Push-ups are great upper-body and core stability exercises. It can also be a good screen to see if the upper body and lower body are connected through a stable core.
The push-up screen allows us to see if the person can automatically stabilize the spine when pressing off the floor. If not, then we should focus on creating more spine stability in a plank or hands and knees position. For some people that have a lot of spinal mobility such as this case, you should start with basic back/pelvis awareness drills so they can better create lumbopelvic positioning and stability prior to moving the limbs. This stability of the spine is critical to avoid injury and stress during higher levels of activity.

    Shouldn’t the elbows positioned much closer to the body? Seems like they perpendicular to the torso which is less effective

    Perpendicular is correct, but with wrists, elbows, and shoulders aligned

    Her head needs to be in a neutral position, not extended. The extended neck is adding to the instability. Once the head is in neutral, a 3ft dowel can be placed on the client with 3 points of contact: back of head, thoracic spine, and tail bone. Space between the lumbar and the dowel is approximately the width of the client’s hand. Client has to keep the dowel steady while performing the birddog movement. The dowel falls off of the client loses the core. (Cotton clothing makes the exercise more doable as spandex is slippery causing the dowel to slide.)
