    I feel so lucky I watched this video before starting my IVF. I found a doctor who is recommending day-3 no PGT-A testing for me. I had several consults with other doctors who only perform day 5 with PGT-A testing FET. I agree with Dr. Gleicher 100%. Thank you for this invaluable information!

    I was not informed of this option when I did my IVF when I was 35. The clinic only mentioned about doing transfer on Day 5. The embryologist updated us that embryos were doing great up until day 4, then on Day 5, he coldly called to inform us that none of the embryos all disintegrated and did not make it to DAY 5. It was so devastating.

    This is so extremely helpful. And I am just so thankful for you sharing. I have felt very stressed about what to do. We have four donated day 3 embryos. They're proven, our donors had their 2 daughters and no failed transfers. I feel confident in the embryos however I don't have so many embryos we can grow to blast and feel confident we will end up with a healthy pregnancy. Now I have the right information moving forward. Thank you.

    This video doesn’t mention blastocyst transfers due to PGD testing (to prevent transfer of embryos with the affected genetic mutation). More cells can be biopsied from a blastocyst compared to day 3 embryo. In addition, cells biopsied from a blastocyst are taken from the trophectoderm, which gives rise to the placenta whereas the lineage of the cell(s) biopsied from day 3 embryo is unknown.

    Same in France in my centre... well here in France though, we have social security so it's not a matter of money not to have transfer because all embryos failed to reached day 5 but still, feels bad... Thanks for this video

    Thanks for the clarification. Wish we saw the video earlier and saved the money. We had a day 5 transfer - embryo reached early blastocyst was good quality says the consultant but it we lost it 3 days before we reach the 2 week wait :-(

    What age though statistically you think that relevant to use 3 day embryos? Or how many eggs retrieved or fertilized is considered a good number for blastocyst instead of 3 day?

    Hello Dr Done my frozen eggs embryos on 5/11/20 which r blastocyst 6day Type A&B I’m 48 age taking estradiol 2mg QID plus injection . Dr check my outline of uterus is good . Had 2 pt came out negative done today blood wk came out negative on 2 wks didn’t do my hcg level . Dr recommend to wait 4 days to do my hcg level then we go from there. My question what went wrong if it’s frozen donor embryos which I understand it’s high rate 4 pregnancy . Can u give me advice thank u .

    Hello doctor... I recently had Ivf with donor eggs but it failed. Embryos transferred were on day 5 grade 1. I have high nk cells does this effects??? I took intralipid with barglob for 6 months. Do I need to go for donor embryo???

    Hi Tara, that's a great question. We would say yes, since culturing embryos to blastocyst stage end up reducing the number of embryos available for freezing as well!

I am 40yrs I was told my egg were not mature on day o so they did on day one and they were fertilized today is gay 4 I am told they did not reach blastocyst. What can I do? Do I ask them to freeze it and get me ready for transfer anyways or it is too late?

    I have come across this video a little too late unfortunately. I had 5 embryos fertilise and all were progressing well on day 3. Day 5 only one was a poor quality blastocyst that was transferred, the rest were morulas. Unfortunately the clinic said they would freeze morulas and if they didn’t turn to blastocyst on day 6 they were no good and wouldn’t make a baby. Day 6 one was a very early blastocyst and not good enough for freezing. My cycle failed and I had none to freeze. I wish I had a 3 day transfer.

    Makes sense. 5 of my friends had ET at Day 3 and all had beautiful healthy babies. Two of them tried again and had zero embryos when asked to wait till Day 5.

    I did not know that embroyos stay alive longer in the uterus versus the lab dish? Why would that effect it? I was told fresh transfer success a 42 year old is 6 percent? I held out any better hope on FET. So I guess I can disregard better chances with FET? :( Wow I am stunned My amh is .745 10 follicles on day 3 62 estradiol 7.8 FHS What do you think is my percent success rate?

    You can freeze earlier. Day 1 frozen have the highest survival change after thawing cause they’re the most stable. There’s not necessarily a better chance with FET. Btw your number look good for your age! Remember to not PGTA test them (check out his other videos).

    I have the same curiosity about what's happening to the embryos if transfered on d3. However, after seeing this video I'm set on going ahead the d3 transfer. I don't want to put any extra stress on the embryos with refreezing and thawing. It's worth the try, this is how they used to do it!

    Elisa Marie I am feeling the same way - after two ivf stim cycles, I responded well to the hormones, creating at least 12 and 13 embryos that fertilized. By Day 5, only 3 and then 1 had made it. They tested them each time and they all cane back abnormal. Not once did my Dr mention DHEA, Acai Berry, antioxidants, etc. I am so frustrated! I’m only 33! Seems like egg quality is an issue and on the first cycle failure my drill already coldly mentioned donor eggs...are you kidding me!? 🙄 This Dr has been so helpful just reading about their research in NY. I’ve bought supplements and just started taking them! I want my androgen levels tested ASAP. 🙏🏼

    Hi, Iam a poor responseder in my first ivf cycle which my doctor chancellor it and started again with another protocol but again only 2 eggs I got , but both fertize and one embryo stop growing after 4 days and other reaches on day 5 blasting stage and freeze, Now my doctor wants to do hystoscopy because they find polyp in uterus and and after that they want me to use other ivf cycle start and do embryos day3 transfer instead of using frozen embryos. I don’t know what I do

    I feel a bit cheated after watching this. My day 5 transfer was cancelled due to the cells not developing fast enough and a day 3 transfer was not even considered.

    I just had my first IVF done. Two embryos retrieved that were grade 2 in day 3 (9 cells or more), pretty good, but the lab had them until day 7 and then we got news that blastocysts were developmentally arrested. So I had none! Now my dc gives me an option to freeze on day 3 instead (I need to finish up 3 IVF cycles as a package). And I asked her about it before IVF too... now I lost two embryos:( and costed me $20k for this cycle! gosh this video is 3 years ago...the best method is to design a protocol for individual patients. In my case I need a protocol for me who are DOR instead of same protocol as “normal” ones

    @@mariaante5708 I actually did total of 4 cycles. The very first cycle I mentioned above was the best one. I didnt have any mature ones from the other 3 cycles. So my chance was really 0%. The doctor said my situation was below average of the same age. So everyone can have different results. Don’t get discouraged! Good luck!

    Because you can compare the outcome - meaning the life birth rate. Compare how many day 3 make it to life birth to how many day 5 make it and the add the ones that didn’t make to day 5 to the day 5 overall outcome. It’s called statistic. Course you can measure it.

    Success rate depends on a lot of factors beyond the grade of the embryos. For example, how is the female patient's endometrium? Does she have autoimmune conditions? If so, are they treated proactively? The grading method itself is also different from center to center. There's no easy answer.

    I was reading one paper according to which the majority (about 80%) of late day 5 embryos have aneuploidy which is the cause of poor development from day 3 to day 5. Eventually these aneuploid embryos will arrest and die. But still, out of these slow day 5 embryos there are still about 15-20% (depending on age) that are good and will lead to pregnancy. That's why some will let the slow ones grow until day 6 or 7 to see if they reach blastocyst stage and then do biopsy and PGS.

    @@yangjianhunt sorry this is very misleading. An embryo in general isn’t meant to grow in a Petri dish in a lab. That’s why some of them won’t survive. Life birth rate is higher overall when you transfer on day 3. Doesn’t mean that the embryo is chromosomally abnormal. It’s too early a stage to know because embryos can fix themselves.

    Dr. Gleicher has another video about it. He usually recommends fresh day-3 transfer if you have limited number of embryos. PGT-A testing is never performed on day 3 embryos, embryo needs to be 5 days to do the testing.
