Arcuate field defects can only be explained by the axotomy of nerve fibers in primary open-angle glaucoma.

Earliest changes take place in the periphery (Isopteric contraction) not in the central fields, no?

Field defects are not noticed separately for each defect, as these form very slowly. The early field defects like isolated scotomas, Seidel’s scotoma and superior arcuate scotomas are not identifiable by the patient. Sometimes, a person may become aware of missing stairs or difficulty in reading if there is a significant involvement of the inferior field, or may notice an inability to appreciate objects from the sides while driving, if there is significant constriction of the peripheral field. In most cases, the person notices a constricted field when it is advanced, & has almost tunnel vision. However, symptoms such as difficult in night driving because of reduced contrast sensitivity may be there in the early stages.

When vision is lost all round, and only a small central field remains, it is called tunnel vision. It is as if one is looking through a tunnel or a straw. It is due to destruction of all the nerve fibers except those in the center, that signs papillo macular bundle

@@yoyo-ps7yd Unfortunately vision that is lost cannot be recovered; however the good news is that treatment can prevent worsening of the condition and preserve whatever vision that is present
