Do not misuse cough syrup.
Codeine metabolism happens in the liver and is mediated primarily in the first pass by CYP 2D6 (to morphine) and CYP 3A4 (norcodeine). Codeine and morphine can be glucuronidated in a second pass of metabolism, but watch for renal function in hospitalized patients as morphine-6-glucuronide is renally excreted. Because it is pharmacologically active as an analgesic as well, use caution in patients with renal failure. Genetic mutations in 2D6 have been observed and result in different pharmacokinetics for codeine. See references below.
This case is 2 cases combined. Both are de-identified. This video is not individual medical advice and are for general educational purposes only. I do not give medical advice over the internet.

    As a recovered heroin addict I can say no words or images can fully express just how awful opioid withdrawals really are. Flu symptoms, vomiting, diarrhea, pain like every bone in your body is broken, profuse sweating, restless legs, feeling cold then hot then cold over and over, constant panic. If you haven’t touched an opioid keep it that way.

You can drink it and it pretty much passes through you. It used to be used as a laxative -- of course, lead was also used as a spice, so historical usage isn't a great indicator that something is safe. Mercury vapor is really bad, and there are lots of mercury compounds that can cause it to pass into the body with truly horrible results. But that jar of silvery liquid? Mostly safe, if you only drink it once. Mostly. If handled right. My wife (a chemistry professor) won't let her undergrads use it in her chem labs, because if not handled right, it's bad times all around.

    A rapper ate spaghetti. This is what happened to his career. “MM” is a man presented in a grand show with sweaty palms and weak knees. He feels like his arms are heavy with nausea. “MM” has an intense anxiety and is choking now. He opens his mouth, but the words he’s trying to say won't come out. To understand why “MM” is having intense symptoms we must travel to earlier that day. Before the big show, “MM” mother made him a plate of pasta. A simple spaghetti and sauce dish made like anywhere else. However, “MM” was very hungry and in a hurry to make the show on time. Instead of one serving of pasta, “MM” had two and half times more in a span of 5 minutes. This is where our first issue begins. “MM” does make it to the venue on time, but the pasta is not sitting well in his stomach. Moments before the show he throws up on his sweater. It is at this point where his arms become heavier and his knees barely are able to support him. The strain to keep standing causes “MM” to heat up rapidly making his hands sweaty. He is rushed to the stage where he is unresponsive. The crowd around him waits for a response as he begins to lose himself in the music. Everyone around “MM” is now in shock about his sickness. His O2 flow and rhythm is abnormal surprising everyone. He survives the event and makes a great Recovery years later.

    A lot of specialty medicines are selected and widely used precisely because the liver doesn't ruin their pharmacological effects within 6 hours by breaking it down. Additional variants are also explored for their ability to remain in circulation without immediately being excreted in the urine. TL;DR: Liver isn't that great at detoxing.

  • @firstnamelastname5474

    @firstnamelastname5474

    @@Baigle1 well what do you expect? Would you want to take a medicine and have it excreted the same minute you took it? Say take a pain medication, do you want it to be excreted way before it has an effect on the body? There are lots of factors to be considered as well, like the bioavailable fraction, Vd of a drug, First-pass hepatic metabolism, overall drug distribution and metabolism, First-order elimination, the half life of a drug, it's pharmacodynamic and pharmacokinetics imterplay, and as such pointed out here in the video as well, pharmacogenomics. So no, I will argue your summary that "the liver isn't great at detoxing" is wrong base on the argument you presented. There is a unique interaction between the liver and medicine, it's not all about how fast the liver can remove it.

My younger bro drank an energy drink one morning to study for a test and couldn’t fall asleep until 3am. But when it comes to medicine, it’s the opposite. I’ll be completely out of my mind on a regular dose of NyQuil (it’s actually a rule in my family that if I take NyQuil, I have to be supervised). My brother takes NyQuil, falls asleep for the next couple hours and wakes up fine.

    As a depressed autistic, I generally feel nothing, but when I do it's usually intensely negative. Whenever Dr Bernard describes the effects of drugs I get a strong urge to try them because I would finally be able to feel something good for once. I manage to remind myself what a terrible idea that is, but it's not easy.

He said take like 50 mg and I would feel a whole bunch better. Which I did. I ended up doing exactly what this dude did and took more and more every week. I eventually landed in the er with a super irregular heartbeat from long QT ( which lots of lope WILL cause). When I got out, I transferred to a rehab place and I went back on suboxone. It’s the only way I have been able to put my demons on lockdown.

    I just watched the entire video, I did not realize that gentleman died. I regret my meme comment in light of the tragedy that occurred.

    @@Existnt160 Hi how are you? I didn’t realize I could delete or edit comments. I do now (found out like a month ago?) and regret all the spelling and grammatical errors, forever left on random videos. Humor is subjective and varies with mood. If you were in a better mood you might be more inclined to laugh.
