    I had an excellent physical therapist in Portland, OR. She taught me to think of my pelvic floor as an elevator. You lift that elevator to the 1st floor, then tighter to 2nd floor, then tighter to 3rd floor. Also for the urge incontency she said to tighter for the count of 5 and relax to lower the elevator down each floor for the count of 15. I wanted to request a video of pelvic floor recovery after a TVM & Sling removal.

    Excellent interview on a much needed but little publicized topic. My father-in-law experienced problems and consequent social isolation with this. Bob, your interview style was relaxed and enjoyable and Sarah offered much valuable information. Kudos to you both.

    That was a very good informative piece. I didn’t know that one would see a physical therapist for this type of concern. Sarah spoke with strong ease and gave good pointers. A topic people don’t really want to talk about, yet, people are having problems and need help.

    One of your best episodes. In my early thirties (35 years ago) I was told to do high reps of 30 seconds, with extreme effort (making all the wrong faces Dr. Sarah talked about). It was a bad experience and I didn’t keep at it. If I had heard her recommendations, and the finesse of the muscle coordination, it would have been much better. Wish I had known about this sooner. Will try again with a better guide. Thank you.

    I decided I'll share a little thing I do. After voiding I say the twinkle little star poem and can void again, and that means I can wait twice as long as if I don't. My cat comes into the bathroom with me and tales my concentration , & that's why I chose a poem, to make me concentrate and stay there a little longer.

    Hello everybody and still on the subject of urinary incontinence, I was quite surprised about the high number of comments. Yes, this special underwear is quite expensive but it should be covered by some sickfund (Europe) or medical insurance (USA). I live in the Middle East and I get compensation for these expenses from the National Insurance Inst. And further, I saw a complaint about this underwear being "bulky": if you have the right size, it should not be bulky and not be "noticeable". Yes of course whenever I leave the house, I allways have 2 spare padded brieves in my (quite big!!) handbag, that is a "must". Apart from all this I am fine and I recommend all the"incontinencers" to stop being embarresd and keep smiling !! And greetings from the Middle East 😊

    My incontinence ended when I realize it was a result of taking too much magnesium. It was in my calcium blend for bone health. Once I stopped those that and found an alage calcium with lower amounts of magnesium, the inconvenience stopped.

    Magnesium is essential to the successful operation of the human body and most people seem to have a shortage of it in their systems. However, I learned the same thing. If I took only 1/4 of a teaspoon of it in water 2 or 3 times a week, that seemed to do the trick without causing bowel issues. I used the powdered form of mag. & ccium blend in a product called 'Calm'. If I overdid the amount it would cause the same "run to the bathroom" issues. I got that product from a vitamin store or it can be indexed online at health food, vitamin sites.

    I went to a Urologist/gynecologist! for both incontinence and UTI’s. I was told about the level exercises, of course but that the UTI’’s was pretty much all in my head (she said it in a nice way) but I can tell you that when I get a UTI, it is very painful! /uncomfortable. I just wish these problems could be resolved!

    Thanks for addressing this and not in embarrassing way.😁 I first noticed it when I would sneeze or cough. I had endometriosis and had hysterectomy 10 years ago. I talked to my doctor and she just said "It's normal" and no other info.

    Just because something is common, doesn't mean that there is nothing that can be done to help. Pelvic floor PT helped improve my QOL immensely after several abdominal surgeries that left me with pain.

    I just saw this on FB today! Was trying to figure out how to contact them regarding this issue! I am so happy that it was discussed today again. I see that it actually was from a year ago. But still very timely. In all my years as a patient of many Ob-Gyns - not a one ever mentioned urinary incontinence nor Pelvic Organ Prolapse to me or had any literature in their waiting rooms. I did have surgery for the Pelvic floor prolapse & they took my uterus also ( I am 76 yo) . Now in the past women would just say they had a Hysterectomy but not mention that they also had a pelvic organ prolapse repair! & we are still afraid to Talk About it & incontinence !! This has got to change. It is too late for me but not for others! Look at all the items in the stores for incontinence & All the ads on tv! It is disgusting that more research & fixes are not available for these issues! Thank u Bob (& Brad) for talking about this & getting this out there!

