    I own an OsteoStrong center in St. George, Utah. We last week we hosted Sonohealth at our center and over 3 days they did 68 REMS scans! My members and others that did the scans were thrilled to gather this more comprehensive data. We will have Sonohealth back in 6 months. Thanks for this great video that is helping those that had the REMS scans fully understand the difference.

    I have a REMS scheduled next week here in AZ. So happy to find a local facility. New patient consult scheduled with a Naturopath that specializes in Osteoporosis next week as well. It seems like they follow very similar foundations as your practice for evaluating the root cause of the bone loss and addressing it first through lifestyle, exercise and supplements first before considering bone drugs. I have watched so many of your videos and implemented many of your recommendations. My Primary Care MD said he had never had these kinds of discussions with a patient before when I saw him 😂. He had no idea what I was talking about when I was asking for bone marker testing. At only 53 with a L-spine T score of -3.6 on Dexa I need to get on top of it now. Looking forward to a stronger future!

    Great video. We have REMS in the UK and it changed my life. We have a company in the UK called Osteoscan which are great. Turned out I had osteopenia and not osteoporosis. Loving your videos

    May I ask if you had it done at a private clinic? I have a very severe diagnosis at the age of 35, but doctors in the public sector do not treat it and they told me to return in 10 years, my private gynecologist saw my tests and wanted to intervene because he said that in 10 years it would be tragic , I imagine it should only be done in a private clinic, thank you

    “We worked on gut dis function” Is it that easy? I came off ppi and can barely eat. I think this is a big part of my overall problem.

    I just found somewhere that has REMS in my area; University of South Florida. I’m calling Monday to schedule an appointment for their bone program! I have been scared and frustrated. My Primary Care doesn’t know enough about Osteoporosis. My last DEXA they did NOT scan my spine, just hips!!!!! My doctor said he couldn’t do anything about it. Time to find someone I can trust to be by my side along this journey. Thank you for sharing your knowledge!!!!

    This is very good information. I was watching Dr Susan E Brown on REM but it was a 1 hour stream and I couldn't finish it. Thanks for condensing it and making it quite clear for the average person like me. This is so interesting to me because I thought I wanted to get a DEXA scan sometime this or next year but now...Only thing is that REM tech equipment is not available yet in my country. Hopefully one day it will come. Always appreciative of all these information. Ignorance is definitely not a bliss!

    Thank you so much for all your content - it has truly been life changing for me! I just returned from Black Mountain and Dr. Mike Lewen - the whole trip was fabulous. He is a gift to osteopenia patients! I learned that the REMS validated the dexa (which was not what I hoped) but it does support my need to fully address my bone loss. I am following all of your suggestions that I've found, and plan to re-do both the dexa and REMS in one year. I have met with my Rheumatologist this week, and she did suggest medications. We negotiated on me taking this one year to try to address it naturally, then we can discuss RX if it still decreases. I am 66 and in good health. I have always weight-trained, but apparently not vigorously enough. MANY thanks @Dr. Doug!

    After the initial shock and fear, turn that energy into fixing the problem...you won't fear what you find out you can do and work towards your goal! I had a horrifying 14% increase in my lumbar, so got to work on diet and nutrients and exercise and feel calm with this plan.

    Thank you for the clear explanation, although I doubt there will be a REMS scanner any time soon in my isolated part of the country. One thing that bothers me about fracture risk calculations is there's never enough information about _who_ is getting fractures. Are they sedentary? Do they drink and/or smoke? How much exercise do they get every week, and how much of that exercise is weight bearing or resistance exercise, and impact exercise? What do they eat, and what supplements are they on? What's their family bone history? What _caused_ the fracture? So many factors to consider. I don't want to live in fear, so I'll keep on keeping on with my current program to improve bone health, and hopefully it will pay off.

    Partially true. The FRAX equation does take into account some risk factors like smoking, alcohol, family history and more. It doesn't take into account other lifestyle habits like exercise, sleep, stress etc. Those are hard to objectify!

    Can you tell me where I can find a REMs machine in my town? How do I find out? My hospitals dexa diagnosed me with osteoporosis. I’m trying to reverse it and I’d like to have my diagnosis confirmed. My hospital sent a notification that their Dexa was not calibrated correctly….! !

    Unfortunately there aren't many. We are working with the company in Italy to determine if there are more but the three we know of are in NJ and two in NC. Dr. Bush and Dr. Lewin in Asheville. I don't technically have one but we work with Dr. Lewin to get our patients into his practice.

