    The culture of overworking in medicine needs to change or the problems in the field will only get worse as time passes. To become a doctor you have to perform very well in undergrad studies, then you have to study intensively for the MCAT, then endure 4 years of med school with lots of stress and long hours, then you have to do well enough on the Step tests to actually get into a residency, otherwise all those years of studying might've been for naught, then during residency in many specialties, you work extremely long hours for years. Finally, after 10+ years of relentless work, you are able to actually have some freedom/autonomy IF you can pull off private practice, but private practice is dying, so probably not. So then you get hired somewhere, and as you say in the video, they want a return on their investment, so you are still working 70+ hour weeks. At this point, you are 30+ years old and absolutely burnt out from all the stress you've endured just to get to the point where you can even *begin* your career. This isn't even considering the fact that you kind of give up your 20's to even have a chance of getting to this point, which to many is the prime of one's life. The question to me is, is this actually necessary to achieve a high degree of competency and quality of care? Or are we just needlessly stressing and exhausting prospective doctors because those that came before had to deal with it too? I personally think that there is at least some room for change in regards to the hours that is expected. 70-100 hour weeks for months on end is just not sustainable. Medicine is fulfilling, but so is putting your kid to bed, watching a movie with your SO, being able to see your friends more than once every 3 months, and just enjoying life outside of the office. Oh yeah, and I forgot the amount of debt that you are left with at the end of it all, which only compounds the stress and the feeling of needing to keep pressing for long hours. And if you fail to pass during med school, or fail to get a residency? Well, the difficulty in paying off that debt just increased.

    As someone who is just an observer and not involved in the field, it seems the system really needs an overhaul. When I heard about how the doctor who created the residency structure was addicted to cocaine and morphine, I found that very hard to believe until I looked it up. I hope the medical system sees optimization much sooner than later, for doctors and patients sake.

    I'm happy that you've found the balance Dr. Webb! It's unfortunate private practice is dying because of insurances and high overhead. it's unfortunate the reality to medicine is not so glamorous and there's many problems now.

    private practice is still feasible if you're smart with your expenses and efficient. very feasible, financially. the only downside is the management side of it and the headaches

    I'm new to the OR, but I've already noticed patterns within specialties. Firstly, I must say I have immense respect for what you surgeons do and how brilliant you all are. It's fascinating to watch you function on such a high level. You obviously are invaluable to humanity. But I have never seen worse behavior than I have from surgeons in neuro/spine field. I'm talking throwing objects across the room, berating and cursing out staff, and just generally being overly insulting to everyone in the room. I understand this is a stressful job, but I can't help but wonder how much of this is a genuine reaction to stress or if these people enjoy being able to be abusive with zero repercussions. It blows me away how comfortable some of these guys are with being absolutely vile to everyone around them. Which, I must add only ruins the productivity of the surgery because everyone is on edge which leads to mistakes. I'm not insinuating that you do any of this.

    I'm not a very productive person but I have noticed that I become unnecessarily aggressive and disrespectful when I'm overworked. Like you described Maybe they're just overworked for so many years that they just became this

    Amen Antonio. I went through similar struggle in choosing a surgical subspecialty, and concern over being able to have a life outside of work. I went to private practice and am thankful for the freedom this provides with my schedule and case volume. The employed model is pushed heavily now, and hope trainees who choose that path are aware of the pros and cons of employed vs private practice. I’ve enjoyed your videos. Best!

    Brazilian Jiu Jitsu may be a great outlet instead of going to the gym. It has helped me by clearing my mind and the team training is so positive. It is also functional fitness at its finest.     This is something I've been really thinking hard about lately!. As an IMG who's into surgery it's much harder, and i was really thinking does it really worth it?! to invest all that effort and time and the end result could be losing ur life and family time.. i was really convinced with the huge number of negative life-work experiences..     1/2 days on medical office and 2/3 days on Surgery days. One on off day for emergency surgery/ be on call/ planning for long and hard surgery? You also take one week of be on called for ortho trauma care.