    Is "positional incontinence" a thing? I seem to suffer more upon standing first thing in the morning. Doesn't happen often. I did the incontinence underwear for a while but prefer not to use them.

    I am surprised that Sarah Haag, and this video, do not discuss BPH, where as the prostate gland grows as men get older, and the prostate gland squeezes on the bladder.

    I heard someone describe the kegal exercise as: sucking in your belly button and at the same time pulling the rectum and belly button towards each other. Ever since that description I have no trouble doing kegals at all.🤗 yay!

    I didn’t realize you had a podcast, and now I’m subscribed to that, as well. I checked for the book on Amazon, and it would be really nice to have it on Kindle! I’ve been dealing with incontinence since my 40s and I’m almost 70. I do kegels and they help, but running and walking can still be a problem. I tried medication that seemed to work for awhile and I’ve been to a PT. The worst thing about this is not being able to take my dog for even a 30 minute walk. Sometimes I have to stop the treadmill to run upstairs to use the bathroom because I’m leaking!! I’m not finished with the podcast (listening on my cellphone) and hoping something there will help!

    In USA, Poise pads work best from my years of experience. Best for absorbtion, smell, staying where they should. Paper pants with pad inside at night works best for me

    @@annewatman677 Hi Anne. So good to see your post! I have learned a lot since then and will share a few tips. I am still having leaking issues but have had some breakthroughs that I think have helped. I am scheduled to try some specialized pelvic PT again, so great timing in your response. 😉 Here are my tips to try: 1) oil of oregano; 2) black seed oil; 3) manganese improves the bladder muscles; 4) not sure if these are for bladder directly but affect muscles: increasing magnesium (I used glysinate form - about 400 daily - ramp up slowly - good to take other forms also) and B1 (Benfotiamine form). Beware 1) and 2) may cause needed detoxing effects also, so start slowly. The number of near miraculous comments I found on both of those in YT comments pushed me to try them out and I agree they are true natural gifts from heaven. In general, to learn more on any of this, I search in YT with something like " natural remedy magnesium" and then search for more in YT or another search engine from tips I learn there.

    @@bajone02 Thanks, Barb! I appreciate the info. I’m so frustrated because I’ve increased the number of Kegels I do daily, and it doesn’t seem to help. At one time I saw a PT, and it’s been so long ago, I honestly don’t remember if that helped. The one thing that really upset me was recently I just started leaking without even feeling like I needed to pee!! I know I have to be careful with Magnesium because of some kidney issue, but I’ll definitely check out the information you provided. Thanks again.

    Thank you for this video, it was very informative. One thing interesting to me especially was that I may be causing my bladder frequency by habit, going often as a precautionary measure. One problem is that we don't have enough public facilities in this country. In fact we don't have public facilities at all. Most of us depend on bathrooms at McDonalds .

    Public facilities are anywhere you need to go along rural portions of roadways! In the city, guys can hide it more easily than gals. If you’re female and have to go in the city, there’s Depends, but then there’s the issue of hiding it, or if it leaks.😢

    "Restrooms for customers only" is cruel and inhumane. If your business model relies on restricting potty access, get another business model. Norms can change. Restricting restroom access and water fountain access (hydration and hygiene) did not help control the pandemic.

    I discovered that urinary incontinence was completely a part of my diabetes. If I ignored my diabetes and ate whatever sugary thing I wanted, I would have a full bladder-emptying accident. Just a few too many carbs and I will leak a few drops. When I keep my carbs to 12-15 grams a meal, I have no trouble at all. When I think of my years of suffering before I made this connection.

