Vitamin D - Vitamin D2, Vitamin D3 and Calcitriol | Doctor Mike Hansen

Vitamin D - Vitamin D2, Vitamin D3 and Calcitriol | Doctor Mike Hansen
What are all of these things?
They refer to the various forms of Vitamin D in our bodies or in the food we eat. For example, vitamin D3 is also known as cholecalciferol; this is made naturally by the body in response to sunlight. Vitamin D2 is also known as Ergocalciferol and comes from plants. Vitamin D supplements come in the form of either Vitamin D3 or Vitamin D2. If you prefer, calcitriol or 1,25-dihydroxy vitamin D is the active form of vitamin D in our body. When sunlight contains ultraviolet B (UVB) radiation, and when it hits your skin cells, it turns 7-dehydrocholesterol into vitamin D3, meaning cholecalciferol. Vitamin D3, in turn, binds to vitamin D-binding protein (VDBP) and is transported to the liver, where it gets converted to calcidiol, aka 25-hydroxyvitamin D.
Calcidiol is the storage form of vitamin D in the body. Calcidiol is later converted to the active form of vitamin D in the body, meaning calcitriol, or 1,25-dihydroxy vitamin D. This conversion of calcidiol into calcitriol mainly occurs in the kidney. But it also takes place in different tissues and cells of the immune system, such as lymph nodes and alveolar macrophages. And not only in alveolar macrophages but the alveoli themselves.
Vitamin D3 is only found naturally in a few different food sources, mainly fatty fish, like cod, swordfish, tuna, and salmon. Milk doesn't naturally contain vitamin D3, but it has been fortified with it for almost 100 years now. But other dairy products made from milk like cheese and ice cream aren't typically fortified with vitamin D and contain only small amounts. So to meet the RDA of 800 IU, older adults would have to drink about 4 cups of fortified milk per day. Vitamin D3 mainly comes from sunlight, though, not food. But the skin's vitamin D production depends on several factors, only some of which you have control.
Depending on the season, where you live, where you travel, the time of day, the clothing you wear, and the umbrella you use, all of these factors determine how much UVB light hits your skin. The sun's rays are most direct between 10 a.m. and 3 p.m. However, the farther you live from the equator, the less UVB radiation you receive. People who live north of about 37° latitude can't make any vitamin D from sunlight from November to March.
Why is this the case?
During the winter months, the earth tilts away from the sun, ultimately leading to fewer sun rays hitting the earth. Having darker skin means less UVB absorption, which means less vitamin D3 production. Also, as we age, our bodies become less efficient at converting UVB light into vitamin D3. This is why older people and darker skin people are more prone to have lower vitamin D levels. Many older people have reduced exposure to sunlight for different reasons and may not be getting enough vitamin D in their diet to make matters worse.
Vitamin D signals the intestines to absorb calcium into the bloodstream.
This happens even if you have enough calcium in your diet. Besides strengthening bones, vitamin D helps reduce fractures in the elderly by preventing muscle deterioration and reducing the chances of falling.
The official definition of a vitamin deficiency means that specific health problems arise due to not having enough of a particular nutrient. True vitamin D deficiency in children causes rickets, a bone disease where the legs become bowed. This is rare in the United States. Vitamin D deficiency in adults can lead to osteomalacia and osteoporosis, leading to bone fractures. So what if you don't have deficiency per se, but you have less-than-ideal levels of a specific vitamin? Well, this can increase your risk of various health issues, even though they are not solely responsible for these problems. This is what we call "Insufficiency."
Most medical societies consider someone with vitamin D insufficiency if their level is between 12 to 20 ng/mL (30 to 50 nmol/L), and Vitamin D deficiency is less than 12 ng/mL (30 nmol/L). Most experts consider normal levels to be above 20 and less than 50 ng/ml. Most people in the United States have values around 20 ng/ml.
But having too high levels can cause other health issues. In one study, levels above 32 ng/ml resulted in people getting less quality sleep. The risk of vitamin D toxicity typically occurs at levels over 100 ng/mL in adults who also ingest substantial calcium amounts. Vitamin D toxicity, whether it's from vitamin D3 or vitamin D2, generally occurs after inappropriate use of vitamin D. It's essential to avoid excess vitamin D in pregnancy because that can cause calcium levels to rise to the point of causing seizures in the mother and developmental problems in the baby.

This is a topic I believe needs more attention. Vitamin D has many roles in the body and we are learning more about it as times goes on.

Hi...what's your perspective on the fact that no medical body or government has bothered to make a major announcement telling people about the mountain of observational and clinical studies(many of them available by march) all pointing to the dramatic protective effects from Covid-19 that vitamin D appears to provide. In our opinion it was malpractice to not mention it in march, but it is now bordering on criminal. Perhaps you have a different take as somewhat of an insider?

