This is a video about iNAP, a new device coming to the UK on the 1st December 2021. The research evidence suggests that it helps people with snoring and sleep apnoea by pulling the tongue and palate forward out of the way so that you can breathe clearly without them obstructing your airway.
In this video I give you information about the process of gaining familiarisation with the iNAP, how to get a tight seal and deep negative pressure so that it will help reduce your snoring and sleep apnoea. I also tell you about the new mouthpiece and plans for the starter kit available at launch. At the end of the video I tell you about the iNAP trial I am running on the NHS.

    Hello Sir, My good Doctor. I neede to make sure you are aware that the videos you are doing... they matter to me. I am extremely grateful not just for time doing these but the time spent in school. your demeanor. There is compassion and caring in your voice and it matters. So Thanks man. You have confirmed a ton of my thoughts. i have been a willing participant of bipap therapy for two decades. it has never been a good solution. and I am in the market for either the inspire implant or another similar device. i also am doing the exercises in your other videos and I have gone rom 260 lbs to 190.. yes in america. But I intend to fight back. I will go gently into that good night sir and your info is very valuable to me. so again thank you from across the pond.

    great videos. I am an MD trying to get the word out in los angeles area. I suspect there is a large cohort that will benefit from some optimized form of negative oral pressure therapy. Very excited to hear when the updated oral appliance will be available in the USA.

    Hi Mr Veer, what strain, if at all, does the iNap apply to the teeth when in use? I cannot use a MAD device due to gum disease in the past so was wondering if this would suit me.

    Hi Vik - I've had a uvuloplasty, and one thing I'm noticing in trying the iNAP is that I think I'm sealed at the front and back, I'll start to fall asleep and then the back of my tongue will fall away from the hard palate, letting air in, and that wakes me up - kind of in a vicious cycle...any ideas on how to prevent the tongue from unsealing in my case?

    You can't handle a full sized mask or nasal cpap? Mate I've been on it for 10+ years and nasal cpap is great. If your nose isn't very good, go and see an ENT. You can use beconase or nasonex and see how you go in the meantime.

    Can you recommend an ENT surgeon near Milton Keynes? I'm really interested in DISE . Finally got referred on the NHS & he put a camera down my nose & throat but saw no problems (I was awake) so I think that's it from the NHS . Would be nice to sleep in the same house as my partner.

    Hello Dr. Veer. Thanks for this video which I was on the lookout for. I am in the middle of MAD adjustments. Out of 8 possible straps I am now on No. 5, meaning 5mm advancement. It has helped but not completely fixed for my tongue and soft palate OSA. If it does not work I may be headed towards the iNAP. Any chance you could ship one here to Toronto for trial, or I could come to London? Or perhaps I should discuss with the distributor on how to get one? Don't think it's available in Canada.

    Hello Dr. Veer, so just wanted to send an update after my above messages. I made contact with the distributor and my cousin in London purchased it for me, and manged to get it to me here in Toronto. I was lucky enough to have success getting a seal in the first 5 minutes and tried it the first night. I was shocked to see a snore score of 0 after 4 hours, the first night. I've never had a snore score of 0. The next night it went up a bit, then down the next night. I think it's going to take some doing as it looks like the seal gets broken at night and then perhaps doesn't reseal properly. It's going in the right direction though and looks like it's going to work. I definitely am starting to feel better.

    Have you considered a custom made mouthpiece that will fit around the teeth perfectly and over the occlusion of incisor teeth as well. This will ensure a good fit and grip that will be more comfortable providing the mouthpiece is not too thick. With the development of digital intraoral scanners, the shape of the anterior teeth, mouth and occlusion can be quickly mapped out and sent digitally to a lab anywhere in the world and a custom made mouthpiece made. If it is made in a country like China the mouthpiece can be made at reasonable costs. You may want to consider it.

    They do this with sleep apnea dental devices. I have had this done in the US. It is not perfect, even with a 3d scan the way it fits in your mouth needs to be adjusted by a dentist who specialises in thus. And I wouldn't send anything to China. I wouldn't trust them at all or give them money. Ask the Uyghurs... If only you could.

    The lights really bother me. I sleep in darkness and I am really annoyed by even a small light when I am sleeping especially flashing lights. In the next iteration of the device I hope you add an option to turn the lights off.

    I was hoping this device would work for me as I know a MAD works but it comes with its own set of problems. It was very easy for me to get a seal, the app is great as it can report back if there’s any problems with it during the night. However this product was not for me, I’m able to snore, I’ve recorded myself sleeping with and with out the device, I can easily get a snore when I test it awake, I’ve tested to see if my tongue position is correct which I think it is. Would I recommend it? It seems to have massively helped others and they have a 30 day trial, so definitely worth a try, especially if it means you might be able to avoid needing to use a dreaded cpap machine.

    I accidentally used the device withouth the drying pad and I'm scared I broke it. It didn't have as much suction for a couple of but now it kind of seems okay again. I can still making snoring noises even though the vacuum is at it's peak. I already had an UPPP.