    Really, allowed only as little per meal! Can it possibly be increased over time? It seems once Diabetic, sugar vengefully reveals its true poisonous colours...     Frequent urination can be the result of suppressed metabolism for example after pregnancies; leading to lack of urinary retention hormones. In my situation this was aggravated by metal toxicity due to dental fillings, and drinking far too much water. After implementing various methods of detoxification , my symptoms were greatly improved. Eventually found most potent dry fasting, and IBT (Inclined Bed Therapy)

    Important to also keep in mind that low levels of either Sodium and/or Potassium may lead to frequent urination. When it comes to considering Calcium (causing muscles to contract) and Magnesium (causing muscles to relax): Too much Calcium may lead to an over-active bladder, while too much Magnesium may lead to lack of retention, since the bladder's sphincter may be too relaxed.

    Corresponding to the topic, my elderly mom had this issue. In the middle of the night she walked to the restroom and didnot make it. She slipped stepping in her urine, fell and hit the ceramic toilet bowl and broke three ribs. Sadely she ended up being taken to the hospital.

    Also this happens due to hormone imbalance. If you're still having trouble I would fide an endocrinologist that would test your hormone level and then prescribed a cream that had a plant-based estrogen and progesterone and testosterone compound in a cream you rub on your inner elbow or behind the knees cuz your body can absorb it that way. Read Suzanne Somers book about menopause she talks about this in there. And the way you find an endrocrinologist is to find a compounding pharmacy and then they can tell you some doctors names that prescribed the hormone cream. I found Leesburg Pharmacy in Leesburg Virginia and they told me about dr. Lee. The cream he prescribed had estrogen and testosterone in one tube and progesterone in the other tube

    Im a male. Can you explain why moving increases my urge to go once i have a small amount of liquid in my bladder? This happened suddenly its like my body became hyper aware of my bladder. Also is it normal to feel a sensation by your sphincters if you press on your bladder the more your bladder fills

    I was surprised that you don’t recommend stopping urine flow to identify the muscle. I learned to use this method during childbirth classes with the caution not to practice this beyond identifying the muscle. Now in my 70’s I still use this exercise to help with bladder control.

    She does mention not OVER-using the technique of stopping & holding back urine flow, because that can sometimes physically confuse the bladder and pelvic floor muscles. From menopause on, I've had trouble stopping the urine flow, and so would try harder and longer. Looks like I might have some other problem--since it happens whether I've had caffeine etc or not--so I'd better ask at my next annual checkup.

    My urge is most reduced when I take in a lot of lemonade. 12 ounces of lemonade with apple cider vinegar each morning and I can go several hours without any urges. Coffee keeps me awake but tends to increase urges a lot. Water mixed with the lemonade is good but water straight will increase my urges. Also, heat reduces urges and so does fast walking.

    seems like I have to retrain myself to go at the first urge - because I won't make it if I put it off. Problem is, I almost don't notice that first urge. In situations where the bathroom isn't easily available - out shopping, say, I'm more conscious of my bladder, and I do better. Thanks for this video!

    The advice someone here gave: 'DON'T LET YOUR BLADDER CONTROL YOU' makes good sense though: Not to give in to go with every urge. The more one goes, the less inclined the bladder will become to hold more? TRAINING YOUR BLADDER is like facing a dragon 😅

    As a child I'd laugh so hard the bladder would go.....too. Floppy bladder for nurses who don't have time to go to the bathroom, delay in voiding..? Hypothyroid disease can alter, I suspect. Decreased in nerve conductivity. Stress incontinence started during pregnancy which ever left. Thyroid help run the kidneys too, hence, myxedema with severe hypothyroidism. 2 liters plus 2 cups coffee.... premenstrual release of fluid running when getting up in am dripping big time... not thru perimenopause 59...yet. Thanks

    What about neurogenic bladder? Since there is nerve damage including paraparesis in my legs will pelvic floor exercises do any good?

