At the AGO's library, the librarians also associate other metadata info to books like - mentions to works and artists in our collection, exhibition history, collections of other museums/galleries, conferences/exhibitions/biennales, donors, and art-history specific keywords and subject matter. Soooo amazing it makes my research so much richer and smoother.

Except what you're talking about here is data and not metadata. The main metadata you'd find in a library book is from the library's stamp to say who owns that copy of the book. You'd also find a date stamp of when the book was due to be returned. That is metadata - as would the Dewey classification number for it.

You have a very humanistic view of meta-data. To me, it's much like markup language that works in the general Internet. In that context, metadata is information that marks up, or adds significant value to, some data that preexists.

Spoken like a true cataloger :-) As a Systems Librarian most of my day is either creating metadata or fixing the connections in the system that uses that metadata. Yes there is LOTS of metadata about people in their accounts. Library patrons for example.
