    It's held up pretty well and yes, I've swapped out the harness for a mayflower one to see if it fit. Gonna do a follow up video on it soon. Still a good rig, not my preferred one though

    @@theguerillamedic for the armed prepared civilian for sustaining and resupplying himself when operating in a small unit or alone, yes to a full size rig with as much mags you could carry! The more fire power you have to suppress and evade the longer you’ll stay in the fight. Look at the battle in Niger where 4 SOF were ambushed and killed. They fought for as long as they could before being overwhelmed. Or look at the current Ukrainian forces fighting in trenches where they’re mostly suppressing fire to frog leap towards objectives.

    Yes scissors. Just not in the spiritus fanny pack, they're carried somewhere else on body. I've got a med fanny by aws that they sent out to play with and the ifak contents plus an additional tourniquet and shears fit in that one easily.

    I don't think it's worth the 300.00. If you are actively practicing tactical medicine, this may be very slim. The Tajmainian Tiger MKII large is wayyyy better and about 1/3rd cheaper. Frankly, it's a rip-off. If the price was 150-200, I think it would be more reasonable. Students where I am at are showing up with these, and it's not going to do the trick (platoon-size coverages). But I guess it depends on the use case and your budget. But god man 300 dollars is balls and you don't even get any panels for the inside. We are experimenting with some new concepts at the schoolhouse, and with the new(ish) blood-on-target capability, this is definitely cutting it close. Good car bag maybe... maybe a good mini-speedball.

    It's definitely a niche bag. For a line medic I'd still stick with something like an M9. I was curious when it came out and had the scheckles. Definitely don't recommend it for everyone.

    What meds are you running? I was thinking Zofran, Diphenhydramine, Epi, Dex, Turbutaline, Calcium, TXA, and Rochephin.

    Depends on what I'm doing, but usually its just bls type meds-epi, zofran, diphenhydramine, non-narcotic analgesics, small thing of lido for crics if time permits, rocephin, some corticosteroids, and albuterol for the most part. If we're out doing some longer term shenanigans in the field I usually I add some more abx, immodium type meds and boo boo stuff. I don't do cool stuff anymore so I don't get txa or als drugs but if I did those would definitely be in there. The former pj's at greenfeet medical do some good stuff and probably have more insight on those.

    I've shoved both in there, not idea. With a 32 oz nalgene it didn't want to zip very well. Im interested in their hydration pouches though for nalgenes and canteens

    Amazing review 👏, immediately subbed. The only thing that I don't like about this chest rig is the velcro, which is a bit loud and annoying. I got a friend who's running the Ghost Chest Rig from barrelandhatchet, and it's so quiet. I tried ordering, but it's out of stock. That's why im planning to get the trex arms chest rig, is the velcro mag annoying for you? I would love to see a review comparison in the future if you have changed to get it

    Thank you for the kind words! I've never minded the sound of velcro mag carriers. The GP pouches have zips so if im trying to be quiet that helps. With a mag pouch, the only time I'm really accessing them is when I'm reloading, so the 30 rounds I just fired off kinda blew any stealth I had. Just my opinion.

    ​@theguerillamedic is there a hole running through the flap you could run shock cord through? On my eagle mag pouches I run shock cord through the flap and attach a toggle to the drainage hole on the bottom of the pouch. To close it I just run the shock cord over the toggle. This allows me to silence the velcro or increase the retention.

    2:20 Spiritus kept the Multicam lines/spots facing horizontal... TRex has sections with vertical Multicam which is not to camo specification. 😢

One is a name brand that was contracted to fit out the military for years and has been widely used and tested and the other hasnt.     Returned the one I got. Hook and loop on the mag flaps was very weak, and with two mags in the pouch there wasn't much purchase of the hook and loop holding the flap down. Otherwise very well made.

