👌🙏👍Thanks👍🙏 0:53 6:29 7:21 7:58 8:19 棘上腱上層的滑囊 8:47 肱骨頭表面的關節軟骨 8:25 棘上腱長軸 9:06 此爲軟骨消失而棘上腱著骨緣交界面，謂之肱骨的解剖頸9:20 9:29 ❇️爲腱著的footprint 9:45 棘上腱短軸 9:59 增厚些的❇️三角肌滑囊空間(黑色帶狀部份) 10:36 探頭橫置掃描肱二腱、棘上腱內側部份 ( 短軸) 10:40 ❇️肩袖間隙 正常時會有一點fluid在肱二腱短軸附近。 10:56 肱二腱，肩袖部位的掃描，通常從找到介於大小節結間的肱二腱這位置做參考座標開始。 11:44 肩胛下腱(上臂應外轉姿) 12:20 13:18 最後步驟，看一下肌肉組織。 15:49 ######################## 1.🔯腱病變 17:11 比較兩側同位置處腱#厚度差2mm 通常有腱病變意義。 ####################### 2.🔯17:49 腱鈣化 18:10 ####################### 3.🔯滑囊炎18:46 ❇️滑囊#內 的高回音是#發炎肥厚的脂肪體 18:59 ♦️ 脂肪組織在滑囊#內 🔯正常時當外展上臂時，腱部會完全隱入肩峰內19:37 肩峰下夾擠現象20:04 ####################### 4.🔯20:47 腱#全厚度斷裂 21:30 較小範圍focal的全層斷裂：通常皆位在距#肱二腱後方1.3～1.7cm範圍內 恰在#棘上棘下腱交界處 22:14 大範圍Massive全層斷裂:可見腱的縮回及空隙為[積液所填佈]而可被超聲探頭壓扁的低回聲。也可見腱的著骨部位的大節結表層皮質骨不平整的超聲變化。 23:12 23:23 腱體表面的平整性若有#局部明顯陷下，則是#小範圍腱#全層斷裂 的表現。 ---------- 23:44 24:17 #SubdeltoidFatHerniation 看起來誤以爲正常,其實是#全層斷裂，只不過缺損空間被脂肪(近似腱的稍高回音)充填滿佔，表層邊緣外型仍看似平整飽滿。 但當仔細由近端往遠端追循腱表面的線條，却會發現腱的遠端線條不著骨在應著骨的footprint處！實則遠端的線條是脂肪表面的線條。 24:28 當#腱全層斷裂 #小範圍時，該局部斷裂深層會出現#CartilageInterfaceSign。 ####################### 5.🔯腱#部份厚度斷裂 26:16 多發生在#articular side 27:07 27:46 腱的部份厚度斷裂時，探頭施壓於腱的外緣並不會向下凹。 註: Extensive partial thickness tear與Full thickness tear則不易區別。 🌷斷裂的病灶須在長短軸切面皆會明確呈低回音 🌷腱內有回音增強的fragments，損傷位置的底層骨面有退化徵 🌷Intrasubstance tear 28:50
