Did nobody in the writing room told D&D that they were making a joke battle script? Hell, Benioff was know for already making the defenders deploy an army outside a forstress wall in Troy. Martin wrote Blackwater and while he made the Lannisters made small sorties outside the walls the majority of their forces were inside King´s Landing walls. An in season 4 we have the Night´s Watch deploying their men in the Wall and behind Castle Black walls.