    Good to know, I'll keep an eye on it. Weak velcro isn't too much of an issue for me, I've got a good tailor that does all sorts of odd projects. If it happens to me I'll throw out an update on ig and what I did to fix it

    Good review. Is there a move toward carrying 8 mags instead of 6? Six seemed pretty common for a long time. I'm also a smaller guy, 5'6" and medium tshirt, so maybe I just don't have enough space for 4 mag pouches with two flanking GP pouches. The Helikon Guardian has served me well, but only carries 6 mags, and is made in PRC. Also, the podcast seems to have disappeared off of Podbean. Are you still doing the podcast? I've referenced you a few times recording the Stuck Pig Medical podcast with MechMedic, and I was wondering if you were still doing the podcast thing. Best, Teddy Bear <><

    Podcast is alive and well, think there was an account error but I just checked and its up. I've got different rigs for different mission sets. Not having the backing of uncle Sam means I've got to carry everything myself sans a friendly resupply (insert scenario here). For me, a standard load out in the field is 10 mags and armor. Without the support of a role 3 or similar, I'm willing to take the weight of more mags and slick armor. I can always down load if the situation dictates. At a mag a minute roughly its just more options. IMO the days of 3 mags and calling it good are done, the old ways of infantry are coming back.

I've been writing up a digest of a paper by Regan Lyon, MD (USAF) on a proposed GW/UW medical system. The numbers she sites for Vietnam KIA support your point. Non-correctable torso trauma (25%) Surgically correctable torso trauma (10%) CNS injury (31%) Obviously, US troops were functioning as an occupying conventional force, usually not choosing the time and place of engagement, but emphasis on small arms has some potential parallels.

    Honestly the TAPS is great, I have zero problems running one. Ive found paperwork in a cargo pocket is the way for me.

    I like the idea with the duct tape on the chest seals. I usually carry a hotel key card wrapped in 2” medical tape. But it’s something id consider. A couple suggestions from Doc to Doc. Consider placing your gloves at the top of the kit or on the outer pouch. If your concerned about BSI, have it be the easiest place for your buddy to get to. (You could have your joes donate blood on the regular, which would tell you if they have any communicable diseases) If you ever find that kit bulky, you could opt for a 6” ace wrap. Its gonna be way cheaper and slimmer then etds. Have your joes pre fill their admin data on the TCCC card. I don’t want to be asking about allergies while I’m treating. A sharpie would be a good addition to the front pouch for writing down information. Additionally somehow the privates always eat sharpies. Can never have enough.

    Wow I've been looking for a video like this for a long time. Much appreciated. Any chance you could go over some of your kit/philosophy for different (patrol/urban/etc) loadouts? As a non-mil-vet doing medic work in Ukraine videos like this are really helpful. Been figuring it out on the fly for a while now so just wanted to say your videos have provided a really helpful insight

    It really depends on what your mission set is. Now days I'm just regular guy so depending on what type of training or what we're doing depends on what I carry and how. For example if you're a fighter first and medic second be able to do your primary role and flex into that medical specialty as a secondary job. If you're more like a traditional stateside medic following a team that's purely medical then you're gear should reflect that. Lotta good dudes putting out great stuff about this doing the job CONUS and OCONUS. CRO medical has a great section like a blog that I've found super insightful.

    Thank you! The basics like tourniquets are foundational for sure, but they're just the basics. Most guys just haven't been exposed to more complex injuries. Excited to do more stuff like this down the road!

    would be pretty cool to see how you set up having a chest rig you can put over your body armour rather than trying to fit everything on a PC; seen quite a few guys fighting in Ukraine talking about how that setup is ideal as they can just take off the chest rig but still be protected while they dig trenches or otherwise doing manual labour that takes up a fair bit of soldier time. Also saw another medic recommend that setup as they could just pop the chest rig off when casevac'ing the casualty so they won't so heavy but still protected. would be cool to see your setup in regards to that

    Thats usually my preferred method, chest rig over armor for all the reasons you mentioned. Especially if its out over a long distance where I can take the armor off and put it in my bag if needed. Lots of options.

    Pouches like the blueforce gear and other thin material dump pouches are good for picking up items from a site that is important... When in the field I use a 1000D pouch from some random company that can hold spent mags till I get a minute to unpack. I also will put random stuff that doesnt quite fit in a pouch on my kit.

    I have a buddy that runs one all the time. It seems to be a good product. I like as much versatility as I can get and the dump pouch can fill alot of roles for me, a pro mask is just one. Just where I'm at in the never ending quest for the prefect set up.

    Nice overview of the Delta Bag, thanks. I wish someone would make an improved, modern version of the venerable M-5 aid bag. The clamshell opening of both the M-5 and the Delta Bag are really the best way to work out of an aid bag.
