Verifying consultant’s signature confirming details above: - what about this part, does my consultant need to sign there in every pages?
Hey. Once the crest form is complete who do you send it to? GMC or the residency program you are applying to?
But fy2 is rotational so which supervising consultatnt will sign it, or we need to have multiple crest form signed by each department consultant?

Anyone can get a CREST form signed. But the question is ‘can I apply for training?’. This depends on eligibility criteria
