Tuesday 5th May, Stroke and Vit D

Large-Vessel Stroke as a Presenting Feature of Covid-19 in the Young (NEJM 28th April)
www.nejm.org/doi/full/10.1056...
5 cases reported from NYC area in under 50s
1 woman, 4 men
Large vessel ischaemic stroke
SARS-CoV-2 infection was diagnosed in all five
Patient 1
33-year-old woman, previously healthy
Cough, headache, and chills lasting 1 week
Progressive dysarthria
Numbness and weakness in the left arm and left leg over 28 hours
Right middle cerebral artery
COVID lung involvement
Antiplatelet therapy then anticoagulation therapy.
Repeat CT angiography on hospital day 10 showed complete resolution of the thrombus
Discharged to rehabilitation
By comparison, every 2 weeks over the previous 12 months, our service has treated, on average, 0.73 patients younger than 50 years of age with large-vessel stroke.
Wuhan, incidence of stroke among hospitalized patients with Covid-19 was approximately 5%, (youngest 55)
Large-vessel stroke was reported, 2004 SARS-CoV-1, Singapore
Coagulopathy and vascular endothelial dysfunction have been proposed as complications of Covid-19.
The association between large-vessel stroke and Covid-19 in young patients requires further investigation.
Two patients in our series delayed calling an ambulance because they were concerned about going to a hospital during the pandemic.
Vit D in Indonesia
papers.ssrn.com/sol3/papers.c...
Patterns of COVID-19 Mortality and Vitamin D: An Indonesian Study (26th April)
Retrospective cohort study which included two cohorts (active and expired) of
780 cases with laboratory-confirmed infection of SARS-CoV-2 in Indonesia
Age, sex, co-morbidity, Vitamin D status, and disease outcome (mortality) were recorded
Serum 25(OH) D levels
1. Normal, greater than 30 ng/ml
2. Insufficient, 21-29 ng/ml
3. Deficient, less than 20 ng/ml. This
Results
Death risk factors, male, increasing age, pre-existing condition, below normal Vitamin D serum level
When controlling for age, sex, and comorbidity, Vitamin D status is strongly associated with COVID-19 mortality outcome of cases.
When compared to cases with normal Vitamin D status, death was approximately 10.12 times more likely for Vitamin D deficient cases (OR=10.12; p less than 0.001).

I’m 54 yrs old... no health issues except mild allergies. A few days ago, I had missed a few days of Vitamin D, and got a mild scratchy throat and some upper bronchial congestion...just a common cold...immediately resumed high dose Vitamin D3....woke up 2 days in a row having sweat through my t-shirt and sheets. Dr John, I have to thank you for raising my awareness of vitamin D deficiency. My mum in the UK has suffered for years with vertigo, or dizziness. Sometimes if she does too much or even just in day to day life, she gets dizzy and has to sit down. After reading more about vitamin D, it turns out that her condition (benign paroxysmal positional vertigo) is linked to not having enough. She was already taking a supplement after I told her about your first video (she also watches you every day now too and has made your broccoli soup), but now she has upped her dose. She's going to go back and get her vitamin D levels checked if she's ever allowed out again, but hopefully this might prove to be the key. It makes me wonder how much money the health service could save simply by promoting it's use more heavily and/or getting more sensible sun exposure when possible. Thank you so much for making these videos, we watch every day from Denver.

Joanne Jones - MY VERTIGO WAS CURED! AFTER INADVERTENTLY STARTING TO TAKE A K2-D3 SUPPLEMENT ABOUT 5 YEARS AGO. My vertigo was debilitating and came with a lot of vomiting. I thought my life was over. Same as another commenter here, I went to the E.R. once and they gave me a prescription for meds (a patch I think), but never took a blood sample! I didn't even get the prescription filled. Shortly after that, I started taking the K2-D3 supplement and my regular symptoms started decreasing in severity, and within 2 weeks, they went away altogether and haven't come back since! (Only once last year when I didn't take my daily doses! But it was very mild.) I also take some additional D3 daily in addition to the K2-D3 capsule. I thought the cure was the K2, but have come to realize it was probably the D3, although I'll NEVER stop taking the K2-D3 capsule because K2 is SO important, especially when taking D3. Hope your Mom gets better soon!!!