    When I was first married in my mid 20s I got a lot of urinary tract infections. One urologist I went to said that if he dilated the end of my urethra that would help. I did submit to this procedure. I regret that! What I eventually figures out is, if I washed my genital area before and after sex I never ever got another urinary tract infection. Too bad this was never told to me by Any of Many Drs I saw. I now wonder if this procedure has brought on my incontinence?

    Yes, I believe there's a strkng connection with good hygiene. I used to get cystitis a lot in my 20s and 30s, until I was advised to wipe from front to back after a bowel movement, which seemed to stop the problem. My GP explained (tongue in cheek) that women have a design flaw, in that the openings of the rectum and the urethra are too close together, allowing bacteria to travel too easily from the former to the latter.

    Best advice to avoid these stress inconsistence issues from leakage from coughing laughing sneezing is to have c section when pregnant to avoid these, my friends father was a OBGYN dr. He said to avoid these issues. I wish I had listened and now I struggle with stress inconsistence. By having a c section you avoid damage from birth to the pelvic floor muscles. Did PT for damage temporary fix.

    I fall under the urge incontinent side & have had this issue since menopause in my early 50’s. I’m 72 now. About age 55, I started wearing incontinent pad protection & in the last 5 years I’ve had to move to the #5 out of 6 sizes & change them 2x & 3x per day/night. It’s getting expensive. I tried kegels but did not feel they helped. Once I start the stream, I cannot stop & that includes on the way to & on the toilet. I really don’t have much urine left to stop when I get to the toilet yet I can’t stop the stream doing a kegel. I always feel I have to pee. That sensation never stops. Even when I’ve emptied my bladder I still feel like I have to pee. If I even think I have to go, the stream starts. Then I say I won’t think it, I’ll just go every time I pass the bathroom but the stream starts & I can’t get my pants down fast enough. All I wear are black slacks bc I still have accidents at times bc the pad is not enough. At night in bed, I’m up every 2 hrs, sometimes less. So I sleep in the chair. Now I have edema in my lower legs & feet & in the last month or so they started weeping & 1 leg has sores & scabs on my calf. At my age it’s nearly impossible to put on compression socks. I feel this condition is related. I used to exercise a lot but with COVID, the gyms are shut down in NJ. I cut down on my water intake & sometimes will have a tea, coffee, juice but not every day as water is my main drink. I am going to find a doctor, maybe at Urgent Care bc this edema thing has me worried. If I lay in bed, it doesn’t go away like it used to when I was in my 50’s & I consulted doctors. They just said cut out salt & I have cut out a lot of salt. I don’t think that is the problem. I’m going to look for the podcast too. Thanks for your info.

    You better get to the doctor! You might have diabetes and if you do you're in danger of losing your toes your foot your leg. You'd be surprised how much hidden salt is in things you eat. My feet and ankles all of a sudden swelled up as if I was pregnant and I couldn't figure out what was going on because I'm not one to Salt my food. But I was visiting back in Pittsburgh and I like the iced tea that was available. And I drank quite a bit of it because I was talking a lot visiting with relatives and it was quite hot. Then I looked at the bottle of iced tea and noticed it had salt in it not sure how much but it was enough to make my feet and ankles swell. But that weeping is something that's not natural and if you have something that won't heal that's usually a sign of diabetes

    Also I would see an endocrinologist. It may help if they checked your hormone level. They can give you plant-based hormone creams made it a compounding drugstore. That may help with the bladder problem that's usually due to lack of estrogen. The way I found an endocrinologist that would do the testing and prescribe the cream was by finding a compounding drugstore and they told me what doctor was working with them. The doctor I found was in Leesburg Virginia and also had an office in Winchester Virginia his name is dr. Lee. Also the compounding pharmacy is Leesburg Pharmacy. I live in Maryland and I can do a telephone consult with Dr Lee and Leesburg Pharmacy mails me my prescription

