[Performance Place website low back article excerpt] Hip flexor pain can wreak havoc on your love for running. Many of us run for weight loss or because it’s something we feel we “need to do” to be healthy. But not Julie. She simply loved the act of running!
A 10-minute marathon runner in her mid-40s, Julie would typically run five days per week. Some days she would run in track groups, rising to the challenge when her friends motivated her to get faster. Other times, she relished the solitude of solo runs.
Running was Julie’s “fix.” So when a cluster of symptoms started keeping her from doing what she loved, she knew she needed to take action. She came to Performance Place® Sports Care after seven months of unsuccessful attempts at treating the pain. I knew a detailed examination and proper guidance on how to decrease her symptoms would get her on the right track.
How It All Started
About 10 months ago, Julie began to notice hip flexor and sit bone pain when she ran. Since she didn’t do anything to create it, she thought it would go away on its own-just like most of her minor running-related issues had in the past.
But this time was different.
Soon, her hip flexor and sit bone were starting to ache at the very first step of her morning run, causing her pain for the entire duration. Her sit bone region would feel aggravated all day, especially exacerbated when she sat for long periods of time at work. The pain weighed on Julie’s mind. She wondered what she could do to make it go away.
When Julie couldn’t stand the aching any longer, she decided to take a break from running for a few days. Her hip flexor pain stopped, so she made the difficult decision of not running for two weeks.
Although it was painful to give up running, she hoped the pause would give the condition time to improve. And it did-until she hit the track again.

    Hey guys. I have been suffering groin -lower back pain for several weeks . I run a lot and I have started to experience side hip stiffness over the time, groin pain after my long runs however after few days rest and stretch ,pain eased and I carried on my training. 8 weeks ago after my last run the growing pain was 10, hip flexors were on fire and I ended up with lower back pain. I have tried Osteopathy, manual& acupuncture therapy ..better but still pain in 5-6. Today I found your video and I did the hoola hoop movement and my side hip stiffness has got much better. I am so happy about it. I know healing takes time but I see the the lights at the end of the tunnel now. Thanks for sharing it you guys do a great work.     I am so thankful for you both to have made this vid. I have been experiencing occasional pain in left lower part that I didnt know how to call. I consulted a GP and gyn who both told me there is nothing wrong with my uterus/ bladder. GP even said she cant explain my source of pain. I have been worrying whether there is any issue with any of my internal organ. I know how every physician would disclaim viewers should consult a doctor. But in my case i always find videos more useful than any doctors...after some stretches, i feel the pain is relieved. Now i know it is either muscle or nerve rather than organ failure. Thank you both ❤️

    Same here. Pain for 10 months straight and ct scan, ultrasound, and colonoscopy... all fine. My left hip is bad so I guess it is pain radiating right to where my left ovary is and it HURTS nearly all the time. I thought for sure I had a cyst or colon cancer.

This is the exact issue that I’m having and I went to my local ortho and he feels it’s my back as I have a lot of damage to my lower back and collapsed discs and pinched nerves. And when you were describing what to do with holding your skin and moving your waist in circles that sounds really interesting and easy to help with the groin pain!

    I am in my 70’s. I was having groin discomfort and heaviness in my left leg and was diagnosed with venous insufficiency in that leg. Dr suggested a groin stent but I have been getting relief by wearing medical compression socks. I am also prone to lower back issues and see a chiropractor twice a month to keep my pelvis in correct alignment. So far, so good. It would probably benefit me to also do exercises to strengthen my core.

    I just found and watched your video. I’ve suffered from a very sore adductor for the last year, which I injured by doing side lunges with weights. It has reduced my walking jogging, and I recently re-aggravated it greatly by running trails in the mountains. I believe I’ve also suffered a linguinal ligament injury as I now also have a deep pain in the front abdominal region (same side as the adductor pain). So I tried the skin rolling technique at the end of the video and I’m blown away. I had immediate results. I’d say the sore adductor is 80% relieved. It never occurred to me that it could be affecting the nerves. I can actually squat down and back up with no pain and this is the first time in one year I am not hurting. However, the frontal pain is still present in the linguinal ligament area so I’m trying to heal that. Thank you so much for this information! It works!

    Ya the skin rolling is really cool huh. There us much more to do though. A common mistake people make is assume that skin rolling is all they need.     I'm a massage therapist been batteling on and off with right hip pain. And u guys nailed it. Doing all ur stretches first thing in the morning. But yes lower back ache present. Thought I had a problem with my overy😳🥴😵 thanks for explaining everything so well.

