Are you confused about which type of B12 is best for you? Are you wondering which form of Vitamin B12 is best, in general? In this article we will look at the different types of B12 and different ways to take B12 . We will look at types of B12 based on genetics and using signs and symptoms to point you in the right direction. First we need to understand the different routes of administration and the different types of B12. From here we can understand which form a B12 is best for you. When it comes to the different types of B12, it really comes down to which molecules are attached to the cobalamin molecule. This structure allows us to determine which one might be best for you. It's also worth pointing out that there are some types of B12 we can exclude from taking at all.

B12 is known as cobalamin. The type of B12 is based on what functional group is attached to it. It could be a 'methyl' group as in methylcobalamin. It can also be a cyanide group as in cyanocobalamin or a hydroxyl group as in hydroxycobalamin, and finally an adenosine as in adenosylcobalamin. Each of those molecules on the front of the cobalamin name are the functional groups. In the case of the 'cyano' and 'hydroxyl' those two molecules are not really adding much value to the physiology of your body. In the case of the 'cyano' group or cyanocobalmin you probably don't want to take it at all. Cyanide is poisonous to our mitochondria and it's just not good for our bodies. In a pinch it will help if you are really really low on vitamin B12 (cobalmin). If no other sources are available, it's ok to take it. It's generally not recommended to put cyanide in your body no matter how small it is, however. Hydroxyl groups do not cause any harm but they are not as beneficial either. So both 'cyano' and hydroxyl are not necessarily that functional and don't add much benefit to the body. Now let's look at those that do add benefit.
Methylcobalamin and adenosylcobalamin do have functional uses. Methylcobalamin has a 'methyl' group on it. If you do have genetic issues with recycling B12, such as an alteration in MTRR (or even MTHFR or MTR), you may need more vitamin B12 in the form of methyl-B12. This will keep those enzymes functioning at their normal capacity. This is a genetic reasons why you might need methylcobalamin. Another reason to look at methylcobalamin is if you have a high homocysteine. High homocysteine occurs in B12 deficiency.
A methylmalonic acid test will be elevated when you have low B12 levels too. The methylmalonic is a more sensitive way to test for B12 deficiency, however. The methylmalonic molecules increase when there is a lack of adenosylcobalamin. This is a type of B12 which is needed for fatty acid and myelin sheath production. The myelin is an important component of your nerves. When you have a lot of nerve issues you may want to use the adenosyl form. The problem with this form is that doesn't come in injectable forms. It is less stable so harder to make in injectable form. There are also limited sources of adenosylcobalamin. Here is a link to adenosylcobalmin you can take by mouth.
The main routes of administration for B12 are oral (swallowing), sublingual (dissolves in your mouth), and injectable or intravenous. We discussed the different types that are available through injectable and intravenous. All the types are available to swallow and I believe sublingual as well. The best route to administer your B12 is injection or intravenous. Of course, that's not freely available to everyone. The next best option is sublingual or under the tongue. The reason sublingual is better has to do with what happens when you swallow the B12. Some people don't have the ability to absorb it. In most cases this is why people become deficient to begin with. Be sure you talk to your doctor if you have questions about this as there are many different things to consider. Clearly if you are deficient you need more B12 and a lot of these different forms can work.
That should give you a better understanding of which form of B12 is best.
Adenosylcobalamin is used more for the nerve conduction and myelin sheath production. If you have issues with methylmelonyl CoA mutase genetically, you may need more of the adenosylcobalamin as well. Adenosyl and methylcobalamin are two of the best forms you can get. Because the adenosine does not come in the injectable form, you may want to use the methylcobalamin instead. Hydroxylcobalamin is a good substitute instead of the cyanocobalamin.

    If you don’t have any issues with methylation, just take the hydroxyl form and let your body use the enzymes to convert it to the adenosylcobalamin and methylcobalamin forms as needed. High oral doses of hydroxylcobalamin will raise serum levels without the need for injections. You run into problems when, you are someone like me who has a MTHFR but also has symptoms of over methylation. It’s a real conundrum. When I take things like SAMe or methylcobalamin or even methylfolate I get aggressive, can’t sleep and kind of just feel overstimulated in a dysphoric way. Even hydroxylcobalamin caused these issues because of its conversion to methylcobalamin. For me, Adenosylcobalamin, thankfully, doesn’t cause as many issues but if I take more than 1000 µg I do get over methylation symptoms because Adenosylcobalamin and methylcobalamin can convert back-and-forth to each other.

    I'm very confused after watching multiple videos, some say cyanocobalamin is good and all the bad stuff i hear about it is not that bad and it's most stable, then some say it's the worst and i should stay away from it. Then some say the methyl one is much better and safer and others say it's unstable and i should take the cheaper cyanocobalamin

    Many doctors only want to give a couple of b12 injections, which a lot of people with the deficiency say won’t help. I have some serious neurological issues as do many others. Do you have any comments about how you would treat someone like me with b12 injections? Frequency/duration?

