  I was done with game before the dlc and then new game plus till the first new clan encounter anyway I did it on new game plus ultra hard the main game alone was hard even with all legendary‘s that was a really realistic experience and the DLC was WOW!!!

  I doubt anyone cares to know, but the glitch at 16:00 is caused by the PC port, now how is it that it occurs on PS4? They literally just ported the PC port onto the PS4, leaving Glitches and everything. What happens is rare, but requires just a few hours minimum. It's funny the first couple times, but is so annoying to just be playing and suddenly lose so much progress because of an unfixed bug... Edit: Just to answer how it triggers... Just playing. It's why the requirement is just a minimum of a few hours.