    Pretty common as a low back referral. I’d say around 70% of people we see with groin pain that have been cleared by urologists, imaging and primary care are simply spine referred cases.     Tilted pelvis was why I was in SO MUCH Pain when I would walk. Thought I was going to die. Had my pelvis put back into place walked out like a new person. I have to get it put back in place from time to time. Thank God I know where to get help.

    @@Performance-Place-chiropractor Does your male practitioner have pelvic torsion or a right AIC pattern? You both probably already know about this but it is new information for me, and I am seeing other people who’s bodies fall into this same posture pattern. For example: It seems like his right leg wants to rotate out as the right pelvis is rotated forward, and as a response his torso looks like it wants to curve back to the right, forcing the right shoulder to role forward enough to make the right trap appear different in shape and also lower than the left trap. I have this same pattern in my body and am beginning to learn about the physiology and science behind it. PRI, I believe is what the practice is called, really takes a holistic approach to how perception and movement are intrinsically linked to physiological health. Thanks for the helpful video!

    This is great! I had a THR in Nov. 2017, I teach yoga and have had groin pain pre-surgery as well as post (currently have the groin pain) and nothing I try has helped. It is not debilitating, but definitely uncomfortable. I look forward to watching more of your videos and trying some of the suggestions!

    Watched your video, have pinched nerve in groin area; not sure which diagnosis it is even after watching the video. Sometimes I’ll be walking in my leg totally gives out because of this hip problem, I’ve been doing some stretches that I learned from you guys, hoping I’m on recovery but still dealing with this, I have had it for a few years, broke my T 12 back in August 2019, not sure if the trauma from that injury caused this. I was pretty active until recently, I was doing CrossFit and walking every day but haven’t been able to do either for the last month, went to a chiropractor that said my hip and pelvis were off so she’s been jerking me around and pushing on it but I’ve only went to her three times so far. I could use your help

    Hi Cheryl! Yes many times it’s hard to decipher what’s exactly going on. In this video we just showed a few of the test that we use but honestly we use a lot more. Figuring out what’s going on is very possible if you have a skilled examiner and a trained eye to figure out what is the cause of the problem. Imaging is sometimes helpful but not typically required. We can help you find out what’s going on but we have to set up a formal time to assess your movements and learn more about what’s going on with you. Not all groin pains are created equal.     I know I have a bad lower back. I've had injections many times. This time I couldn't even raise my leg to sit on my bed. I could only lay down if I went face down and then roll and straighten my leg. Standing and sitting hurt so I'd walk and stretch. This went on for months. I had 2 rounds of a dose pack. I can't take arthritis medicine because of stage 4 CKD. It finally got better in the last 6 weeks. I can lift my leg and I can walk without fear of my hip giving out on me. I do need hip replacement but I'm not a surgery type of person. I love your demonstration.

    i cant believe it! I've had pain on walking in my front groin area for weeks and the "hula hoop" method seems to have reduced it so that I can walk normally!!!

    I recently started to to have groin pain again. I started about two years ago after one of my soccer games with a really sharp pain that was really uncomfortable and I could barley walk but a few weeks passed and the pain went away and hadn't come back since . As things start to get better with the current pandemic, I started to play soccer again and I'm beginning to feel groin pain again however, it's a different type of pain that limits my movement just a bit. I do feel my lower back a little bit stiff and less flexible. Do you guys think they correlate with each other and are both the cause for the pain in my groin?

    Thank you very much... I just stumbled on your page and I recently started noticing the pain and also a slight round protusion at the same spot you mention, although my Doctor has recommended drug to me and I'm already on them. Thank you very much for the explanation. Plz do advise me on other things to do. God bless you cause I'm a bit nervous.. hope it can reduce with time???

    Hip stuff Hi there, I have a pretty intense nervy feeling going through my groin at the crease, with one particular hot spot that is medial and inferior to the ASIS. It shoots through my adductor/gracillis into the medial part of my knee, and further down at times. This doesn’t seem to add up. I also am fine with the hip flexor tests, and feel no pain and a lot better when my hurt knee (left) is to my chest. When i stand up straight is when it hurts the most, and at that hot spot, and shoots down more. Sometimes its wrapped into my hip and low back, an achiness, but also from throwing myself off, and not walking correct. When i use the theragun in low back area, it refers through the groin and adductor again. Would that still be L2-L4? Ive had a supposed L4 bulging disc long ago and had sciatica running down the right side of my leg, which rarely bothers me these days, and do strengthening exercises when it flares up. This left leg groin issue, ive never had before, and dont recall doing anything to cause it or injure myself, and no popping in hip either. Thoughts?