    I have MTHFR gene mutations and slow COMT. Methyls make me unable to sleep. I tried adeno b12 and had a histamine response to a very small amount. What is wrong with me?😢

    Hi Doc. Very interesting video. I have always struggled with very low b12 levels and some recent blood work indicated I again still too low. I have always self prescribed my supplements in the past but my doctor prescribed me Cyanocobalamin at 1000mcg daily. I was told to take for 3 months and return for a follow up blood test. First week of consumption I started to feel the benefits but in week two I could feel something was wrong. Random increased anxiety, freezing hands and feet, waking up in the night and heavily constipated. I messaged my doc who told me to keep taking the b12 and everything was fine but I made such a complaint that she told me to go to get another b12 test 1.5 weeks after my first. My results were now in the green (was 130 and now 288), but I was feeling like absolute crap. So i decided to stop taking the b12 against my docs wishes. 4 days after stopping I am still having crazy anxiety and cold hands and feet. I had such a bad panic attack yesterday that I went to Urgent Care to be told that i needed to see my doctor which I am seeing on Wednesday, so not helpful at all. Being obsessed with health and supplementation, I decided to research b12 myself and have gone down a large rabbit hole. Turns out the b12 I have been prescribed is the worst one to take out of the 4 and is potentially really damaging me. I have a life long history of anxiety anyway and whenever i have illnesses I get attacked in my gut. What is happening to me right now is all stemming from my gut from these horrible pink Cyanocobalamin pills. Would you recommend I get a genetic test to see for sure which of the b12s would work best for me?     Sounds like you are in a tough spot. I can say that most people can process the cyanocobalamin just fine. You probably just need to explain your concerns to your doctor and take it super slow. Good luck

I’ve actually moved away from conventional doctors and now seeing a holistic specialist who I love and know she’s going to get me better. I’m not touching b12 until I get my mental health under control

    If someone is not able to absorb b12 orally, will their b12 serum levels still go up as a result of supplementation but not actually be able to be used by the body? How do you determine whether oral supplementation is absorbed?

    Last year i used to take 1500 mcg methycobal b12 per day and 1500 mcg per day i felt better now i got feet burning or tingling and weakness again im looking for high dosage of b12 to take once a day because i do take magnesium also .. can i take desighn for health brand. Ultra b12 -folate 2000 mcg b12 +680 mg dfe folate? And recently i got acid reflux so i drink apple cider vinegar too 2 times a day is it ok to take b12 and magnesium 1 hour after apple cider vinegar?     You had acid reflux and then u have apple cider vinegar … 😂why in the world would you do that!!!! It is so acid you need to take baking soda in like 600 ml of water and that would stabilize the pH of your stomach!!! Remember you have to own a body that is alkaline but alkaline is a balance 🤓🤷🏼‍♀️

    I have been supplementing with methylcobalamin for years and had bloodwork done and serum level was 1425. I feel like I have symptoms though of B12 deficiency. I have read that you can still be deficient in B12 even if blood serum is high. How can one be absorbing it enough to get in the blood stream, but not get it in the cells to be utilized properly? Obviously, I am absorbing it orally if it is getting into my blood stream, correct?

    @@jackiewatson5665 Hi, it is amazing you say that because doing more research I have come across Vitamin D deficiency countless times and you are right it is very commonly low and some of the symptoms definitely line up. I have been taking D3+K2 5000 IU daily for about a month now and feel I am improving with some of the symptoms. I think you are absolutely right. Appreciate your insight!

    Being a gastric bypass patient, I want the best bang for my buck. 😊 I found drops that have Methyl. Adenoseo, and Hydroxocobalamin would this be beneficial to me as a sublingual? Cyanocobalamin is just too available and I don't think it's a good option for me.

    I bit disappointed in this video. If you have a problem known to be caused by B12 deficiency, then you're already talking to a doctor. But most people, including vegans, are just looking for the "best" source for general nutritional needs. I wish you would have talked about that instead.

    No, I mean the "best" type of supplement to take. I had to put "best" in quotation marks because it seems there's no general consensus as to what type is best. Some say that cyanocobalamin is best because it's the most stable. But it also adds cyanide to your body. Not a good thing over an extended period of time. Some say methylcobalamin is best because it's more natural. But one "expert" (a toxicologist MD) says it will greatly increase the amount of mercury floating in your body if you have mercury teeth fillings. So is hydroxocobalamin the best? Some "experts" say so. I was just wanting yet another opinion. Thanks. @@swintegrative
