not taking it seriously. Almost 4 months still numb in saddle. It feels horrible, super uncomfortable. I don't think I have true friends, they don't believe me is a real condition and that sh*t happens. Moneyless and tired. If this comes back worst, surely I'll be suicidal without any doubt, because I don't want to suffer this. Because not one single doctor take this seriously in argentina.

    Have acid trips and it will help the nerves reconnect. I have managed to undo about 90% of the damage having a trip every month or so.