    Sometimes a vitaminD deficiency can cause edema. Low carbs and not eating between meals can also help. Worth a try? Also important to get checked by GP, there are many causes I had edema for years, upped vitD to 10k IU daily for one month for covid reasons and within a couple of weeks edema in feet had reduced, reverted to prescribed 1k IU vitD and edema came back, so out of curiosity upped to 10k IU and edema reduced - worth a try? It may work for you also. Check out Dr Jason Fung on KZread regarding low carb and intermittent fasting (16:8) when I tried his system my 6-12 night trips to loo stopped, you don't have to be fully diabetic for this to happen. The hormones are very powerful and in my case it's insulin spikes that contribute to incontinence on the same level as you describe, hope this helps you, worth looking at?? Oh and Dr Jason Fung diet helped the edema also. I also don't think it's the salt and if drinking less isn't working then that's not the problem? Your kidneys need good levels of liquid to stay healthy, if you don't have kidney disease

    Monna Provost - You used to go to the gyms to work out but stopped because of the COVID19? It just does not make sense to stop exercising. You can exercise at home, in a park, anywhere in nature, YOU DO NOT NEED TO GO TO A GYM. Also, so many guys who believe that only gym workouts are good are greatly mistaken. Many of those extremists are performing faaaar too much and too intensively., resulting in many injuries. What you need now is gentle exercises for the whole body, but especially for the oedema of your legs and feet. Obviously, your lymphatic system cannot do its function without you moving. Remember it does not have a pump as the circulation system does.

    I wonder how one can measure how much urine is voided without walking around with a measuring cup all the time. I'd like to start a urinary log and would like to measure it, but do I really want to use a measuring cup in the bathroom?

    Ana Larson - PT -also physical therapy or just call it exercises or movements are certainly a must every day. Bob & Brad will guide you. Whether you think they are helping or not, most of us - there are always some who are just fine without - absolutely need to do a few essential exercises - always with deep breathing - every day, otherwise, your body will just stiffen up and create aches and pains you never imagined could affect you.

    Don't let your bladder control You. If you are always near a bathroom, it's easy to go more often than you need. This is one cause of urges. Relax and breath, wait and keep exercising to hold it awhile, sometimes you won't need to go at all, it was just an urge. Keep tract of your urges. Understand how to control your urges and kegels are not to tighten your whole pelvic floor, it's a very small part of your muscle control on your urethra, and most people do these wrong. Kegels method is very misleading and a bladder specialist never uses this large area to control bladder issues. It's a much tinier muscle than the floor.

    Pessary does help me with prolapse some that came back soon after surgery anyways. Not a bother now. I didn't like the idea at first either.

    When I was a kid, my father erected an outhouse while he was building our house. We kids liked having an outside one while we played. We were disappointed when he installed an indoor toilet and took down the outhouse.

    Itsfunneh there was an outhouse at our house when we moved into it and only a toilet in the basement in the shower I believe when the people built the house they lived in the basement because there was a big kitchen sink down there near where my mom had her washer and dryer. We had an acre of ground and played baseball in the backyard and we also had a horse so we loved going in outhouse it was a two-seater and we weren't embarrassed to go in there at the same time if we were just going in to take a tinkle

    A lot of families have kept the outhouse after the great event of getting an INDOOR toilet. My sister & her fiance got married 35 years ago in the old Hudson Valley farmhouse they shared with friends. To help handle the feasting and dancing crowd, they'd dressed up the still-functioning outhouse with a lantern, flowers and quicklime--posting a note on how to use it!

    Some older homes had for the outside toilet only a semented hole in the floor; so that one had to squat when using it. My thought was: How very primitive! Only recently learned that apparently this is anatomically the most healthy way to eliminate.

    I have this problem and even though my doctor prescribed medication, it is unaffordable here as I also have 2 stents to the heart and taking quite a lot of medication, and also diabetes (but no medication at this stage).

