One thing I forgot to mention is that the test for bile acids in your bloodstream can take 4-7 days to return, so if you are presenting heavy with symptoms at the end of your pregnancy, your provider might recommend an induction for suspected cholestasis versus waiting for the results.
In today's video we discuss the function of the liver and the most common pregnancy-related liver condition: cholestasis. We'll review risk factors, symptoms, diagnosis, issues and treatment for intrahepatic cholestasis of pregnancy.

    I had this with my third baby. I had to take medication for it and deliver him at 37.3 weeks. I also had gestational diabetes with my son. He is healthy and 9 months old.

    Me too had it with my second son monthly blood draws medication and I had to deliver a month early I also had way to much water and high blood pressure was not fun lol but all worth it

    Thank you so much for talking about this! I had no idea about it while I was pregnant until I came across an Infograph that talked about the exact symptoms I was having. I was so itchy I couldn’t sleep and I could hardly function throughout the day due to the itching. I was induced at 39 weeks (2 days after I had testing done) due to my liver enzymes being scary high. I think it’s important to talk about this more often and bring awareness to it for pregnant women.

    THANK YOU!!!! I had never heard of Cholestasis until I had it and then I had a terrible time getting diagnosed. None of the doctors in my area have treated it. It's so important to get this information out for other Mamas who might be in the same situation 🖤🖤🖤🖤

    Thank you so much for posting! I'm currently trying her for Baby #7. I have never had issues with ICP, but this will be my first pregnancy AFTER 35 and also, my last sonogram showed a gallstone. It hasn't caused me any issues and had I not had the sono I wouldn't have even known I had it. So I still have my gallbladder. But because of the stone and my age I'm really worried about getting ICP with my next pregnancy. So I am SO glad you made this video! It makes me feel so much better!

    Thank you for bringing recognition to this awful thing!! All three of my babies are Itchy Babes. I was blessed to have an amazing midwife that delivers in-hospital and helped me through ICP! I just had my gallbladder removed after my third baby, because I developed 6 gallstones! They weren't present after my first two babies, but sure were there during my third pregnancy.

    Thanks for making this video. I had ICP twice and the first time even with severely elevated liver enzymes I got sent home by the L&D ER and told it was just morning sickness at 34 weeks. I came back 2 weeks later and still got dismissed. Thankfully i had an emergency induction at 37 weeks when my OB figured it out. My last pregnancy a few months ago i got induced at 36 weeks because my numbers still went up even with ursodiol. I also didn’t have severe itching last time but this time it was crazy and started at 20 weeks.

    It's rarely that I leave a comment but I wanted to share my experience for those who may be in need of advice.. you see I'm from a north African country (Tunisia) and this condition is so rare that when I had it during my third pregnancy the doctors didn't even have a clue.. I always suffered an extreme case of hyperemesis gravidarum in all my pregnancies (had to be hospitalized during my first trimester) So everyone assumed it was the same case with my third one.. but it was both, it was hell. The itching, the sickness and above all my potassium levels were getting low. I was 17 weeks then .. I was nearly dead and when it was obvious that it may be something else.. they ran the tests and they confirmed cholestasis .. I lost the baby because it was so early on. Never neglect the itching, plz I hope everyone have a safe pregnancy and take care of yourself plz.

    I’m 37w pregnant and I’ve been recently tested for hepatic function due to terrible itch on the belly. I was pretty sure it was just my seasonal allergy, since I experienced the itching before and indeed my liver function was fine thankfully

    THANK YOU! I’m a 2x ICP mama. I’m going to begetting my gallbladder removed in the next few months, I have gallbladder disease. My daughter was delivered at 37 weeks and she was tiny for her GA. My placenta had to be evaluated after her birth because it was decaying. It was CRAZY. So glad I’m done having babies!

    you always have perfect timing. i watched you during my first pregnancy and you talked about all of the things i needed to know then and now i’m in my third trimester of my second pregnancy and i just had to get blood drawn to see if i have cholestasis!

    Thank you so much! This is my 4th baby only 16weeks and still trying to get diagnosed waiting on lab results. Very itchy everywhere especially at night. I have been researching and the word still born is scary to see pop up with chloestasis.

    I had this during all 3 of my pregnancies with the intense itching and nausea for my first baby. I got my gallbladder out after my first because I ended up with big gall stones so I had less itching with my other 2. My doctors induced me at 36 weeks, 36 weeks and 1 day, and 37 weeks and 1 day. I had to take meds 3 times a day until birth and had non stress tests and ultrasounds every week. It was not fun but we got 3 beautiful babies out of it so it was worth it! ❤️ no one in my family had it though.

