When I saw this film for the first time, I understood why Andrew pretended to regress at the end. All the descriptions of lobotomy were about making the patient feel no pain or anything else, and, once he remembered what had happened to his wife and children and the guilt he felt about it (since he believed it was his fault for not getting his wife help when she talked to him about the "insect" in her brain), he didn't want to feel anymore. But I also agree with Anita's sympathy for the doctors and all the effort they went through. At a time in history when psychiatric medicine was treating patients horribly and jabbing icepicks through their eye sockets to lobotomize them and keep them docile, these doctors were trying to find a new kind of therapy that would leave those barbaric days behind and treat patients like human beings. Andrew was their last chance to prove their method worked and get funding, support, etc. to keep developing it. Had they been successful with him, it could've saved not only Andrew but also countless future patients who might've been helped by the new methods rather than subjected to lobotomies, shock treatment, and the inhumane treatment that was common at the time. Tragically, though, Andrew felt like he couldn't live with the knowledge of what he'd done so he pretended it failed to save himself pain, but thereby doomed the patients who came after him to the old ways. And you know Andrew knew what he was doing and hadn't regressed because of his final quote to Dr. Sheehan/Chuck: "Would it be better to live as a monster or die as a good man?" He was telling him indirectly what he was doing and why. You can see in Dr. Sheehan's reaction that he heard what was actually being said too. When Andrew first says the bit about "We gotta get off this island, Chuck" and seems to have regressed, Sheehan looks stricken and then looks over at the other doctors and shakes his head, signaling to them that the experiment didn't work and the lobotomy would go ahead. But then, just before he gets up and heads off to be lobotomized, he hits Sheehan with "live as a monster or die as a good man?" line -- a bit of dialogue that, if he had truly regressed, would've been a very random question but not so if he hadn't regressed -- and Sheehan looks at him again, this time confused and uncertain if what he thought was being said was actually being said. But then Andrew gives him a knowing look as he walked away that lets him (and us) know that he hasn't regressed and he knows exactly what he's doing. He's choosing to be lobotomized so the pain of his memories will stop and he can just live as an emotionless zombie. In that way, the body lives on but Andrew - the man he was - is dead. I also thought it was interesting that you mentioned his desire to kill Laedis because, at the end of the day, by pretending to regress and letting himself get lobotomized, that's exactly what he did. He killed Andrew Laedis by letting them remove his humanity and will -- the things that made him a person and gave him his identity -- with an icepick through the eye socket.

To address your question about whether Andrew was crazy the whole time, the answer is yes. All the stuff they told him at the end was true. On a second watch - once you know the twist - it becomes much more obvious that the staff of Shutter Island aren't part of some evil conspiracy; they're humoring a patient and roleplaying with him at the request of the doctors to let him play out his fantasy in an attempt to bring him back to reality when he follows his delusions to the end and they lead nowhere. The actors in the movie did an amazing job because, on a first watch, they all seem sus or scared because that's the expectation that's been set in the audience's mind. On a second watch, you can see that they're just normal staff members playing along, and the reason they seem like they're lying is because they're playing along with his delusion, the reason they seem nervous and a little sketchy in their answers is because they're just regular people and not particularly good at roleplaying, etc. Even moments like when "Teddy" and "Chuck" first arrive on the island, we see the guards all looking at him harshly and holding their guns very aggressively. Because, at that time in the movie, we believe he's a US Marshall and there's something weird happening on the island, we interpret that as the guards being unnecessarily aggressive toward a cop investigating something they don't want him to investigate. In reality, they're acting that way because he's a dangerous mental patient that the doctors are letting run around and pretend to be a cop as part of some new, radical therapy and they don't know if he's going to get violent and hurt someone. You also see subtle moments like when the guard tells them that they have to surrender their weapons when they first arrive on the island, "Chuck" struggles to get his holster open. Why? Because he's a doctor roleplaying as a Marshall and isn't used to having a gun or unlatching a holster. Also, if you look closely, you'll notice his badge is upside down. True, along the way, there are a lot of surreal moments that seem hallucinatory, and not just in his dreams. And they are hallucinatory, because Andrew is not seeing reality as it really is. If you watch carefully on a second viewing, you can see moments like when "Chuck" brings the patient who wrote "RUN" in the notebook a glass of water, the glass is visible in one shot, then, when she's drinking it, there's no glass of water in her hand (she's just miming drinking), and then she sets down a glass with water in it. This is a clue to the audience that Andrew is an unreliable narrator, and his mind is blocking out images related to his painful memory. In the example mentioned above, he's not seeing the glass of water in her hand because his mind is connecting it to the memory of finding his children dead in the water of the lake, and so his mind is blocking out the image of water glass in the woman's hand. There's a lot of those moments throughout the movie, most attached to fire or water, all relating to his painful memory of the tragedy and his story surrounding it. Even right from the beginning, when he's getting seasick on the ferry. The sight of all the water is making him nauseous and uncomfortable because it's connecting to the memory of the water in the lake where his kids died and his brain is fighting that memory down. I definitely suggest watching the movie again if you haven't already because you pick up a lot more of the subtleties on a second watch and it starts to make way more sense what's real and what isn't.

Thank you for reacting to one of my all-time personal favourite psychological horror drama films which is a rarity of Martin Scorsese to direct given his high profile for Mean Streets, Taxi Driver, Raging Bull, Goodfellas, Casino and recently Killers of the Flower Moon. It took me 3 times to watch the film to see the signs that Teddy Daniels/Andrew Laeddis (cool fictional character names) is not who we thought he is. At the end of the film, his last words “What could be much worse in this place: live as a monster or die as a good man” is an indicator that Laeddis opted out for the “lighthouse icepick” due to the fact he got to the point of no longer handling the trauma of killing his wife for killing his kids and not wanting to regress restarting the entire cutting-edge roleplay all over again. As much as I know that it was all a roleplay: I loved the scene of Dr Rachel Solando in the cave, got a feeling it’s real (yet Dr Cawley brushed it off as fake hallucinations in Laeddis’ mind) as I could see this play a role in the plot of a continuation miniseries that I call “Escape From Shutter Island”. No matter if he was thinking of something in the past that’s true or not - the US army liberation of Dachau (among the early Nazi campsites to appear in the Holocaust) resulted in the soldiers and surviving Holocaust prisoners participated in a revenge killing (execution squad-esque) on the camp guards except those who faced the music in the criminal courts. It did happen in our world. An investigation conducted by US army Lieutenant-colonel Joseph Whittaker was suppressed by US army general George Patton and a Bavaria governor to cover up what Laeddis described as “not warfare but murder [on both sides]”. I do believe Laeddis’ involvement in the Dachau liberation and revenge killings was true.

Shutter Island on a first watch seems like the most evil and messed up place, but then you realize that they actually try their best to treat some of the worst or at least most violent patients the mental institutions have to deal with (and at the time of the movie any medication like Chlorpromazine was just being tested -1952 is when that one came out)

Defense mechanisms activate when reality becomes incompatible with life as a way to protect oneself from death. Figuratively speaking, the brain understands that the organism is in danger and takes measures to protect it. The doctor, by removing the defense mechanism, essentially killed Andrew, who was unable to cope with reality. He wanted to help him, but apparently he didn't fully understand how it works. In those days, psychotherapy was only in its infancy. Without protection from the damage caused by the awareness of the events that had occurred, Andrew decided to end it, and that was what his madness had been protecting him from. He chose death as a good person over life as a monster.