I agree. I am amazed at how well he explains medical information in a clear way. Also how he tries to keep it scientific by taking into account the strengths and limitations of the studies he shows. But... After seeing this video I'm getting more and more concerned about his own bias when if comes to vitamine D. There is definitely some evidence of a correlation, but this doesn't mean causality. In this study they should have corrected for other factors. For example for obesity because this is linked with lower vit D and could explain the real reason for higher mortality. Also other facors like an inactive unhealthy lifestyle will probably give lower vit D. So I think he should also made this nuance because he is aware of bias in studies. But please do take vitamine D, there might be some effect. Especially if you have a deficiency, and many people do. Just realise that healthy food, normal weight, active lifestyle and handhygiene in combination with physical distancing will have a much much larger effect and is more proven. Just like John also says. John Campbell I hope you read this, because you reach a lot of people and have a good impact on many people's lives. But try to stay scientific, especially when stating the mortality would be lower tenfold by vit D alone. Please be careful with these statement and be aware of your own biases and tunnel visions. Some people might get a false sense of safety by taking vitamine D and take other risks. (I read this way of thinking in the comments below.) Or worse people taking overdose because they think the more the better (remember your video about the bleech and ethanol/methanol, not everybode is able to see the risks, especially when there is panic).

Where I live near Dallas, Texas, within a week we had 4 people age 17 - 30 die from blood clots. 3 were strokes, one was liver failure due to multiple blood clots in the liver, she was 17, and had gone to the ER 3 days prior with severe abdominal pain, 2 weeks after testing positive. She was sent home with "some medication" including one for pain. She was a healthy African American high school student. She was found unresponsive 2 days after the ER visit. All of the 4 under age 30 were African American. All had minor respiratory symptoms. My concern is that doctors still are generally viewing this as a respiratory disease. But the majority of deaths are cardiovascular, even in ICU. It attacks the endothelium, the lining of blood vessels as well, there are ACE receptors there as well as in the lungs (and heart, and kidney). Hospitals are reporting that the CoVID-19 patient's blood is even clogging dialysis machines, and many require dialysis due to renal injury from the illness, and the blood is also clotting in the tubes when blood samples are drawn. My concern is that D-dimer and other blood clotting disorder markers aren't being monitored along with respiratory symptoms on the patients testing positive but not requiring hospitalization.

I've been interested in the subject of Vit D for 10 years and started supplementing unfortunately not many GPs know much about it. Dr Soram Khalsa has written a book on the subject The Vitamin D Revolution which is very informative and what got me started. I tend to only take in the colder months as we get lots of sun here, although it is very powerful and I can't stay out in it for too long in the summer months as we don't have much of an ozone layer. We are limited to 1000 IU on over the counter Vit D here although you can have a prescription for a much higher doses which is normally only offered to post menopausal women. A specific Vit D test costs extra when having a blood test also. Thanks for your daily updates. Best wishes from New Zealand

I guess its a good thing here in Norway that doctors check, with blood tests, the level of vitamins & minerals in serum? They check this, at a minimum, once a year and they do send sms with analysis of what you need to take as supplemental to your diet. Thank you Dr. JC for teaching us so much. I will never be able to repay you, but I will try to remember you in my prayers every day. I was probably infected 7-11 march, but without your advice and guidance this could possibly ended differently, and vit D was a big part of my very mild symptoms. God bless you Sir!

I’ve been using ultra vitamin D for months since cancer treatment damaged the immune system, I have just tried to order more to restock and it’s all out of stock. Another supplier was limiting to 1 pak per customer but they have also no stock. I did manage to source a supply in the end but the message must be out there, unless there are more Campbell followers than I thought. I found the increase in stroke cases a bit alarming because I have already had 5 (luckily any permanent damage has not prevented me from working full time). I think I will stay at home in my cocoon for a bit longer (my job allows me to work at home, so again I am very lucky) 👍🍀. Be safe everyone 🇬🇧

This was on the Living DNA website : Contrary to its name, vitamin D is not a vitamin, but a type of hormone produced by your skin that is activated in your liver and kidneys. Our body produces vitamin D in the skin in response to direct sunlight. The amount of vitamin D our bodies can create from sunlight is impacted by the type of season, time of day, latitude, altitude, kind of cloud cover, amount of air pollution, use of sunblock or sunscreen, clothing, skin pigmentation, and age. It's also important to note that those with higher melanin content in their skin will require a longer time in the sun in comparison to those with less pigmentation as the melanin absorbs some of the sunlight so less is available for vitamin D production.

If I remember it right, it’s fat soluble and can be stored in the body’s fat reserves. It may be important for some people to get their levels tested as anyone overweight may need to take more D or lose a few pounds of fat. D3 that the body makes comes from uv-b rays which don’t travel well through clouds or glass unlike uv-a which also does deeper damage than b rays if out in strong sun. Dietary vitamin D can also be found in brown mushrooms in significant amounts but best to get most of it directly from sunlight if you can and if taking it in supplement form it works well with vitamin k2 mk7 to ensure healthy bones. K2 helps control calcium and reduce its build up in arteries and muscle. Definitely worth looking into more, if only to fact check to a higher degree.

