Main reasons this happens. 1. Companies and captains who are negligent, and overcrowd the boats making them severely unbalanced and too heavy to be stable in wavy conditions even if the waves are not big. 2. They go into conditions they could sustain but were not properly designed for that, with water drainage system to drain the water that waves deposit on the boats quick enough without causing stress and adding weight for too long (this is actually a reason that a Roll-on-Roll-off boat in the UK sank decades ago and was called the British Titanic). Drainage systems are extremely important, that's why fishing boats in shows like Deadliest Catch can have absolutely massive waves crashing on them dumping humongous amounts on water on the boats and they are still stable and floating without a hassle. 3. Negligence maintaining the boats, a leak starts and pumps suddenly fail and the safety system like closing sections of the ships off, and having good ballast tanks, because making money is more important than having good reputation for keeping people safe, because you know, keeping equipment and people safe and operational is too costly and the CEO might not be able to but his new Pagani, Bugatti or private jet if he spends a bit of money keeping the people safe.

This is in the Caribbean, these waters are VERY dangerous as they are absolutely filled with dangerous sharks, this is a place you definitely DO NOT want to swim in. I would stay on the boat until was absolute NOT safe anymore in any possible way before jumping in the water in the Caribbean seas.

Did an island cruise in Turks & Caicos some 25 years ago on a skiff that at one point got hung up on a sandbar. As it was a rum-punch-inclusive tour, within half a minute every guy onboard (and a few of the ladies) had leaped over the side and collectively managed to heave-ho us free. Then we all climbed back aboard and celebrated our can-do success with another round (or several!)

I went out on a fishing charter a few weeks ago and it was rough. They almost didn’t go out but it looked like fun to me. We where maybe 2 miles offshore not sure but could see land a long swim away. My daughter unfortunately got extremely seasick from this trip. They where by the looks of it a short swim away and all had life jackets. I would have gone for the swim instead of possibly getting stuck under the boat. Have to take into account the direction the currents the water was going. What a sad way to go. People who can’t swim shouldn’t go on a boat anywhere.

Almost the same happened to me in Thailand. I was on the same type of ferry and the boat pilot was kinda bad I could tell right away as he collided with another boat when the ride started. When we were about to go back to the island we came from we got stuck on a boulder or something and he just kept flooring the engine until the ferry tilted towards one side. Luckily we were close to the harbor so some random dude with a speedboat came by and rescued us.

You've got to figure that in this situation the #1 danger is being trapped inside the sinking boat and drowning. So, in order to alleviate that risk, the best course of action is to NOT be inside the boat. Especially when you're that close to shore, already have lifejackets on, and are in relatively warm water.
