Extended Depth of Focus (EDOF) lenses are implanted at the time of Cataract Surgery to improve a patient's far and intermediate range vision. The two most popular EDOF lenses in the U.S. are the Vivity and Symfony IOL's. Which one is better? Watch this video to find out.

    I remove more Vivity for dissatisfaction with vision than just about any other IOL. The quality of vision based on modulation transfer function and mesopic glare vision is far worse with Vivity than Symfony and there is plenty of data on that. Symfony is virtually identical to a non aspheric monofocal for higher spatial frequency modulation transfer function (and better under mesopic conditions with a 5mm pupil) while Vivity is actually worse than a multifocal in this regard. The Vivity does have less starbursts/halos than the Symfony but quality of vision isn't close between the 2 lenses.

    I had terrible nighttime dysphotopsia for a few months after Vivity replacements in both eyes. After 5 months it is much better and I feel pretty comfortable driving at night. I’m happy with them now but it took a while to adjust to them. By the way, one surgeon said I did not need toric lenses and the second surgeon I consulted with said I would need toric lenses. I had toric lenses implanted and I wonder if maybe that’s why I had such bad dysphotopsia.

    The toric IOL is not going to contribute to your dysphotopsia. If you need the toric you need it and if you don't you don't. you might be borderline but can't tell without looking at the numbers. I take out quite a few Vivity IOLs but mostly it's for quality of vision issues rather than dysphotopsia.

    Would love to see the same study comparing to the Symfony Optiblue. I’ve implanted similar amounts of Vivity and Symfony Optiblue lenses and I think both are great lenses, but I think at this point, I’d give the slight edge to the Vivity. Thanks for your video based on the data!

    I had a Vivity lens placed 4 days ago . I have no glare, halos, or starbursts. Completely comfortable. Thanks for the video.

    Sure . I am 68yrs old . Sphere: -5; Cylinder: +2.25. Non- dominant eye with amblyopia. Set Vivity lens to -.75 in hope to increase near vision . Result as good as could hope for considering limitations due to amblyopia(I can’t read small print).

    I can drive at night . No halos ; normal amount of glare that I would expect any lens would have .

    I like my Vivity. Dr, You’ve been so good at answering my questions. I just had YAG on my right eye (Vivity Toric, placed 4/2023). Ever since the procedure, I have a strange flicker/glare that circulates every now and then whenever I’m looking down and look up near a light source. Is this normal? Will it go away? It’s only temporary and then vision become clear again, it’s a noticeable flicker/glare whenever I move my head/eye, however, normally it’s when I look down and then up gain. Could this be a symptom of a pit or damage to my IOL?? It’s only been 3 days. Thanks in advance!!!!!

    Dr. - please comment on yellow vs clear lens. Article and studies seem ambiguous with no clear difference other than slight tint to vision (yellow being closer to the natural eye and warmer colors). Offhand, rose colored glasses seem better to me, joke. What do you use? Thank you.

    About 20-50% of patients who have undergone lens replacement surgery may experience secondary cataracts 6-18 months after surgery. It also requires treatment and consultation with a doctor. Secondary cataract after lens replacement This complication is more common in young patients. Secondary cataracts often develop in children who have undergone surgery to remove congenital cataracts. In elderly patients, fibrosis of the posterior capsule of the lens may be observed after surgery, but it is secondary cataracts that are observed mainly in younger patients. What is she like?

    Intraocular lenses become dislodged due to scarring of the lens capsule, trauma, lens ligament atrophy, and laser treatment for secondary cataracts. Secondary cataracts (opacity of the posterior capsule of the lens) sooner or later develop in almost all patients who have undergone lens removal.

    Just had my second (dominant) eye done with the vivity. I waited a month after my first because of slightly increased distance blurry and startburts and halos. The doctor said he over corrected for my astigmatism of that eye. Went with less for the next eye, but not much better. It's only a week out so maybe my second eye will heal better. Mid and near vision is definitely better, but distance and night (flares/glare ) is worse. I can get away without glasses mostly, but driving and distance blurry and flares etc...Would having Lasik done on both eyes help correct the distance blurriness and minimize the flares/glares?

    I'm having cataract surgery in three weeks. After much discussion and testing, my surgeon recommended LAL lens for my dominant right eye and the Apthera EDOF lens for my left eye. Anyone out there had both of these lenses implanted? Would really love to hear your opinion. Thanks

    Does the toric version of the Vivity lens have the same outcomes as the standard Vivity or does it give some halos/glare/starbursts (in patients with significant astigmatism) ?

    Good question! In a patient with significant astigmatism, a properly placed Vivity Toric would provide more favorable vision (day & night) than a non-Toric Vivity. In a patient without astigmatism, a non-Toric Vivity would provide more favorable vision (day & night) than a Toric Vivity. Of course, the lens power needs to be appropriate for the patient. If a person without any astigmatism gets a Toric lens of any kind, the astigmatism would increase for that patient (since the Toric lens is not spherical).

    @@DoctorKrad hi Doctor Krad love your concise vids. Unfortunately ive had my right dominant eye fitted with an Alcon monofocus and i have 20/25 distance vision and decent focus of my phone at arms length. Not sure what to do with my left eye which is very short sighted -8.5, cataract and astigmatism. Everyone has ruled out a multifocal lens for some reason. So choices are edof, monovision or same lens as right eye. I also find the low light contrast not great with alcorn mono lens. I play golf and ocassionally ride a motor bike, bushwalk. Ideally i would like to be reading glasses free. Currently i feel i have some depth of field issues due to my poor sight in left eye. Any thoughts thanks.

    Hi. I'm having an EDOF Vivity lens put in soon, but I'm thinking of having it done with close up and intermediate vision without glasses, and distance with wearing glasses. Do you have any experience with people choosing this and is it a good idea? Thank you.

    Like I read in one post, someone said there is plenty to see behind 1 meter than before it. I’m myopic -8-9 and trust me I wish I could see a bit more far than that. I think it’s better to use glasses for close but to see better far, but maybe to some extend??? My auntie chose to have slightly worse far vision and she has -1.75 and -2.25 after operation but she can read and see better around the house and have no glasses (she used to have -6). So she thinks the light distance glasses are better than -6 lenses. She is happy

    My eye doc with over 5K of cat surgeries only uses Clareon Vivity for the premium lenses. One eye required a Toric lens to fix astigmatism. He also added a slight amount of mini monovision/blended for extra depth of field. Vision is now 20/20 and no glasses for any distance.

    @@Rita50 I don't know the exact values between the eyes. One eye was set for sharper close up focus and one eye was set for sharper distant focus. It's very easy to test by holding a hand over each eye and checking the focus. Since he added a slight amount of monovision (blended vision) the doctor just called it mini-monovision (not a medical term). The brain (most people?) will neuro-adapt the images together into a sharp image at a wider range of distances. It's been about 18 months since surgery and no problems. I can read books/computer/TV/drive/... without glasses. Now I just wear non-prescription glasses for safety/sun.

    I would disagree with this. Synergy has very good near vision and provides a depth of focus from near to midrange but distance vision is not quite as good as Symfony or a monofocal and it also has fairly significant diffractive dysphotopsia.. I will mix and match Symfony in the dominant eye and SYnergy in the non dominant eye for someone who can sacrifice a little quality of vision in the non dom. eye for distance but I rarely use it bilaterally. I do not like the Vivity lens and rarely (never) use it and very very rarely use Panoptix because I think the Synergy gives better near vision and the material is better. @@letranger5