    I have been using iNap for about a month now. I'm still snoring and can't feel uvula touching back of my tounge. I'm on -105 cmH2o. Can deepening negative pressure help in my case?

    I'm very interested in the iNap for my obstructive sleep apnea problems and in the process of looking into this further since I feel I'm a good candidate who is thin, but with a very narrow and high pallet. One thing I'm wondering is whether one can train their tongue using myofunctional therapy to achieve the same thing? For me, I find that I do pretty well if I can keep my mouth closed with my tongue at the roof of my mouth and only breathe thru my nose, which sometimes I have problems with doing. I have experimented with using medical tape to close my mouth which seems to work and forces me to breathe thru my nose; however, this scares me a little bit when I start to feel stuffiness in my nose during the night. I make sure I fold the ends of the tape so that I can quickly get it off my mouth if I wake up and feel like I need more air quickly (which hasn't happened yet).

    Have you not tried the cheap tounge retainer they just sucks the tounge out. Very cheap online. You might have better success

    Sure, but if all 3 aren't needed, why use them? Long term I doubt you'll be happy with the dental device.

    I have OSA & using BiPAP machine but I really need to remove, my doctor advised bimaxillary jaw surgery, you think surgery is best option to revome OSA.

    If you have severe sleep apnea, or really can't handle nasal cpap and your nose is clear and works well most of the time (if not, get it fixed), then yes, bimaxillary surgery is a pretty decent option that really tries to solve several problems at once in terms of airway obstruction. Peace

    Been using cosistantly for a month and getting there - i only got one mouthpiece and would like to try a few others? Also any chance not to have such a bright light? I have had to stick a plaster over to dim it!

    Are you in the UK? My Snore Clinic doesn't stock anything anymore, my mouthpiece split after 2 months and now my iNap is useless because they don't sell them anymore! They say everything is out of stock but I bet they never get anymore in stock.

    Hi Vic, I’m really interested in trying this new device, I’m still waiting to go to the sleep clinic in Liverpool, been over 18months now, been told I won’t get an appointment till the end of the year, (Dec 2022) I hope I’m still married by then lol, my snoring/ loud breathing is driving my wife crazy, My GP, is 100% sure I’ve sleep apnea/copd and has referred me to the clinic, and recently tried again to ask them it’s urgent, but obviously since Covid, they have a massive backlog, is it possible I could get hold of one of these devices to try myself, even if I have to buy one directly from the supplier, not sure I can manage waiting till Dec, so anything I can do, to help myself ( I would even consider going private) if I could afford it…. I did look at buying a second hand machine on eBay, although my GP advised against it. Many thanks. Bob

    Is there a relationship between sleep apnea and teeth grinding? I have both so a mandibular advancement device won’t work and the cpap device just inflated my stomach. If one problem can be fixed will it solve the other? Thanks for the videos.

    There is a strong association, but the reason why this happens isn't completely clear. Are you sure that a MAD won't work? they often do in patients with grinding and OSA.

    @@VikVeerENTSurgeon The MAD held my jaw forward but wouldn’t let it move from side to side. When I did start to grind it felt like my teeth were trying to rip themselves out.

    @@stephenoreilly12 that's what is happening to me and I am due to pay the final Instalment of my personalised mad device. It is eradicating my snoring BUT I'm sleeping way worse due to waking up with crunching thr device, with dry mouth and sore jaw 😔

    @@stephenoreilly12 okay, let me give you some quick answers. You might not like the feeling of your teeth grinding with the splint, and while it may seem like its bothersome, without the protection of the splint you would slowly ruin your teeth even worse. They don't use soft material with splints because they find it can make the grinding problem worse, but I'm not buying that in my anecdotal collection of experiences with people. Considering how they are designed, it is likely an engineering issue that is yet to be solved. Try muscle relaxation exercises, supplement with magnesium and b complex vitamins.

    I'm signed up with the Nuffield house sleep clinic (Ldn bridge campus of Kings) - if you want a guinea pig for this, please let me know! (Pre-treatment AHI of >80). Would love one of these for combined therapy with my CPAP, and to use on its own so I can finally go camping again.

    Hi Vik, I have used the iNap (in the U.S. for about 3 months) long enough for it to feel comfortable. It stays in the whole night. The app shows that I have a really good and consistent seal through most of the night. But my oximeter still shows oxygen drops. (seemingly while the iNap is showing a good seal) The oxygen drops wake me up and the result is that I still don't get enough stage 4 (slow wave) sleep. (and of course, I'm waking up when I should be sleeping) I am wondering whether a higher pressure of suction would solve this. What do you think?

    Hard to know. We don't know enough about iNAP for me to give you clear research data, and I think I would want to see you in a DISE before I could give you meaningful advice. My feeling is that greater suction won't help. Really focusing on feeling that tongue roll forward and getting the positioning right so that you can't simulate snoring whilst awake I think will be a more fruitful path. That's my experience anyway.