    @@ilu6281 I definitely experienced less itching after getting my gallbladder out. I’d ask your doctor about it if you’re worried though. I always got it around 30 weeks but I think you can get it super early. I had to take medicine 3 times a day and monitor my babies super well and get induced early but they were all born happy and healthy! It was pretty scary the first time I got it but once I started to understand what I needed to do and what my doctors could do to help, I felt much more comfortable!

    Thanks for covering this. This is my 4th pregnancy with cholestasis. After my second child, I had gall stones and then my gall bladder removed. There's definitely a hormone factor for me. I experienced delayed puberty resulting in first period not arriving until I was 16. Also, lack of body hair, not as severe as my grandmother but like leg hair, armpit hair, pubic hair, we don't really get much of it. Late puberty and obstetric cholestasis are common in my family. I had strange side effects from the pill in my early 20's and dropped it. I had ICP with my first 3 children in my 20's. Then at 40 we lost our 12 yo to liver cancer. We decided to have another baby but my husband has secondary infertility. We ended up going for IVF but during my treatment, my body overreacted to relatively low hormone treatments. The worst part being Ovarian Hyperstimulation Syndrome. But we were successfully and I'm currently 34 weeks pregnant with twins. But once again, I have obstetric cholestasis. I'm entirely convinced my body struggles with heavy hormone fluctuations and that's what triggers cholestasis for me.

    My doctor tried to Diagnose me with cholestasis, i had severe itching on my hands and feet, but it wasn't the palms or the soles! Just the entire area around them. She immediately said that it was this, didn't even look at my hands and feet even though there were plenty of bumps. It eventually went away after A few weeks but I was miserable. And I felt scared and lost because my doctor had me doing all types of things and talking about a high risk of baby coming early or doing very early delivery. I was freshly in the 20 week range. I've had Exzema for years and I personally think thats what it was cause all my tests came back fine. Baby delivered full term as well and is perfectly healthy except he also has very bad exzema. But still, having symptoms very close to this was surreal to say the least

    I had this with my first daughter. My belly, hands and lower legs were SO itchy! I asked my OB if we could test for it. He tapped me on the leg and obnoxiously assured me he was certain that wasn't the case but we could test to make me feel better. A week later, I got a call saying my levels were elevated and they needed to induce me. I asked when to come in and they said, "How soon can you get here?" My OB was on vacation, thankfully, cause I was blessed with the best OB in their system for delivery! I had never heard of this until I looked up that symptom. I'm glad I was strong enough to ask.

    @LetyourLightShine good! I hope you don't have it. Until then, put a thick layer of lotion on your hands, legs/feet and belly before bed. That should take away some of the itchiness for a while.

    I had this twice! Can't wait to watch! Also had a similar feeling that ended up diagnosing me with mono induced hepatitis and getting my gallbladder out (never had stones, but failed the HIDA scan)! My first OB wrote me off about...but between pregnancy 1 and 2, I saw this on the Baby Story. Brought it up to a new OB with pregnancy 2 and sure enough it was cholestasis. I scratched the bottom of my feet with the pampered chef stone scraper to itch them. It was miserable.

    Wow…. Pretty sure I had this in my first pregnancy. I’m watching this like “yup had that, yup that happened, uh huh… then omg I had this and had no idea!” I was extremely sick up until 16 weeks. I did have the itching in the last several weeks, but it wasn’t severe enough that I brought it up to my midwife. It was only annoying at night when I was trying to sleep but I completely forgot about it during the day. I had no idea it was a symptom of anything, I just assumed it was another weird pregnancy related dry skin thing. When my water broke, it was filled with meconium - luckily my baby did not aspirate it. He was born on his due date, but had to be in the NICU for over a week due to respiratory distress syndrome and bradycardia. His birth was traumatic so when I questioned why he was having so much trouble the doctors blamed it all on the birth. Shortly after he was born I had to get my gallbladder removed due to gallstones. My son is now 2.5 and thriving and I’m much happier without my gallbladder and have since had another way less eventful pregnancy and birth. But wow, you put all the puzzle pieces together for me! I’m realizing now things could have gone much worse.