    A couple of years back, I watched an excellent discussion between two doctors - an American and a French doctor both of whom specialised in osteoporosis. I didn't know it at the time, but the French doctor was speaking about this method of screening because he showed this fragility measure chart and illustrated what this meant with a couple of case studies, and spoke about how clinicians are able to use the information to support treatment options. This sort of information would definitely help me make decisions - but I can't access a REMs.

    Goodness. I’ve hiked all my life and have a healthy lifestyle. I got the DEXA scan but not full body. It came out slight osteopenia. My doctor immediately sent a message and asked if I wanted to start medication. I don’t take or need any medication and the thought irked me. I passed on medication for what WHO says is slight bone loss. Personally I have no faith in WHO or the CDC. I’m a healthy 67 year old that works full time in finance. I do move fast and have literally flown to landings on stairways at home and work over the past decade. I appreciate your 2 videos I’ve seen so far. Thanks for the information.

    With slight osteopenia on DEXA at 65 I would not expect you would meet criteria for medication even in the traditional medical model!

    Same here…dr wanted to put me on big pharma meds asap. I refused. My dentist and oral surgeon who are friends told me to never take those meds. I’m doing everything I can with exercise, nutrition and supplement to try to reverse my diagnosis. I was also sent a letter that the dexa machine was not calibrated properly so I’m not sure I have a correct diagnosis…!

    The REMS Echolight seems to be more informative than DEXA, and the fragility score gives people more data and even some positive news. That may lead patients to reconsider taking medications, which could have a negative impact on medical practice revenue. I wonder if that contributes to REMS Echolight not being widely available. New technologies can be disruptive and face an uphill battle.

    I was just diagnosed with osteoporosis with DEXA. I refuse to take the drugs…they do not stop fractures and actually make fractures more imminent. More people should know that.

    Found us a very interesting video. Nobody ever has mentioned this or I haven’t read about it before. Would you please do some profiles on people with much worse T scores minus -4.2 in the spine and -3.8 in the hips. Doctors are only suggesting Bisphosnates. Long-standing G.I. problems and a super high sensitivity to any pharmaceuticals makes me highly concerned about even trying them. However, the doctors are basically saying you’re going to end up in a nursing home or die within the year of a hip fracture.

    Very informative. Can you provide the centers in the US that offer REMS?. My REMS score was quite a bit worse than my DEXA. I’m a bit skeptical of the results because everyone I know who had a REMS test the result was better than DEXA. My fragility score however was 28 and 33. So there’s some room for improvement there.

    I would love to have a REMS scan done. I am a -3 in both hips. My foot slipped in a hole, and I broke my ankle 4 yrs ago. I fell directly down on my right hip, on the cement, with no problem. It wasn’t even sore after that.

    Thank you so very much for all lectures, great knowledge. I live IN Florida, where would be possible to get REMS test. My T-score is 4.4 spine and 3.2 hip on DEXA. Dr want me start alendronate . Previously i was on forteo for 7 months . I don’t like to go on meds. I would like more testing to be sure.

    I'm going to find out if there's a REM in my area. I was told earlier this week that I have osteoporosis in my left arm. They want me to start Zometa infusion right away. Funny how I told them I will do my research first. I left a message next day that I have found great info on a better course of action. I've never received a call back to find out what my plan is. Carnivore lifestyle helps but it can be reversed with weight training, walking (I can't do intermittent walk/jog anymore because my back is messed up and I'm already 5" Shorter at 68). I started K2 (with mk7 or something like that) upped D3, already taking magnesium with zinc and D3. I've fallen in the past a lot but only broke ribs from what I landed on. I've met so many people that have reversed osteoporosis, refusing drugs (some started and had serious side effects).

    I started magnesium and K1& 2 with mk7, plus vitamin d3 and calcium, my dexa showed improvement in my spine and hip, but worse in my femur, Im still on the shelf with meds

    Well my t-score is terrible, and I've already fractured multiple times, but I'm not afraid to hike, bike, walk...I'm afraid to run, and I'm not doing exercise classes where I don't know what's coming (because I'm too stupid to say, "Nope can't do that," if I feel good, and /or don't want to always be figuring out how to modify,) but...obviously there's plenty I CAN do. But it would be interesting to do REMS for the sake of comparison.

    @@Dr_DougLucasreally? That’s good to hear. I just got dexamated and I’m being told to proceed with a level of caution that defies physical existence. I used to take brisk walks, so maybe it’s ok to just take walks. I know you can’t comment because I’m not there for you to examine, and the only reason my posting this comment is to draw attention to yours, which I truly believe a lot of ppl do not hear from the industry, which causes depression and a decision to just give up.     @@Ed_Mann Thanks for sharing. Knowing where the safety barrier is is very challenging. You're correct that most providers will swing to the conservative side to protect themselves and have very low tolerance for risk.

