A stroke occurs when the blood supply to a certain part of the brain is reduced or interrupted. Without oxygen and nutrients from the blood, brain cells cannot function properly and eventually die.
There are 2 major types of strokes: ISCHEMIC stroke caused by a BLOCKED artery, and HEMORRHAGIC stroke caused by a RUPTURED artery.
Ischemic stroke happens when a blood clot OBSTRUCTS an artery. In some patients, the clot forms locally, inside the blood vessels that supply the brain. This occurs when fatty deposits in an artery, or cholesterol plaques, rupture and trigger blood clotting. In other cases, a clot may travel to the brain from elsewhere in the body. Most commonly, this happens in patients with atrial fibrillation, a heart condition in which the heart does not pump properly, blood stagnates in its chambers and this facilitates blood clotting. The clots may then pass into the bloodstream, get stuck in smaller arteries of the brain and block them.
Hemorrhagic stroke, on the other hand, occurs when an artery leaks or ruptures. This can result from high blood pressure, overuse of blood-thinners/anticoagulant drugs, or abnormal formations of blood vessels such as aneurysms and AVMs.
As a hemorrhage takes place, brain tissues located BEYOND the site of bleeding are deprived of blood supply. Bleeding also induces contraction of blood vessels, narrowing them and thus further limiting blood flow.
Stroke symptoms may include one or more of the following:
- Paralysis of muscles of the face, arms or legs: inability to smile, raise an arm, or difficulty walking.
- Slurred speech or inability to understand simple speech.
- Sudden and severe headache, vomiting, dizziness or reduced consciousness.
Cerebral stroke is a medical emergency and requires immediate attention. It is essential to determine if a stroke is ischemic or hemorrhagic before attempting treatment. This is because certain drugs used for treatment of ischemic strokes, such as blood thinners, may CRITICALLY aggravate bleeding in hemorrhagic strokes.
- For ischemic strokes, emergency treatment aims to restore blood flow by removing blood clots. Medication, such as aspirin and tissue plasminogen activator, TPA, are usually the first options. TPA may be given intravenously, or, in the case the symptoms have advanced, delivered directly to the brain via a catheter inserted through an artery at the groin. Blood clots may also be removed mechanically by a special device delivered through a catheter.
- Emergency treatment for hemorrhagic strokes, on the other hand, aims to stop bleeding, reduce blood pressure, and prevent vasospasm and seizures. These goals are usually achieved by a variety of drugs. If the bleeding is significant, surgery may be required to drain the blood and reduce intracranial pressure.
Preventive treatments for strokes include:
- Removal of cholesterol plaques in carotid arteries that supply the brain
- Widening of narrowed carotid arteries with a balloon, and sometimes, a stent. This is usually done with a catheter inserted at the groin.
- Various procedures to prevent rupturing of brain aneurysms, such as clipping and embolization.
- Removal or embolization of vascular malformations
- Bypassing the problematic artery

I had a stroke three weeks ago to the day. I’m 34 and the docs can’t say why it happened. Stress is the silent killer, I do smoke quite a lot also. My main worry is bhoe to get back my personality. I feel like I’m just going through the motions since I left hospital and I have three kids to provide for too. The thiirsday after I just woke up and the “lights came back on” it was overwhelming that time, and took me a day to come round to the fact that’s I have made some really poor life decisions in my time but I thank God to still be here. I survived ! 🙏

At my breaking point right now. My grandmother had a cerebral attack yesterday while sleeping, saw her going senseless and taking deep breaths. At first it was not diagnosed. Now the doctors have said she has clots in her brain and a surgery is required otherwise she'll not survive. She is a septuagenarian and we know she won't be able to survive the surgery. It's so much pain to lose a loved one but it pains even more to see them going through the pain during their last days. Please, please, please, express your love and respect to your near and dear ones everyday.

My father was a heavy drinker which end causing liver cirrhosis and in our way to hospital the symptoms of stroke occur at first we were clueless and when we reached he has lost consciousness. We were told that he had a stroke, one of the nerve in the brain was ruptured and the blood flow hadn't been stopping. We spent 5 days in hospital at last the doctor told us it was not possible to practice surgery since the liver already have cirrhosis so it may end up as failure. We got a release after 2 weeks he passed away in my arms. Life took a hard turn since then as I'm the older son and a student in middle of bachelor's degree responsibility kicks in. I'm still confused...

My 90-year old mother had a cerebral stroke a few months ago. Thank God only her speech was affected and she has recovered well. Our problem is that mom refuses to stop smoking. The family and her doctor have tried to warn her repeatedly that another stroke is likely to happen due to her high blood her pressure but she just doesn’t care. The family is divided; some say, “let her be, at least she’ll die happy”. Others say, “she needs to stop otherwise she might end up an invalid”. We take the cigarettes away but somehow someone provides her with some. To top it off, she refuses to take her meds. Mom says, “I’m not in pain. Why do I need to take those pills?”What a dilemma!

She can respond to commands by blinking. She is 71 and a diabetic. After a bad fall and having my head hit the ground, I had an MRI to see if I had a concussion. The doctor told me I didn't have a concussion but have had several strokes. As an octogenarian, no part of my my body still works very well including short term memory, but I can't pinpoint anything that tells me I have had a stroke.

My grandmother had a stroke two days ago, a day that also happened to be her 94th birthday. She ended up surviving but is completely bedridden and unable to speak. She is being brought to my parent's house tomorrow and is expected to pass within the next week. It was the one thing she always said she didn't want to happen. Whatever conscious force is still inside her I hope is at least enough to experience a handful of comforting moments surrounded by family playing her music and reading her letters written to her by her late husband 60 years ago. I hope the stoke at least spared enough of her to experience that comfort.
