    I would love that. Not in the best interest of the imaging centers or hospitals that run them though. DEXA is a tool but probably not as important as we are led on to believe.

    Dr Doug: I am 71, was very athletic all my life but did too much…afflicted with Spondy, knees, hips. I am 5 ft tall, weight 94 and have a scoliosis. On regular DEXA, -3.9. I desperately need a fusion at L4/5 (exhausted all other options) and of course no neurosurgeon wants to touch me. Want to go get REMS with Dr. Bush. Hoping for a different outcome. BTW, fell three times (stupid stuff) and never fractured.

    Hello. Surgery on osteoporotic bone is riskier. Sounds like a comprehensive plan is in need. Have you come to our masterclass? A REMS would be helpful too. There is one in Asheville if that’s any closer.

    Hi Doug. Thanks for talk on REMS I have been having them in Melbourne Australia. Over the last year my TScore has improved -1.8 neck of femor and -1.7 lumbar spine. But fragility score is deteriorating in nof 34.5 sept 23 to 36.8 dec 23 Now in yellow zone for my age 66 yrs But TScore improved slightly. My dilemma is I have recently fractured from a traumatic accident and pelvis fractured 8 weeks prior to echo but healed well. My CTX and PINP both increased with fracture healing. I wonder how that will impact my fragility score. ? Can you help me understand how I can improve fragility and how long will my bone,markers stay up and is this impacting my bone strength? No bone drugs. Supplements and weight training before and hopefully bounce back soon to start weights again Thanking you

    Hi there. You have great data to utilize. Let me help you see it how we see it. First, if you are recovering from a fracture and are off your bones the fragility score may worsen simply due to that but should improve when load is reapplied. If you have bone turnover markers consider using the ratio we use to see what direction they are heading. We use P1NP/(CTX/1000). You should get a number between 100 and 200. Higher is better. Those units are in ng/mL.

    Hi Todd, NTX is okay but CTX is easier for us as we are getting blood draws anyway. No one likes to test urine if they can avoid it!

    I have had an NTX urine done with a score of 94. Is that a high reabsorption rate? My lumber DEXA showed-3.5. Is NTX or CTX more accurate? That k you for your work. I found an Osteostrong near me!

    Why would an Endocrinologist refuse to do bone marker Tests. Her response was we don't use them. I asked for them to be done as part of my initial blood work. How do I get them done in Australia?

    Hi Doug..l'm a professional dancer..been diagnised with osteoporosis 4.5 t-score on my spine. I feel strong and healthy and l can't believe it. Also l won't take meds..Could you explain what bone microarquitecture is. And also what is bone quality independently of bone density? Thanks so much.

    Great questions! Density and quality make up strength together. They are both important but more goes into strength than density. Micro architecture describes the thickness and natural design the bone makes during remodeling. It is thin and sparse in osteoporosis and thick and abundant in peak bone mass. Hope that helps!

    Hello, That number is likely referring to your T-score. T-score is the statistical number used to identify osteoporosis. -2.1 is not technically osteoporosis and may represent concern depending on the situation.     I’m not aware of any REMS machines in the Southwest.TBS I don’t find particularly helpful.

    Hello! No, it doesn't and it shouldn't. DEXA is a tool and provides useful information but our bones are more than a repository for minerals.