    I'm only 67 years old and in extremely good health, but my T scores are horrible at -3.5., and four-point at the L5. I was in an auto accident and my whole sacrum has large long screws and rods, so that's probably why the L5 is bad but the rest of my body is not that good either. I'm too young to be having this and I have no idea why I have it. I do not drink. I have moderate exercise every day and when it's real nice, I actually hike with my son. But I do bring walking sticks now, just because of the fear of uneven ground could make me fall. I took Tymlos two years but that's all

    Re sensitivity vs specificity. Is it correct that sensitivity= how good the test is at finding the desired info ( a good test doesn’t miss many cases of osteoporosis ) Specificity= how good the test is at avoiding false positives (a good test will not say you have something that you don’t)

    Correct, sensitivity is the ability of a test to identify a true positive and specificity a true negative.

    Just had a BMD assessment on DEXA with ability to do TBS, trabecular bone score. Dexa BMD not good (-4.25 in one spot the rest like -3.20s in spine) but TBS is in normal range for 50+ female post menopausal. BMD report says "high risk of fracture" and no mention of the TBS!

    When you give a risk value for hip fracture what part of the hip, i.e.., bones, are you referring to? Does this risk include the result of a fall or injury to that area? What about risk of a fracture to other bones such as skull or lower arms or legs? How do "stress fractures" figure into all this?

    Hi there. There are only certain bones and parts of bones that are well studies. Imaging will generally report on femoral neck (true hip fracture) and spine. But, osteoporosis is a full body disease so there is increased fracture risk globally that is assumed.

    Dexa isn't the gold standard if it isn't the best. This battle between the screeners is a fine example of what is wrong with "healthcare" in the U.S. It's not about what is best for the patient. It's a bureaucracy, i.e., the Healthcare Industrial Complex, with the sole goal of lining the pockets of the "right" people.

    I haven't yet finished listening to the presentation yet but wonder what other tests in addition to the Dexa I should request? I want to know more about why I am losing bone and what the quality is in addition to the density. My doc isn't particularly helpful as she keeps pushing meds, which I am resistant to. I'm 73 with osteopenia in spine and osteoporosis in hips. I exercise regularly, including walking and wt. training and try to eat well. Guess we'll see if my wt-bearing activity shows an improvement this May....

    I just saw an endocrinologist today for osteoporosis. He had no idea what an echo light rems was and said it was not FDA approved? Is that correct?

    I am convinced that REMS is more accurate than DEXA because it looks at both bone density and strength, however there are only 1 or 2 clinics that offer this, and they are both in North Carolina! I don't understand that. How can we make this more available to everyone? Most doctors are not even familiar with REMS, and it takes forever to make changes in the medical field. I am in CA and considering making a trip to North Carolina for this, but I do not know if anyone out here will understand the results. Very frustrating!

    A REMS scan saved me from starting bisphosphonate treatment. I am 50 years old, 2 years ago I entered menopause and a year ago I had a DEXA T score of -3.6 in lombar spine and -1.8 for hip. The endocrinologist has been pressuring me for some time to start treatment with bisphosphonates, but after watching this video I learned about REMS for the first time. Fortunately, I found one in my city. The new scan at the REMS machine showed a T-score of -1.8! I don't even have osteoporosis. Have you encountered such score differences in your practice? The endocrinologist still asks me to do a new DEXA (on a different machine than the one that gave me a score of -3.6) to confirm this last result, because unfortunately in my country REMS is not included in the medical guide as a method official / acceptable scan. Therefore, I will do a new DEXA in a few days. I hope to receive a result as close as possible to REMS. Thank you for all the information work you do! For me it meant a huge difference!

    Wentfor a REMS this morning. Meeting with your team tomorrow. In three years went from barely osteopenic to barely osteoporotic. Eat well, exercise- what? Stepped up my game, now hoping for good results and better information. Question… Why would DEXA say my femur is worst, and REMS say my spine is worse?

    Very excited to learn about this new protocol for evaluating bones! But I just spent the last 45 minutes trying to find a LOCATION where this is done. Found nothing.     I have been trying to find a location in or near California, but I only come up with Central Carolina Orthopaedic in Sanford, North Carolina and StrengthX in Asheville, North Carolina. Why those are the only 2 places in the entire U.S., I don't know, and I find it very frustrating as well. I have even considered making a trip out there just for this. Hopefully you are close enough to take advantage of it.

    Everything on my dexa stated osteopenia 2 years ago, I have just completed another dexa and numbers for spine are now normal, gone up 4.5%, my hip has gone up 0.5% but still osteopenic and my femoral neck is osteoporosis. I am not on meds, Im wondering if the magnesium, Vitamin K1&2 with MK7 , vitamin D and calcium helped? Not sure why the femoral neck number was worse.