I'm a neuro in NYC. Just a correction on your summary of young patient stroke data from the New England Journal of Medicine: the 5 strokes were in 1 NYC HOSPITAL SYSTEM in a 2 week period, & that system sees ~20 in that age group per year, therefore the deviation was roughly 5x the expected occurrence. The data was NOT THE WHOLE CITY as you stated. We are certainly seeing other strokes & pulmonary emboli as well. The prevalence is such that we feel there should be a public service announcement regarding signs and symptoms that patients can look out for. Bear in mind, NYC has young demographics yet we're seeing a tremendous uptick in clotting leading to PE & IS in young patients who never presented during initial covid illness

There is always a risk of catching Covid-19 (or other things) in the hospital or the clinic but you have to seek help when you need it! The hospital is preparing not just for the pandemic but to help all the other people they usually expect to see.

I started taking 3000 to 5000 IU of vitamin D a day, 18 months ago. Before that, our pre-school daughter was bringing diseases home and getting me sick, what felt like every other week. Since I started the higher volumes of vitamin D, I’ve only been only slightly sick once in the last 18 months. I live by vitamin D now. If I don’t get any sun the previous day, I pop 5000 IU the next morning. If I got some midday sun the previous day, I pop 3000 IU. It’s been working great.

Vitamin D is so important, you need blood levels between 150-200 nmol to balance your immune system so it functions correctly. I have auto immune diseases and it's the best thing i have ever done.

Levels went slowly up but I have not been ill more than very mild cold maybe 4 or 5 in last 6 years. Before 10-20. This is only thing I changed and stopped all miracle pills from pharmacies. And the good news is that there's no upper limit / toxic level of K2. You can pretty much take as much as you want of K2 safely, although researchers like Kiran Krishnan say you need a minimum of 320 mcg of K2 per dose for it to be effective. I eat natto every day AND take K2 (along with zinc, magnesium, etc.) and take 10,000 IUs of vitamin D a day.

If you haven’t been diagnosed with low levels of vitamin D you only need 400 - 800 IE ( = 10 - 20μg) / day. As I’m a healthy 73 yo white male and try to spend as much time outside as possible , I only take 400 IE (10μg) per day in winter and nothing in summer. As vitamin D is fat soluble it gets stored in your body, you don’t need extra intake if you didn’t get any sun previous day. In Sweden the recommendation is to take max 800 IE / day unless in special cases, and then only as prescribed by your doctor. You are obviously a special case, but for other people don’t go above 800 IE (20μg) / day. If you take 50μg /day as Dr Campell suggests for many months it could have some negative side effects (the kidneys). Remember that in normal cases, you cannot improve your immune system with a very high level of vitamin D in your blood, it is only if you have deficiency of vitamin D that can make it better by adding extra vitamin D

I became gradually ill 10 years ago with an unknown chronic fatigue type health issue. Suffered from very achy and painful legs, especially my shins, unable to stand or walk for more than 20minutes at a time, recurrent chest infections & bowel issues, and brain fog. Finally a new GP diagnosed me with level 10 vit d and I was given loading doses. It took months to totally get well, but since then I have not been ill. If only I'd known this as it meant I had to stop work, had to sell my home to survive, but I am well and healthy and blessed with a good life at 62 years of age. I used high factor sunscreen, worked indoors and lived in the Uk. I now sit in sunshine at noon in summer for 20 minutes, then use a sunblock, and there is even a vit d sunbed hire in my area. VitD3 supplements are affordable and I take supplements from September to May and have blood tests for my levels twice a year to check I am in range. This is not “ death” it’s the flu sparky, and since our tests suck the numbers of people who had such mild symptom they didn’t know they had it are 100’s of times higher. Get it you’ll see the fear mongering kept me in isolation with the damn thing I could have gotten an anti viral and cut the recovery time in half. Any pneumonia is going to to stress on the body. If you have had a blood clot well... guessing your odds are higher? If you have high blood pressure and a you are coughing your head off your pulse is going to be way up there... I had it! I’m fine! I’m asthmatic I didn’t even get pneumonia.... this has to stop! You can not stay in lock down because a dancer threw a clot. I have thought for a while this illness has been around for a month or two before Christmas. Myself personally, and my partner had the weirdest of colds in early January. It wasn't a flu, more intense than the usual seasonal colds, and it went on for at least 2 weeks. Infact alot of people here in Ireland did too. My doctor nor my pharmacist knew what it was but they even commented on the "cold" going around as many, many people seemed to of had it. It took me weeks to fully get over it. Antibiotics did nothing for it, exact same symptoms as Covid-19 too. I find it odd that more people aren't recalling that. As it was certainly strange and obvious it was not the usual cold or a flu. It's only when I seen the case in France in December in a news article today did I decide to bring it up again.