    I had ICP with my first and had my gallbladder out shortly after because I was having frequent gallbladder attacks. Thankfully I never got it with my second! We're currently planning baby number 3 so we'll see if I can beat the odds twice in a row!

    My first cholestasis test came back normal. After 6 more weeks of whole body itching I couldn't treat because I react poorly to antihistamines, they tested me again. My overall levels were in normal range, but because one specific level was a little high, my symptoms were so hard, and I was 39+6 at that point, they decided to diagnose and induce me that afternoon and baby was born exactly on his due date. I also did not know until my kid was over a year old that I'm autistic. Do you know if neurodiversity has any effect on cholestasis risk? I had none of the risk factors you brought up and also none of the potential complications you brought up. I know risk is a matter of probability and improbable things are bound to happen to somebody, but I'm curious if any research has been done on my question.

    39 weeks tomorrow and have been testing for cholestasis for the past two month due to mild itchiness. Levels went from 8, 15, 11, 14. Doctors don’t know whether to induce or let me go to term. They said my levels are inconclusive. They recognize that over 10 is a positive but have not diagnosed me and said my results are inconclusive. They said 40 is usually the number to vegan being concerned. Not sure what to do at this point. I’m on the medication and have no itching so they will reevaluate again monday

    Thank you so much! I’m 10 weeks pregnant with my fifth baby, and have had confirmed cholestasis with every other one. Itching and upper stomach pain, as well as exhaustion started a few days ago, but my doctor was hesitant to test because it’s so early. I’m fairly certain that this is the same itching and discomfort I have been through four times already. Has anyone ever heard of it having such an early onset after so many consecutive cholestasis pregnancies? Also curious if anyone saw relief in symptoms with dietary changes? I’ve noticed the less fat I eat, the less discomfort I experience. Currently trying to stick with mostly fruits, veggies, whole grains, and lean meats and dairy.

    I had issues with my liver and bile the month before I got pregnant I was hospitalized due to extremely high liver enzymes and then found out we were having a baby I’ve had itchy hands later in my first trimester as well as my 2nd trimester should I be concerned it isn’t everyday though ?

    Would this make the baby hypoglycemic? I had a baby in nicu that had hypoglycemia when the mother had IHC. I drew the conclusion that if the cholestasis was possibly blocking the pancreatic duct, maybe the mother would be hyperglycemia from insulin release being blocked & maybe the baby was used to needing to produce more insulin prior to birth & needed to catch up post birth. But I wasn’t sure. Any thoughts?

    My daughter got diagnosed with ICP with her pregnancy. After giving birth 1 month ago she is still itching and has inflammation on both tights & ankles. Is there anything that she could take over the counter to relieve the pain. Thank you

    I was really confused by my office at the end of my pregnancy. I am 99% sure I had coliostasis. I had so many of these symptoms and they did run a basic blood panel. Everything came back fine and when I asked about further testing they told me they could order a liver panel but wouldn’t because I already over 39 weeks and they figured I would deliver soon. I did end up delivering at 40w3d but so much could have happened in those days in between. Is it common for Drs to overlook things if you’re that far along in pregnancy or is this something I should advocate more for next time, should it happen again?

    I had this with my twin pregnancy. Started around...30 weeks I think? It was miserable. My liver bile was super high for a year afterwards too. I was induced at 36w because of this. Gallbladder out after my first (2.5 months post partum).

    It's sooo important to talk about this. I was in to see the OB for my routine appointment. I have excoriation (an anxiety disorder), so the severe itching was awful and I had open wounds all over my legs/feet. He asked me about them, I told him that I have excoriation and that I think I must have mosquito bites (um there were no visible bites). Fast forward to that night, I'm absolutely miserable and searching why I'm itching. I was dumbfounded when I found Cholestasis. I immediately messaged my provider and got in the next day for the blood tests. And yes...I had it. From then on I was monitored in the office 2-3x per week. The only way I could sleep was with cold wash clothes and and a fan on my feet and hands (basically numbing them). SUPER scary when my dad died of liver cancer. :(

    If you're itchy anywhere in pregnancy, please ask to be tested. My doctor didn't test me because my hands and feet weren't itchy. It was only my stomach that was itchy. I didn't get a diagnosis until 34-35 weeks when I found a doctor who was willing to test.

    Yes! So many women experience cholestasis without itchy hands and feet! They may not be as common as those with that symptom, but the standard advice that it will be hands and feet I’m sure has caused many women to never get diagnosed!