    I had issues with incontinence as it got gradually worse. Had laminectomy and fusion an L4&5 in Oct. 2020. Immediately incontinence was gone. Immediate. Then a couple months later was back with a vengeance and worse than ever. My surgeon just blew me off. I’m so upset and don’t know where to turn.

    The LEAST THE DR CAN DO is order PT for ones that evaluate and treat incontinence. I have Parkinson’s now at 67. I I was diagnosed officially in March 2017 but realized all the treatments for my back and neck with pain,severe spasms and poor control of my left arm and hand it may have been a couple years ear.ire. When I went to a uro gun Dr with my mother that had issues w incontinence and I believe Parkinson’s as well. We both had bladder instillations of a formula of solucortef,antibiotic,lidocaine and other such things. Had to hold it 30 min. Then was allowed to pee.She did acystogram ( picture of bladder) and my bladder was visualized on the screen. As she moved the endoscope around I had the urge to pee. I ran to the bathroom ,but before I got up I saw the inside of my bladder turn completely red and seem to be on fire. I had relief for 6 months, went again good results. Then she was no longer on my insurance plan.I didnt go to another urogyn until 2 summers ago.It was a hot summer, I had leakage and wore pads,hating it! I was taking care of young guy in a wheelchair that was paralyzed from hips down. He always had infections, I took Jim to Drs hospitals,his Dad died while he was in a assisted living and the house was a wreck I spent hours cleaning up in the heat , cleaning him up,snanging wounds.I tried to use bathroom upstairs cuz he was so vgermy but w urgency sometimes I had to use his, I had a uti and a vaginal infection( but from pads in the heat) ugh. I can’t do kegals, because my muscles were so tight w spasms I needed to relax!I took antibiotics,boric acid in a capsule nightly that was a compounded,and Premarin cream twice a week. Done! I got it back this summer and same treatment. Feels like a uti but it isn’t.. burns, pressure, stings to pee. I forgot in my monologue that I went to have surgery to lift my bladder, I had a cystocele,,but when she got in,I had a rectocele and she said she rebuilt my pelvic floor in 2004. The reason Inwent to 2nd dr was I thought my surgery needed to be repaired. This I was an interesting show, she’s hilarious. Sorry so long but she’s right,it’s very annoying and I’m tired of it

    It could be that since you were dry fasting before and for a (little) while after surgery; the urinary retention hormones were re-set?

    I absolutely hate the adverts for (sexy) nappies for women. I would love an operation (sling) but they have banned them because whatever material they were using caused lots of problems..I have stress and urgency incontinence. I used a contraption you insert that gave electrical pulses...I don't drink caffeine and I try to do kegal exercises but jeez half the times I don't feel anything..Incontinence is depressing ☹

    My mother had a sling, and she's now in a wheelchair at age 40 because the operation was botched. However, I have two friends who use Oxytrol patches, and they swear by them. You should look into this. It's cheaper and more effective for some people.

    I’d you have continued problems with urge incontinence or urgency you should see an urologist. You may have a bladder issue that can be treated like Interstitial Cystitis/Painful Bladder Syndrome, a UTI, or bladder spasms.

    What an excellent professional down to earth interview about the value of specialized physical therapy for urinary incontinence. It was a very non-technical and very comprehensive discussion, as if the discussion was about cooking or some similar non-threatening universal topic. I think it is important to also mention that this interview was all about "physical therapy" for urinary incontinence (because Bob and Brad are the most famous physical therapist "on-the-internet!"🤩) But - there can be a medical side to the issue that physical therapy alone cannot fix. Some women should start with their OBGyn first, because there can be a hormonal deficit causing the problem. Some people may have an immediate urgent need for control of a severe problem that a medical specialist visit and medication can rapidly fix. (Examples: must attend wedding, embarrassment with opposite sex, job function, etc., etc., etc.) It is good there is a lot of help of all kinds for urinary incontinence!

    I have the opposite problem my bladder is too stretched too big how do you shrink it