    Any thoughts if the pain is in the area described, but more constant not related to movement? For example, from what I've seen pain between the groin and left-thigh (but not on the side/hip area), is often diagnosed as Greater Trochanteric Pain Syndrome / trochanteric bursitis . However, for older adults, it can also be iliopsoas bursitis , or even tendonitis or muscle damage (Pectineus , Psoas, etc). To complicate matters, there may be an underlying (kidney?) infection or swollen lymp / inguinal nodes as the root cause, in which case antibiotics may help. I even saw some videos that suggest stretching should NOT be done, and instead advocate ice, anti-inflammatory, sleeping on the 'good' side with a pillow between the legs , and Prolotherapy and/or prp (platelet rich plasma) therapy!

There are many odd hip pain diagnoses that will not respond to movement therapy and may require more urgent medical attention. I cover them in the book.     Great video! I have been experiencing groin pain for the last 3-4 years that comes and goes. When I don’t do any activity for a few weeks it gets better, but it gets strained really easily upon returning to activity. I have tried seeing a physical therapist, chiropractor, gotten two MRI’s done, and nothing is working. The pain radiates down my hamstring and down my hip. When I have pain, my mobility is very limited. I cannot lift my leg and even have trouble getting into my car as was mentioned in the video. What would you recommend for this pain?

    Wow, thanks for this information… I’ve had extreme pain in my right front pelvic groin for over two weeks or more …it feels like a deep tendon …tried message, heat, ice, stretches ..nothing seems to relieve it. When I watched the video bringing my knee up to chest while on my back it’s very painful, hard to put on sock and shoe and lifting leg to get in car the same … I ride a stationary bike 30 mins a day med level which doesn’t seem to bother it…. I was blaming the slight heel lift that was placed in my orthotics that I’ve worn for about four years not really trusting the podiatrist that I had one shorter leg when he never even measured them. So I guess I must have hip flexor pain and need to get an exercise regimen going to help myself before I reach out for further help. If you have any ideas for me would love to hear them please. Thank you!

    I’ve been doing the stretches and exercises for over two months now to get rid of the groin pain but I still have it. I ride the stationary bike daily still. It is such a hassle putting on a sock and shoe and getting into car. It does seem better with inactivity and for a very short time after exercising. It doesn’t radiate anywhere else that I could tell. Is there anything else I should be doing to get rid of this?

    OMG, my pain goes to a 10 when I crunch and press against my flexed knee. Thank you for posting this video. I've applied Kinesio tape for bursitis and IT-hip pain without any real relief. I think I'm ultimately going to need to see an Ortho or physical therapist.

    Oh wow, as I was watching this I was sure it was going to be the second thing mentioned but as it progressed and you mentioned referred pain, I was thinking naahhh but I did the hula hoop test and wow! Amazingly different and better! So now I know...I have been having pain in my sciatica on that same side for years off and on since carrying my first child and as I’m getting older and have Lyme I’ve noticed it’s been more frequent, some low back pain in addition so I’m wondering if it is related to that. I know I need more exercise but it has been hard being ill. But thanks for this, that was cool!!

    I've just started having left sided groin pain. It's starts when I'm lying down. It's a dull type cramp. I have Cervical Radicalopathy in my neck, and bilateral sacroiliac joints arthritis. I think this is the problem. I only get this groin pain when at rest. Thanks for this. From UK 🇬🇧

    I m writing this down hopefully someone with same experience as me or even you med’s will understand my words Back in mid July 22 I had a strong random pinch in my hip, to be more specific in the frontal part of the body as I learned from your video “groin”. Several days went by and my pain got severe, I couldn’t even walk. I used to explain to every doctor that it was a feeling of a sharp thing itchy feeling about 3-4 mins After over 12 ( neurosurgeons , ginecologist, gastroenterologist etc) doctors I still had no diagnosis, all good my organs intact I even made a MRI still nothing showed up! Eventually the last doctor told me to check my mental health according to him I may somatize all my pain. Did that, actually my pain was still there. During this spring it went slowly away and I eventually thought that was it, lived my life like I did before but a strange feeling was still there… Now, after a few months of relief my pain is back. Same spot, same feeling. This time I checked it at a kinetotherapist. He told me that my hip is somehow “blocked” and suggested me to try some physiotherapy and also kinetics. I am still in pain and concerned that this may not be my diagnosis… Also, I am a tattoo artist and I work only on the chair in a shrimp position 🫠 just for context. Any advice or similar experience would be helpful. Hugs and good health for anyone reading this ❤

