    Great guest and topic! Yes healthcare is a challenge on all fronts . Despite difficulties I feel fortunate to live in Canada. That aside , I feel it is my responsibility to be proactive with my health, healthful eating and exercise . A priority I’ve held for a long time. Just turning 70 I am proud of the care I’ve taken.

    What a great episode. So much information. I’m a Nurse Practitioner. The Explanation of Benefits is unbelievable. The Doctors, Hospitals, Nurse Practitioners, etc. charge a lot because we’re going to get knocked down so much. I live in Central Illinois. My 15-20 minute visit charge is 240.00 for a Medicare patient. Reason being, under the Medicare Contract Medicare knocks my charge down to 80.00. Other Insurance Companies has their own rates but they all knock you down considerably. Just like Hospitals for example, my mom passed away 3 months ago. She had a CT scan. The Hospital charged 900.00 but Medicare knocked it down to 30.00. Thirty dollars!!! Everything she said was 100% correct! You definitely need to schedule 2 different appointments for different issues. One if you need a med check & one for your annual, for example. And writing down family history… YES!!!! Your medications, previous surgeries, etc. It’s not that we don’t want to spend time with you, it’s that we only have so much time. I miss being able to spend time with patients. I took the last 3 years off on a leave of absence to take care of my dear, sweet mom who had Alzheimer’s Disease. I’m an only child. We were so close. She passed away 3 months ago. I really don’t know if I want to go back. I love being able to spend time with patients. And we can’t do that anymore! Insurance prevents it. So I don’t know what I’m going to do. My husband is retired. I’m only 60. Only! 😊 This was Fantastic information and a great interview Dominique!!!🙌🙌🙌👏👏👏💜💜💜

    Thank you for sharing all of that with us. I am so sorry you lost your mom. I lost My mom to dementia and maybe some alzheimers 7 yrs ago. A year later, my dad. They were married 61 yrs. I miss them both.

    One of my dear friends is an internal medicine doctor. She doesn’t want to be a doctor anymore. It’s due to insurance companies who dictate how doctors are able to treat patients and the continuity of health care is so disconnected.

    There's also the matter of government "authorities" dictating treatments during "public health emergencies," which I imagine would be discouraging for doctors who like to work independently and use their own judgment. The system is beyond broken.

    I feel that sometimes the staff at the desk, who are the ones that get the ball rolling , in tests, or call backs with info, test results, etc. drop the ball. Some are not educated enough, not good at their job, or too overwhelmed..I have found that the delays in my health care at times, is all because of the desk staff. I found out that the test for my heart, showed a severe problem that indicated that I needed a triple bypass . I was given the information 4 weeks later. The staff was to handle the results and notify the Dr. I get more information with utube MDs when it comes to informing me with treatments, side effects, and all around information. .

    Coming from someone who has worked in the medical field forever, if the doctor does not have a nurse or certified medical assistant to help him/her out with follow up then that office may be at risk for poor follow through - especially if it is a busy practice. Also, look at online reviews of the practice and physician to see if there are red flags. Having great doctors and management is key to retaining good staff; oftentimes having a lot of turnover equals staff not invested in doing a thorough job. And if your health is ever compromised due to staff or physician issues seek legal advice.

    My doctor spends 20 minutes with me 2 times a year because that's what Medicare pays for. I can't afford functional doctors.

    Great topic and guest. I feel fortunate in the DC Metro area that, when I experienced breast cancer last year, I had a team of three INOVA doctors (all women) who met weekly to review each patient’s status and agree on the best courses of treatment. My surgeon, medical oncologist and radiation oncologist were lovely, sympathetic and knowledgeable about my situation and treatment plan. I see them every six months and feel very well cared for. In addition, I continue to see my integrative doctor, and now an integrative oncologist who supports the conventional treatment I receive which she enhances with dietary modifications, supplements, and stress and sleep management. I pay more for the integrative practice care, which includes a membership fee but also accepts insurance. I find the doctors in this area are receptive to alternative approaches and treatments. I’m sure part of it is this area is highly educated and affluent and patients, therefore, tend to be more demanding.

    Hi Dominique. What a great topic. I've worked in the medical field for over 30 years & agree 100% with Dr. Ghofrany. I'm a big advocate for patients knowing their insurance plans & if they don't understand their coverage call your insurance plan & ask questions. As a Practice Administrator I'm big on staff being trained in all aspects of a medical practice. Healthcare providers practiced medicine they shouldn't be trying to train their staff. They should be willing though to spend some money on seminars, conferences & allow their staff to take continuing education courses which will benefit the whole practice. The biggest culprit in the healthcare field is the insurance companies who are allowed to do whatever they want without government involvement. As such if the government will not do anything for its citizens, it is up to the consumers to advocate for themselves & their families to get things changed for the better. We as American citizens deserve better healthcare & we need to demand some change. Thanks for allowing us to view our opinions helpfully without judgement.