  • @FriendlyPharmacy5

    @FriendlyPharmacy5

    3 жыл бұрын

JustNewsNetwork Well I wouldn’t call myself any kind of insider however, We know that vitaminD is not a cure for Covid-19 so there may be some apprehension around recommending it and then having people think it is a cure and they neglect to take other precautionary measures. Just a guess. Also, right now the focus is more on finding a cure, or a vaccine. Dr.’s and other officials will be hesitant to accept anything but a good RCT before declaring the effects of vitamin D as truth. This pandemic has been riddled by misinformation and so there is maybe even more hesitancy right now to communicate about things we aren’t absolutely certain of, without a randomized controlled trial. I realize the evidence for the role of Vitamin D in immune function is increasing, this is just my guess of what may happening. Just a few thoughts there, just an opinion, again, I’m not an expert.

Just had my D3 level checked at my annual physical. My level was 50 nmol/ml I've been supplementing at 3,000ius daily for the last 3 months. Prior to that I had supplemented at 2,000ius daily for years using the gel cap form. I'm biracial, German/Nigerian with medium brown skin and live just north of Chicago. I control portion size strictly, drink no soft drinks, eat very little sweets as I'm still in a project to reduc my weight from 270 to 190. I was an office manager during my working years and lead a very indoor lifestyle in retirement. I'm 66, would say I'm in very good overall health, stable 195 lbs, with no joint pain of any kind and no other health issues. I also supplement with a generic mega-multi vitamin and a turmeric capsule. I have been taking the turmeric for more than 20 years, a multi-vitamin religiously for more than 40 years. As an asthma/chronic bronchitis sufferer I jumped on an ad-hoc study by some local area nurses using D3 supplementation in an effort to lower worked missed due to colds. The study showed an 80% reduction in lost time because of the supplementation. I've gone from a cold a month, bronchitis 2-3 times a year, with constant nasal drainage to almost perfect respiratory health since I added D3 to my regular asthma meds almost 10 years ago. I highly recommend that everyone supplement 1,000ius until you get your numbers checked, unless you have a contraindicating medical condition.

Arthur Dick, you are totally not correct sir, as you recommend do you own research into Dr. Campbell’s credentials, he has extensive nursing experience, the middle part of his career was a nurse educator at Cumbria University, he has published nurse educational books and he supports nurse education in poorer nations. The man is not a Government mouth piece at all, far from it, he has medical contacts throughout the world due to his educational respect and status in that field. You do this genuine medical educator a disservice, but that’s your loss.

WOW, and thank you again. Living in the far North of the US, there are jokes about it being "Vitamin D time!" To hear you say "You can NOT just get enough from sunlight!" It seems that everyone has to take vitamin D supplement in the winter. But, almost every doctor I know tests for this, and you get the "So, it's time for the vitamin D!" A few times I've been low enough, to have a kind of BUMP week to force it back up for a while. A few of my friends argue that NO ONE needs to take a vitamin, but most of my senior friends tend to take a vitamin in the winter. Then summer hits and most of us are fine. Thank you again.

Thank you Dr. Hansen! Your presentations are wonderful. I live in northern Indiana where we have many cloudy/rainy days. It was good to get your recommendation for proper dosage as 3 years ago I was determined to be deficient. I have convinced my mother to start supplementing also. Doctors should know the Vit D status of EVERY person who contracts a severe case of COVID 19! In fact doctors should ask dozens of questions to those afflicted so that the citizenry can, with time, know ALL the risk factors not just the ones that are obvious, e.g. having COPD. I'm looking forward to your next presentation.

Vitamin D is a very important one. When I moved to Canada in 2015 from a tropical country and Winter kicked in I started suffering depression. There where many factors as I had a lot of stress and some other problems. I never felt like not wanting to do anything before. So I quickly asked for mental help and the Psychologist recommended a Vit-D3 tests, my levels where low. I started supplementing and sun bathing and my depression went away in three days despite all the stress and other issues getting even worse. I'm not of very dark skin so I can imagine how thing can be for other people. I also understood why people living in northern latitudes get depressed so easy, as depression is quite uncommon in tropical latitudes. Actually in my home country most people think that Europeans and North Americans get depressed because they are "soft", but maybe is more related to their metabolism.

@Citizenthirteen I call it Spring fever. When my wife joined me she came in winter. She used to say, people here are so boring, they don't go out. On the first cool day, people were out like crazy ants. I told, her Spring fever is kinking in, it will only get better. Our schedule was so busy we had to refuse a few invites. Sadly, this Spring-Summer season is going the horrible. My BBQ is still cold :-(.

@Henrik Wallin I get your point but for example here in Canada I fear doctors and politicians won't talk about it for fear of backlash. The moment they say that if you have darker skin you should be more concerned about your Vit-D levels a lot of SJW will call it on racism. It is sad that when there is an objective issue like this politics cannot addressed it properly because of political correctness and a few of crazy people that deny simple facts. I never saw in my home country so much BS about race as I see here, and we are quite an interesting mix of all colors and shapes. I feel there is much racism here with all the PC BS than in my home country.