    So I have lower back problems where I cant move sometimes when it fully goes, however I have noticed I get pain in my right groin and hip that keeps me awake. I ruptured a disc a few years back, any ideas to help me out? I cant get seen by the osteo for 2 weeks 😫

    as a chiropractor many times groin pain is reversed due to the irregular position of the pubib synthesis. A simple (but slightly uncomfortable) drop adjustment takes care of it. Structure = Function

    60 year old male. Told I have a psoas problem resulting from 3 years of covid hunched over my computer at home. Super painful ! I see all these stretches but jeez, it is PAINFUL ! ….left side only. I used to do Brazilian Jiu Jitsu….finally had to quit. Hurts putting on a sock, getting into a car, sitting down.

    Having pain from scar tissue calcifications four years after sports hernia and adductor lengthening procedure. There may be nerves trapped in that scar tissue that may be pinching. Dont know about treatmemt here in nj? Any thoughts? Thanks Lou

    Two years ago I went from no hip pain to max hip pain. Started with walking after sitting, then went to pain in pubic area on my new bike (I changed seats to no avail) then I had trouble with "side hurdle movements" -- mainly lifting either leg up and over the bike to mount it. I now have to lay my bike down on the ground and stand over it then pick i up under my legs. I have lost a lot of mobility in my legs despite walking, biking, elliptical and strength training every day. Lateral moves are the most painful. Sometimes it refers to the flank or even lower back. Sometimes after a long walk I will have pain in the inner thigh and groin area where the hip joins the leg and even some slight swelling in lower pelvis although it does not look or like a hernia and my pelvis is totally normal looking and feeling the rest of the time even when lifting weights and coughing so I dont think it is that. My docs only want to go the CT then surgery route. I have a hard time accepting I went from zero hip arthritis pain to constant limping overnight. I got new shes that helped immediately but then I got a bunion and started limping differently. It's literally a floating pain that moves around that whole region but the one constant is side hurdle extreme pain. I tried to lift my leg sideways over a two foot tall rope recently and got stuck in mid air the pain in the hip was so bad and stayed that way for days. I tried stretching and holy crap, I have lost most if not all flexibility. I appreciate your videos because it gives me some idea of what I can do to try to get this back to normal.

But it all comes does to doing a through physical examination with requires movement assessment and load testing the spine, pelvis and hips. Images are a small part of the puzzle but most of the time they are used to make decisions that should correlate with physical exam findings.     Pain in groin (on point) after splitting leg. After that hip internal rotation also restricted in one leg. And whenever pain arises there will be a pain in lowerback occur. What could be the possible... Thanks in advance for answering... Love from india

    Going to show this to my daughter. She is a competitive figure skater and has been experiencing groin pain off and on for a couple months. It’s odd. She is able to stretch (full splits all ways, over splits on left and right leg, bend, jump, spin, run, etc without pain but sometimes on the ice, when she makes a quick movement like stopping or an unplanned direction change where she has to bring her leg inward and across her body (adductor muscles??) it’s a sharp and intense pain. She has taken time off, was very consistent with several weeks of PT which has helped a little but, I’m wondering if it’s not actually her groin that’s bothering her maybe her back? Going to a new Dr today.. fingers crossed!