    Lots of GP's in the UK are still worried about it, due to lack of training! The increased risk is low: there are around 5 extra cases of breast cancer in every 1,000 women who take combined HRT for 5 years. The risk increases the longer you take it, and the older you are. It falls again after you stop taking it. There is little or no increase in the risk of breast cancer from oestrogen-only HRT

    Do you have any information that might help me understand why my bones might hurt so much. They told me my ribs look like they belong to a ghost. This has progressed past the time of my hyperparathyroidism surgery. I had surgery in January of 2021. My front of my hips are very sensitive too.

    Yes, TBS is generally better BUT I don't find it tremendously useful. We are looking for more REMS machines but right now can only find the 3 listed on the Echolite site. One of them happens to be in Asheville, NC where we are.

    I live in the Uk. I have been diagnosed with end stage osteoporosis. I had dexa scans and it said I was 110% of some of my age. The orthopaedic surgeon said the results are rubbish. I have now broken 9 vertebrae, and pelvis. In the last three years. I have broken bones all my life. Zoleadronic acid did not work to stop Fractures. I am being put onto Evenity. Is this going to improve things.

    Yes. It means both. Fracture risk in general we are referring to fragility fracture which would include spontaneous fractures or those from minimal impact.

    Can changing machines be that different? I went up and down .8 points going once to a different machine because it was closer to me. What is the error rate in doing that. It was same hospital but different location.

    There is variation between machines but how much is not clear. There are also differences in operator and technique. If there is question I recommend my patients repeat on the same machine. Eliminate as many variables as possible.

    Another really informative video. Thank you. You mentioned Dexa results can differ from 'machine to machine'. I've read this before. I've also been told that what's meant be 'machine to machine' is not the actual machine but the make of machine. In other words, you need to be tested on the same make of machine.     Hi, Probably not going to get a clear answer on that. Each machine has to be calibrated and maintained. I don't know what the rules are on this but I wouldn't be surprised if they weren't followed closely. For the most accurate reading I recommend my patients go to the same machine if possible!

    I was diagnosed with osteoporosis at age 66. I’m 67 now. dr wanted to put me on big pharma meds asap. I refused. My dentist and oral surgeon who are friends told me to never take those meds. I’m doing everything I can with exercise, nutrition and supplement to try to reverse my diagnosis. I was also sent a letter that the dexa machine was not calibrated properly so I’m not sure I have a correct diagnosis…! I would like to be tested with this new ultrasound machine…where is it available?can you help here?

    Depends on the goals. IGF-1 products like sermorelin/CJC/IPA are great, AOD-9604 for those looking to optimize muscle gains and limit fat gains, BPC-157/KPV for gut, neuro peptides etc etc etc.

    Thank you for this information. I had a REMS scan at the beginning of May and have the chance to do another in September. Do you think that would be valuable or too soon? The scan in May showed a worse score than Dexa but my fragility score was in the normal range.

    I don't recommend them to my patients less than 12 months apart. I have seen improvement in as little as 6 but it could negatively impact an otherwise good course by showing worsening bone that is just not mineralized yet.

    Why is it that every time osteoporosis comes up doctors only talk about fracture risk? As if fracture risk would be the only issue. They look at a dexa-scan, which shows quantity, not quality, and they let the pharma-industry push them into prescribing pills that make your bone old and brittle, and that ruin your eyes. If you have been in an accident, like I have, and if you have a hernia as a result of it, you want to know about height loss, disc pressure etc., the kind of stuff that shows up on MRI. A dexa-scan is flawed in many ways, but it's cheap and the insurers are pushing it. A vQCT scan which can distinguish between the cortical and trabecular bone should be the gold standard.

    DEXA is flawed for sure but your asking different questions. Soft tissue damage from disc injury is diagnosed by MRI. A quantitative CT tells us a lot about bones but the radiation dose is too high to get regularly. It's a challenging space for sure.

    @@Dr_DougLucas OK. I checked the radiation dose of QCT, and that is around 200 microsievert. But using a photon-counting CT scanner that dose can be lowered further by 30% while doubling the precision. 140 microsievert is the equivalent of 10 dexa-scans. Not negligible, but not dramatic either.

    I was being pushed to start IVs due to my Dexa. The endo asked if I had any fractures. Def scared me. Had a Rems and I have Strong Bones!!!!!! What a scam they’ve got going with Dexa… cui bono?

    My gyn would not write an rx for REMS even though it is self pay. She did offer to write an rx for drugs. I got an rx elsewhere and will be going in March.