    I think I have it instarted with itching hands feet and now the belly and breast omg and my ob comes back in 2 weeks I'm so frustrated don't know if I should go to primary doctor

    FTM here, 33 weeks and just found out i indeed have ICP. My doc wasnt even gunna test me because i had a rash... My fiancee just basically told doc to test me anyways since i was ao itchy EVERYWHERE! Started itching 3 weeks ago but thought it was normal... But glad im on meds now and being monitored.

    I heard somewhere that with 2 years of having a baby there’s a high percentage (can’t remember the actual amount) that a woman is going to have surgery and most of the time it’s to have the gallbladder removed. I’m not sure how but it sounds like this is all somehow connected 🤯

    Got diagnosed a little over a week ago and I'm literally starving, having one small meal a day because this pregnancy is not letting me tolerate chicken or turkey and almost all the vegetables I like which is like tomatoes, onions, jalapenos, ect has started giving me heartburn and some times will even causes me to have a flare up and it puts me in intense pain. I'm only 22 weeks and I've been losing more weight then gaining but baby is measuring healthily so.

    I dont have a gallbladder. This itching has it where sleeping is a no. I'm on medication. But it's like it makes it worse.

    My fiance has lightly yellow palms. Almost no discoloration, but absolutely no itching in her palms or feet. No pale stood or yellowing of the eyes. She’s 15 weeks pregnant tomorrow, is there anything we should be concerned about?

    Had with my first, was induced at 37 weeks. 57.5hr induction was not fun and we had a week nicu stay after she was born.

    Is a severe itching while you are 10 weeks pregnant harmful because I just had a miscarriage from itching every night

    Yes! Because it is a condition of your liver and not your gallbladder!

    17 wks and told have slightly raised enzyme in liver blood results scan tomorrow on liver so scared 😱

    Mine started on my stomach and back, then moved to my head and ankles and then moved to the hands and feet. I knew about this so contacted my doctor the day after I noticed the itching on my hands and feet during the night. She tested me that day and my bile acids were at 120. She also mentioned that with itching she retests every week with patients who have the itching, because sometimes symptoms can start before lab work shows elevated levels.

    I didn’t have this but I had SEVERE vomiting my entire pregnancy 😐 Vomited like anywhere from 3-10 times a day, it sucked, it vomited up until the week I delivered. Everything made me sick! It was terrible and the doctors didn’t know why this happed and the medications didn’t help 😖

    My sister and cousin had HG (hyperemesis gravidarum) which is basically an inability to hold any food down during pregnancy. It was terrible. Zofran helped but not perfectly. My cousin had to have multiple IVs to just stay hydrated throughout those months.

    I had this with ALL 4 of my pregnancies. It was torture. I itched EVERYWHERE. It wasn’t until my last pregnancy that I was diagnosed. The other doctors didn’t seem to care

    I had cholestasis in my first pregnancy with my son, but I only had the itching on my legs.... it's happening again now... I'm 23 weeks, and I have high liver enzymes already...even though I had my gallbladder removed in 2015.

    I had my gallbladder removed at 22 weeks pregnant and now I am 29 weeks along. Yesterday my palms and feet started itching so bad and today my whole body including my face itches. Should I be worried? I don’t have another appointment for about two weeks

    Ladies, please don’t just think generalized extreme itching is normal either. It isn’t. I didn’t present as having itchy palms or soles at first, just generalized itching but my night (which quickly also turned into all-day) itching was so debilitating for me and was starting to impede on my mental health that my MFM and OB took me seriously and began treating me for Choleastasis without the official diagnosis. I started URSO and was bumped up to 3x per day, or 900mg. The itching began around week 26 of pregnancy and I’m now in week 32, with a scheduled induction for week 37 for my twins’ birth due to the itching alone. That’s right, my blood work has not changed in the past 6 weeks but they’re still taking my symptoms seriously. If you are having any severe itching at all, please ask your doctor to be tested. It could save you and your baby’s life and you absolutely CAN have it without your blood work showing it. 🌸

    I got really bad rashes on my belly my doctors told me it was puppps+ eczema bc i do already have eczema 😢 my bile levels were normal at first but they just went up. Im worried for my baby is there anything i can do or stop eating to help my bile levels🥲

    I have gilberts syndrome, my liver enzymes are always high. I too have itching problem in arms, back, belly and legs. Not at the palm and sole. Could this be cholestatis? 36th week now