    Glad to see you liked the video. And hopefully your daughter watches it. I’d say a strong 75% of the groin pain cases that we see that do not respond to typical rehabilitation of the hip tend to be from the spine. The challenging part is that many peoples back does not really hurt. It may be stiff, or even tender but for most people who have this type of problem, it is very hard for them to believe it’s coming from the back because the hip hurts so much. Figuring out if it is coming from the back all comes down to a thorough physical examination. X-ray and MRI is not often required. In treatment typically entails good coaching of corrective exercises, hand, selecting, stretches, and figuring out the root cause. If you want some help, we offer in person and virtual sessions.     So I have had left side groin pain now for about 5 months. Initially started at the bottom reprise area and radiator over to private parts. Went to the hospital and they did an ultrasound, they didn’t find any hernias. I have gone to physical therapy a couple times and I was given a list of workouts to try to get it to feel better. They helped, but not so much. I do need to be more consistent about them though. Anyways… It started after a night of kickboxing class. Three things happened during the class that could have been the issue. 1) an overdone inward stretch targeting hip motion. 2) hanging from the squat racks and trying to do straight leg lifts for an ab workout, or 3) a kick to the groin. I had at this point been getting back into kickboxing after a year break. I’ve been known to push things a bit hard sometimes. So what started as pain located in the crease between the inner thigh and pubic area, became radiated pain around entire hip socket, pain in the tissue just beside the iliac spine, and sometimes radiating to the abs. It’s never more than a 4 or so, but hiking for example can exacerbate it, or any exercise that uses leg lifting at all too much. Rest helps, ibuprofen helps, but sometimes if I forget to take ibuprofen before bed, I get a restless feeling in that hip area. Like restless leg syndrome except localized to the groin area. It’s really weird, and annoying. Well this evening while laying in bed on my right side (so my injured side up), I massaged inside that crease area to feel it out some and I MIGHT have massaged some of my problem away? It feels quite a bit looser and relaxed, however I fear it could be temporary? I don’t know. I was hoping I could get some advice or see if you guys have ever treated somebody with like symptoms? Thanks, and great video. I’m gonna try the hula hoop pinch next 👍🏻

    Man, this sounds EXACTLY like what I have going on. Discomfort started after a similar workout to yours... then anytime after running it would come back for 2 or 3 days at a time (kind of felt like the dull ache you get moments after a hit to the testicles) I got 2 ultra sounds and everything came back normal. Pain was gone for about a month until i started hitting the elliptical for an hour before each weight workout. 2 weeks into this training program and the feeling is back. I'm wondering if it's the psoas, entrapped removal nerve.... or something of that nature. Did you get any idea what yours was?

    @@lukenietsch1232 I’ve been told it’s probably nerve entrapment. I believe I strained my internal oblique, and even though it’s healed, it’s entrapped a nerve. I’m going to get a nerve blocker to help I guess, and it’s their way of troubleshooting. Strange thing is that I injured a back muscle recently, and while my back was hurting like hell for a few days, the groin pain and whatever completely vanished. The moment my back felt better, groin pain back. So annoying man

    @@tommyredline6432 man, I can relate 100 percent. It's funny though, mine doesn't hurt during any specific movement- it's just sort of a weird nerve-pain that comes OK and off. I've noticed that after running its really bad for a few days, to the point where I'm just sitting there waiting for the feeling to happen, and then I end up obsessing over it. Also, wondering if part if it is psychological because if I'm out doing something that has my mind occupied- I don't notice it. Definitely resonates to lower back and inside of left thigh at times as well. So frustrating!

    @@lukenietsch1232 yeah man. Have you had the rib pain in front yet? I’m kinda dealing with that now. Same deal, no rhyme or rhythm. I did notice that when I would agitate the groin, it was because of doing big steps that needed a bit of twisting motion into it. Like, carrying something heavy uphill. You almost lean into your step. That’s what did it for me. Cause I could jump rope, do agility ladders, no pain. But leaning into steps with resistance, that did it.

    is this necessarily an injury or just something that comes upon you .. i have not done anything to hurt myself but moving the wrong way can cause me to double over in pain and have to stop still in my place for a second

    I was 72 and in fair physical health when I had a total right-hip replacement eleven months ago and continue to have pain in the front ‘crease’ area of the hip and the area along my inner thigh muscle. Walking isn’t a problem but, if I kneel down the pain we’ll return while rising up. Also, I do stretch out prior to my Karate lessons but again the pain restricts full use when kicking or rotating on my right-foot. Any recommendations is greatly appreciated. Thank you in advance.

    @@Performance-Place-chiropractor - I appreciate the fast response so much. My surgery was done by the VA and their still not up to speed, from this CV-19 shenanigans. My surgery was put off for a year, as it was deemed ‘non-essential’. My primary VA doctor is not on full time which makes it difficult for an approved consultation with Pain Management or Physical Therapy.     Interesting, I used to be able to sit cross legged, but now I get pain at the top of my leg/ groin on one side when trying to sit crossed leg, could you give me any idea on the problem, weak adductor muscles?

    I had left hip replacement 91/2 years ago. I had pain on my groin since day one post- op surgery. The pain didn’t go away and my doctor keeps saying there’s nothing wrong. I have been suffering from groin pain getting into the car and whenever I moved my leg. I had therapy x 3 times nothing helps me. Is it hip infringement or hip flexor or something wrong with the hip implant? Thank you guys for your channel.