    Wow MUCHAS GRACIAS Dominique!!!! This was very informative and needed so we can understand how this “works” I had lots of questions and complaints in my mind with my doctor’s visits. Also this issue confirms how we personally have to take care of our health daily and just save money for when we really need sick care. This doctor is great!!! hopefully she can be the pioneer of the medicine integration for all.

    Wow, I feel so blessed. First of all, I changed all my doctors to female doctors and roughly in the "middle aged" range. So my age or a bit younger or older. late 40 to early 60. All of mine spend as long as I need in a my appointments. I NEVER feel rushed and I felt heard. I don't want to stereotype, but maybe there is something to that equation.

    I have cultivated my team on my own. It is up to the individual to learn and ask the necessary questions to advocate for themselves. My Western doctor and my naturopath don't always agree, and ultimately, it is up to me to decide for myself and endure their criticism.

    August will be 2 years for me on Medicare. It's a challenge for sure, the Dr's are limited, the dental and vision care is not good, even worse than the medical care. I'm not familiar with the prescription part because I'm not prescribed meds, thankfully. Very enlightening podcast. My mind is more open to the Dr's and the system. And not just focused on me, me, me. I'm a person of an open mind, so it was easy for me to be open to learn from your guest. I didn't meet my Daddy and many other family members until in my 30's, there are a lot of family history I just don't know. Not all nurses and or Dr's understand that. May I ask what is the vaginal HA product? I need it. Thank you Dominique ❤

    Why don’t doctors have more time? I waited 50 minutes past my appointment time to see my primary care doctor. Don’t understand why ? Do I need to find a new doctor? I’m 68 years old and feel dismissed.

    I'm surprised about the length of that wait. In my area Dr's seem to have all gone to the concept that pt's. who are late, 5 to maybe 7 or 8 min's., are considered to be late & have to reschedule.

    I am a previous telephone customer service agent with Rx and multiple health insurance. I am now venturing out launching a website to help caregivers and patients research their insurance questions. Coming from a Medical office management / insurance customer care I’m recognizing the average call time requirement for call centers is 6 minutes. Even more extreme than the 15 minute visit. I’d love to have a conversation to discuss further.

    I now get my physical, if you can call it that, with my clothes on. It seems doctor's have to see so many "units" per day so, we are "units"...and we pay for this. Seems to me you're better served being your own health advocate.

    The best healthcare have ever received was in a college for traditional Chinese medicine. The diagnostic procedure by its very nature requires that the practitioner spend 30 min or more examining and talking to their patient. The colleges have excellent teachers who supervise all care. I have never felt so pampered in a medical setting. The colleges usually charge very little for their treatments. Going back to a conventional doctor after that experience is a huge downgrade! Of course, independent doctors of Traditional Chinese medicine are a great way to go, but will cost much more.

    That is such a great question that I have molded over frequently with my colleagues and my parents who are retired physicians… The only answer I can really give you is that it sounds simplistic, but the doctors are literally busy seeing the patients every day… So if we stepped away from seeing the patients in order to advocate and lobby for ourselves we wouldn’t be able to… It’s kind of different than any other field when you think about it because of your doing doesn’t really involve any background work or prep work or anything it literally involves sitting face-to-face with patients so anytime away from that is challenging for your practice… I know that doesn’t sound like a great excuse but historically not banded together well and that is certainly a problem!

    @@shievaghofrany It does sound like a poor excuse. Patients have very little power because we need you, and insurance companies think that we don't exist. They've taken over your jobs in major ways, even not covering much-needed treatments. Perhaps if doctors could stop for a moment and see how we could band together. This must really piss you off that they tell you what you can and can not do. I held a very close friend's hand as she died because she wasn't covered for bowel cancer by her insurance company. By the time she finally was operated on pro bono, it was too late. The cancer had spread to her lungs. I heard it the night she died. It was horrible and I lost a special friend. Insurance companies are allowing your patients to die. This pisses me off.

    I kindly disagree with the guest's proposition the patient has to compartamentalize questions in separate appointments (as cited, I am not supposed to ask anything beyond answering her screening questions within the realm of preventitive care). It negates the guest's very premise of INTEGRATIVE medicine where the patient has to be seen as a unit, as a whole.

    I don’t have insurance and getting alone better without it. I feel I get better care. Health insurance is a scam. I had my two births without health insurance. It was less then $1500 for my entire pregnancy all the way through birth.

    If, like me, have multiple doctors, the lack of communication between doctors is basically non-existent. I’d love to see more communication between the patient’s doctors in order to provide the best care.
