The inquest into the death of Aoife Johnston at University Hospital Limerick has recorded a verdict of death by medical misadventure.
The 16-year-old died with sepsis in December 2022.
Aoife's family have welcomed the verdict but say they have to continue to live without her.
The inquest had heard distressing evidence of what happened in the emergency department, described as a death trap, dangerous and dysfunctional.
