USCSB

The CSB is an independent federal agency charged with investigating incidents and hazards that result, or may result, in the catastrophic release of extremely hazardous substances. The agency’s core mission activities include conducting incident investigations; formulating preventive or mitigative recommendations based on investigation findings and advocating for their implementation; issuing reports containing the findings, conclusions, and recommendations arising from incident investigations; and conducting studies on chemical hazards.

The agency's board members are appointed by the president subject to Senate confirmation. The Board does not issue citations or fines but makes safety recommendations to companies, industry organizations, labor groups, and regulatory agencies such as OSHA and EPA.     No safety = dead workers. No regulation = dead workers.. looking at the drill industry from outside, looks like company’s from 120 years ago.. company mottos production before safety.. after accident, company comes in worries more about getting back up and running.. so glad I put my mind to work not my body..

    Oh look in other comment section full of victim blaming. We're all capable of doing something as dumb as this. There's a reason everything is supposed to be designed so that the right way is the easiest and the wrong way is the hardest. We have no idea who these employees were. Whether they were forced into the position with insufficient education, whether they just weren't paid enough to care, or they were under stress or overworked. None of us help anything along by calling them stupid.

    WOW, i drilled Old School, from debts from 9k' to 20k+' we had Nothing like this. The Tecnology these days is amazing, the Driller sitting in a cozy room in an office chair, not getting a drop of mud on him, Crazy. How deep were they before turning Horizontal and how many feet of pipe? Ive done Directional Drilling a whole different process. U dont turn the pipe and Bit, the Pipe dies not turn and the Mud pumped down whole turns the Bit. Never a vertical hole, then turning a 90° Horizontal. Technology here is outstanding, id like to see how a derp hole lsnd Rig is run these days ... Amazing!

    The reports where questions were skipped were approved for a reason… they knew. I wouldn’t have signed off on that form either, but management said okay don’t put not applicable because it is applicable, and don’t put no either but nobody would put their name on a form saying yes it was safe so they just left it blank and moved on.

    In Los Angeles, the LAPD confiscated a ton of illegal fireworks, placed them in a large pressure vessel, and exploded them. 35 homes were severely damaged, and 80 people were displaced. Oooops!

    I like listening to the Causality podcast on these incidents better. These videos are straight from the source, but give a, "sure thats obvious after the fact" tone. I feel when listening to the Causality podcasts I naturally put myself in the workers place. I feel what they did leading up to the disaster is natural. I'm surprised when things go off the rails and I feel responsibility in the emotional part of my brain. I walk away feeling reprogrammed. Like I got to live a life experience, walk away, and know to be vigilant for similar factors arising in the work I do every day. The CSB videos just leave you with the impression of, "duh, why would you use a heat gun in a confined space with flammable resin." You don't feel how tenacious and natural the misteps leading up to distaster are, and therefire you don't learn to spot them in your own actions.

    A good example is the Hyatt Regency Disaster. If you look at it after the fact, knowing ahead of time that the bolted connection in the diagram in front of you is what caused the failure, the mistake is obvious. But that particular failure mode is not the lesson to learn. That situation will never come up again in that same configuration. The lesson is in how it happened. A frustrated contractor on a tight deadline. A phone call to you about a design that's impossible to assemble. You have egg on your face, you're feeling like an idiot. The contractor suggests a change. Off the cuff you say yes, that's a great idea, do that. You say it because it solved the problem you had your attention on, the one that was making you look stupid in the moment. Phone call ends, and you get distracted by the next thing. You don't go back and run the numbers. You don't take a second look to realize it will assemble, but only carry half the load. THAT'S the lesson. Spot that situation. Next time you get that call, remeber the implications of making an off the cuff decision. Say, I'll get back to you. Think it through.
