    Best video on YT on this topic. I'd like to add something about the weird terminology here, just details but might help someone: 1) haptocorrin = transcobalamin I (and I think formerly aka transcobalamin III) = cobalophilin = R-protein = R-factor = R-binder ["R" stands for "rapid electrophoretic mobility"]. 2) The intrinsic factor receptor in the terminal ileum is also called "cubilin"; the receptor in the kidney that's responsible for the B12 reabs. is the same like the one in the terminal ileum (it is also cubilin!) with the small difference that megalin was added to this receptor (which means this receptor is more "elaborate" than the one in the terminal ileum). Also, in order for cubilin to work, you need a membrane protein that will make cubilin stay put (and will help in the endocytosis, once B12 bound), this protein being called "amnionless". 3) A mutation in cubilin or amnionless (autosomal recessive) leads to the Imerslund-Gräsbeck syndrome (syn.: defect of enterocyte intrinsic factor receptor, enterocyte cobalamin malabsorption, megaloblastic anemia 1, juvenile pernicious anemia with proteinuria due to selective intestinal malabsorption of vitamin B12), 1:200k in Finland/ Norway --> patients will have problems with both, B12 abs. AND reabs (remember, cubilin/ amnionless is present in both organs). 4) B12 bound to transcobalamin II (the transporter in the blood) is called "Holotranscobalamin" ("active B12"). 5) Haptocorrin is also produced in the stomach, not only by the salivary glands; and, as said in the vid, haptocorrin can bind 80% of the B12 in the blood making it unavailable for use.     How do we get "Holotranscobalamin" ("active B12") blood tests in the US? It's weird, every reference to this seems to bring up UK web pages. I can get a Vitamin B12 and Vitamin B-12 Binding Capacity, Unsaturated (Transcobalamin) blood test in the US. Would I literally have to do medical tourism just to get this test? Hahahaha.

    I like your videos very much. I guess you're studying to be a doctor, which is super full on anyway, let alone uploading videos too.     Question..... can anyone explain or give clarity on the following ... does any change to typical Purine & Pyrimidine, folate, B6 or B12 pathways during pre or early post natal periods result in heritable genetics that could be passed from that baby should they procreate in later life?

    If I had a normal to high level of Serum B12 but a very low (half the lower reference range) of Vitamin B-12 Binding Capacity, Unsaturated (Transcobalamin), is this a problem? If I supplemented orally B-12, would that raise Transcobalamin? Or maybe I'm not understanding it well enough. Is it an issue if my Transcobalamin is low, even if my serum B12 is normal? What kind of a doctor should I talk to? It's hard to get one that cares. Hematologist? (It seems like "Transcobalamin Deficiency" is a pretty specialized thing and most doctors only care if you are bleeding out so they can stitch you up. Not about complex metabolic deficiencies that might just make you feel slightly crappy or have other risk factors later in life.) I have a few TCN2 gene variants, but I don't even know what kind of doctor I can get to care about something like that. Most of those variants do not have any pubmed/research papers associated with them. 6:46 I've never taken an MMA blood test, but it's definitely on my list of things to do! :-D

    And again, the lesson here is chewing your food and taking time eating is key. I think one the reasons why I was deficient in nutrients in general was eating too fast and drinking too much coffee. The latter makes your stress levels go high which makes you chew less and faster. Oh boy!

    Hi, great video! So if I take a capsule, the b12 doesn’t get a chance to bind to the Haptocorrin, does that mean my supplement is just getting disintegrated in my stomach acid?

    That is why b12 supplements don't come in capsule form, they come in sublingual form. Which means they go under the tongue and dissolve with your saliva. Otherwise, yes, that would be the case.

    Not necessarily, I take a 1000mcg tablet and find it works best if taken with the first mouthful of food at breakfast every day. My B12 levels are around 750 so it appears to be getting there. Perhaps a tablet is better to bypass the stomach acids.

    B12 produced by Propionibacterium and this is not big problem for vegetarians. We can drink fermented coconut kefir from Propionibacterium or consume probiotics.I assume that in sufficient quantities these bacteria can synthesize B12 right in intestines.

    I wonder if I'm a poor absorber of b12 because I was taking 25mcg of b12 a day and had a little tingling in my legs and hands occasionally. I was not eating animal protien. Now I take 1000mcg a day. I might go down to 250mcg like dr. Michael Greger recommends. But I feel good now.

    I cut back on the b6 supplements as this could disrupt the absorption of b12, I believe because b6 affects homocysteine levels. I try to eat lots of healthy food and get it naturally now. I only supplement a couple of milligrams, like 6-12mg of b6. I bumped up my b12 to 2500mcg of b12 a week. I take the cyanocobalamin. I no longer get those tingles in my legs. I also do not cook with salt. I sprinkle 1/4 teaspoon of sea salt on my food a day. Too much salt is bad for circulation. I read in Mayoclinic that we do not need to even add extra salt because it is our food. I sprinkle a little for insurance because a little is vital, but adding too much adds water volume to our blood and constricts our veins which makes our heart work harder. My circulation is so much better now that I eat no animal protein. I still take a half teaspoon of fish oil a day for vein and brain health. Also, potassium rich foods lower blood pressure and help circulation. Check out the nutrition facts.org videos and get a used copy of "The china study". by T. Colin Campbell.     Most people vegan Vegetarian or not do not ingest B12 at all unless its fortitfied in the food its self why do you treat B12 as a Plantbased diet problem? If you dont wash your fruits and vegetables in nature (No GMO's and no pesticides ) you'll get B12 or if you eat animal large intestines.... And you dont need that much of it.

    Read up on "Pernicious anemia". Get a B12 injection, take a B12 tablet daily with your first mouthful of food each day. Eat liver ...lambs ( lightly fry slices, dusted with seasoned flour ) liver has a greater concentration of B12 than beef.

    My blood test shows that I have over 1300 (PG/ML) of B12, my bilirubin over 1.7 (MG/DL), and high levels of bad cholesterol. Every year when I see those High levels of B12 on my yearly physical exams I ask my Doctor if that is OK, and I always receive the same answer. "No worries It is fine." I have a feeling that it is not fine.

    It's okay! I study B12 and having too much B12 in the body is harmless. It stays in the system a more than other vitamins because it cannot be produced in the human body and we must obtain it from external sources. It also loves water (and we are 70% water) so having some of it residually in the body is to be expected.

    My b12 is 240 and I’m tired. All the time. I’m not even vegan. I have acid reflux my stomach may be to acidic and killing the b12. But it’s strange.

    Acid reflux is usually caused by too little stomach acid: the LES(lower esophogeal sphincter), that sits between the opening of your stomach and the end of your esophagus, needs acid with a very low ph(high acidity) for it to close properly. If there's not enough acid, then it 'thinks' that there's no food being digested and that it can relax. Gut dysbiosis can cause a whole load of gastrointestinal issues - including acid reflux or GERD(gastroespohgeal reflux disease). For example, SIBO(Small Intestinal Bacterial Overgrowth) is when bacteria overgrow in your small intestine, they produce gasses that force the LES to relax. In addition, H. Pylori may actually directly cause SIBO: since it neutralises stomach acid by the means of production of ammonia by catalysing urea by the enzyme urease. In simple terms, gut imbalances make the usual acidic environment of the stomach less harsh and more alkaline, signalling the LES to open and back up acid or food contents into the throat, causing Heartburn, GERD, acid reflux and etc. Since strong acidity is required to absorb certain nutrients, that also explains your B12 deficiency.