    @@VikVeerENTSurgeon Hello sir my father is detected with obstructive sleep apnea, and many times suffered from sleep apnea attack in episode he is unable to breathe slowly his breathing goes down, oxygen saturation while in sleep goes down, snoring increases and breathing motion gets faster and after 1- 2 minutes ends up in awakening up from sleep disturbingly and some time very aggressive physical actions occurs in sleep itself when we awakens him by moving him he is unaware what happened in that scenario, pls guide for further treatment....

    I've been trying iNAP for a couple of months now, and I'm getting very high sealing percentage times. However, I'm not getting good sleep, and it wakes me often whenever it kicks on. Unfortunately, the unit now has started to shut down every night as the vacuum pressures exceed -200 cmH20. I've noticed that this high pressure occurs when I suck in, which I have to do before swallowing. The pressure keeps increasing and increasing until to goes above -200 and shuts down. Tech support apparently doesn't have any answers for me, so I'm probably planning to give up and return the device. I was so hopeful, but it's not working for me.

    There are some other cheep small devices for pulling the toungh out, non of those worked. This will cost aprox 1000 dollars, whats the difference?

    Agreed, there are devices which cost less than £5, but they aren't very comfortable. If you have found that you can tolerate a tongue retaining device all night and it makes no difference to you, then perhaps iNAP is not for you. I would get a DISE so you can work out what your issue is. Only then you will know which device will work for you.

    Hi Vik, Thanks for making such amazing videos, it's really helpful. I am 38 years old and my weight is 72 kg. I am having OSA with AHI level 80+ and deviated nasal septum. I had two surgery down “Modified UPPP and Excision inferior turbinate partial or complete”. The tonsils were also removed in the same surgery. After surgery, my condition was much better. But after one year my condition is slowing becoming the same as previous. I tried to use CPAP machine but was not able to tolerate it after 1-2 hours. The air starts leaking when the pressure increases to 18-20 (max). Maybe I need a different CPAP mask which I more convenient and easier to use. Right now I have Full face mask. Kindly advise on the selection of CPAP masks with their sizes. I am a mouth breather in the sleeping time. Thanks

    With a situation that bad, you probably need jaw lengthening surgery. If not that, a jaw position splint. With such high pressures as you would need, I would use a full size mask. This iNAP device would probably be inadequate for someone like you by itself, but used together with a cpap (if you can be bothered) could be worth a try.

    Try taping your mouth. There are youtube videos for that. Use nasal pillow cpap

    Sorry to hear that your modified UPPP and nasal operations don't work as expected. I also had a modified UPPP months ago and it doesn't seem to improve much, rather my mouth seal seems to get worse (soft palate seal loss), which leads to more mouth leak, and further causes aerophagia when using CPAP. I founds a couple of things improve my situation, V-COM helps with aerophagia and mouth leak, wedge pillow might reduce acid reflux or improve apnea, mouth taping and chin strap improve jaw dropping. BiLevel technically should also help with the comfort, but still experimenting ..

    Hello Dr Veer ... can you please answer this important question involving sleep apnea ? You have mentioned in a couple of your videos about the tongue falling back on the airway and causing people to not breathe . Can you do surgery on the tongue to reduce its size and to help with sleep apnea ? I understood what you were saying about the soft palete and how it is important for breathing and eating and drinking . And it is normally in one of 2 positions , so it alone is not the cause of the apnea . Can you tell what can be done it the ENT doctor mentions that the patient has a large tongue .. thank you , please answer. I am one of the many many people suffering from sleep apnea and I am currently on the waiting list to see another ENT surgeon.

    For all those who suffer from sleep apnea. Please try the face former therapy. Before the therapy I had an ahi value of 30. After the therapy my ahi value is under 10. I have freed myself from the cpap device. So far I had to use the device for five years lsng. With face former therapy you have to be very patient and disciplined.

    Thanks for this. I had not heard of it. I see it works on neurophysiological principals and how combining face former device with exercises can be beneficial for oral health, swallowing and pelvic floor strengthening and vagal issues. I have several issues that appear related to the vagus nerve so it may well be worth a try.

    I tried iNap alone but not adequate enough to stop my snoring. Tried iNap with Nasal CPAP but pressure from my CPAP pushes iNap mouthpiece breaking seal when I'm in my deep sleep

    It makes sense that positive pressure shoots air into your mouth while inap is sucking air out, accumulatively it gets strong enough to push mouthpiece out. I am thinking about dual use inap with cpap, thanks for sharing your experience! Best luck!

    Thanks for the comment. At the moment we don't have the evidence for this either way. Occasionally we see an epiglottic trapdoor improve with a mandibular advancement device or surgery so it is possible this will work as well. Very hard to be less vague without the research evidence.

    I work full time on the NHS (about 55 hours a week), and I do private practice in my spare time (about 6 hours a week) and youtube for about 30 minutes a day.

    I am based in staffordshire and recently told I have severe OSA. Then told moderate. Got a cpap and high blood pressure this has all come on very recently. I am slim fit and it doesn’t make sense. I have seen maxi facial and it was a waste of time. I am waiting for a appointment with ENT.
