    I like how they keep Coxs one flaw as a Dr consistent; As soon as psych gets mentioned he's walks away.

    I think the structure of this episode, it was more about making a musical than making it 100% accurate and it have to be consistent with the characters as we've grown to know them. JD listens to Cox, who have this problem with psych, which leads to the different tests before finally making the right choice.

    Oh my gosh! That sounds amazing!!! I absolutely love that song. There wasn't much medically to react to, so I cut it out, but I did thoroughly enjoy it... And have sang it on occasion too 😝 such a catchy song!!

    To be fair, they postponed the proper imaging so much to get more songs in. Otherwise the episode would have been barely half its length.

    Scrubs had the patient die often enough that we did not know if she was going to wake up. It made the happy endings in the show so much sweeter

    You didn’t mention the final scene of this episode, in which she seams to actually start to miss the singing. That part always seamed so important to me. You never realize what you’ll miss until after it’s gone

    This episode and Subspace Rhapsody from Strange New Worlds are my absolute favourite musical episodes in non-musical shows. Both did it perfectly, and managed to blend in the reason for people singing seamlessly, without taking you out of the immersion of the show.

    I LOVE MUSICALS. This is definitely my favorite musical episode of a tv show. I feel like each song is like a Broadway song. Welcome to Sacred Heart: Clearly a "Be Our Guest" parody, The company showstopper song Everything Comes Down to Poo: The silly song Gonna Miss you Carla: The upbeat song The Rant Song: The fast song, in particular, "I Am the Very Model of a Modern Major General" When The Truth Comes Out: The Act 1 Finale, clearly a Les Mis parody "One Day More" Guy Love: The ballad song (My favorite song in the episode) For The Last Time, I'm Dominican: The Latina dance song Friends Forever: The 50s do-wop song, clearly a Parody of "We Go Together: That's What's Gonna Happen: The serious song, but the Act 2 Finale song

    I loved your reaction when seeing the MRI machine and mouthing "finally...yes!". I have watched this Scrubs episode many times and just now realized the same thing you did...why didn't they do the imaging first!!

    I really like these reviews. Scrubs is by far my favourite show. It would be interesting if you could react to Season 1 Episode 5 "My 2 dads" . JD is invited by Kelso to go golfing with him and another doctor. The 4. person to show up is actually Dr. Cox. Kelso and Cox have an indirect discussion about how the clinic should be run (and both have very good points imho), indirect because both narate their arguments to JD while golfing without the other person being present.

    my big thing is... how did she see them singing welcome to sacred heart? dr kox and kelso, multiple surgeons and other doctors. even the janitor. she doesn't know they exist yet, UNLESS they all happen to be in the parking lot at the moment she arrives

    @@JessTheMD ha, there are so many! The biggest one is that the patient who hears people sing is Broadway actress Stephanie D’Abruzzo. Each of the songs is based on a famous Broadway or film song, or put into the style of a famous Broadway song. “Welcome to Sacred Heart” is similar to Be Our Guest from Beauty and the Beast; or Lullaby of Broadway from the musical 42nd Street. “Everything Comes Down to Poo” isn’t based on anything in particular but the song was written because one of the shows medical advisors said that stool samples seem to be asked for, regardless of diagnosis. Dr Cox’s “Rant Song” is similar to “Modern Major General” from The Pirates of Penzance. It’s called a “patter song”, where they sing really fast and get faster throughout the song. “When the Truth Comes Out” is similar to “Do You Hear the People Sing?” From Les Miserables. “Guy Love” is based on every romance song in every musical ever. 😂 “For the last time, I’m Dominican” is based on “Tango: Maureen” from Rent. The last song, “Friends Forever” is based on “We Go Together” from Grease. The slower part, “What’s Going to Happen?” Is based on “Will I?” from Rent. Feel free to search up those songs; you’ll definitely be able to see the resemblance!

    No, I am a TRUE scrubs fan!!! I haven’t watched it much in the last probably 10 years (other than reacting to it) but back in the day, I watched it all the way through SEVERAL times 😂 there are definitely a lot of parts I’ve forgotten since then though lol

    This episode is music and drama first and fitting the medicine around it. Definitely one of my favorite episodes and it is as much fun watching the DVD extras about the making of this episode. We used to watch Scrubs a lot. The Michael J Fox episodes were always fun, not sure if you have done them already. 😅

    Such a good episode - with the imaging, this episode probably took place in the span of hours, so I could see it taking that long for a CT (even an urgent one). When I had to get a CT back in 2015 for reddish purple swollen feet + fever + gran# 0.51 (arrived at 6 PM), I was sitting out in the hallway for 45 minutes for an ultrasound, and then was stuck in the hallway for an hour or two to be brought back to the ER (the report beat me back to the ER!). Then I was in the hallway for a CT around 12:45 AM, got the CT at 1:30 AM. So things can take a tiny bit of time. and, well, if she wasn't immediately rolled into the CT machine we wouldn't have gotten the Janitor song!

    I totally agree with you - though I think my big shtick with it in this episode was not that it was taking time to get imaging, but that it was almost immediately ruled by Dr. Cox as a psych issue BEFORE imaging was obtained... That's where it didn't quite sit right with me 😊

    @@JessTheMD yeah, that was pretty hilarious - imaging should be the first thing after check on, IV placement, and relevant blood work. It's not like this was at a point in the series where Cod would've been off his game, either (although I haven't done a full rewatch in 8 years, and I paused halfway through my last rewatch 2 years ago when I fell behind on the rewatch podcast).

    In earlier episodes, they made a big deal about trying to cut the line to get scans done or risk having to wait until Monday to get them done (though I forget what kind of scans they were). It's possible the delay was due to limited resources and they just never came out and said that.

    I completely get the delay in scanning after waiting for many MANY hours after a bunch of weird symptoms (suddenly falling, double vision, headache, nausea, difficulty to walk or retain balance etc). Turns out - small stroke! At the age of 28. Preparing for the operation as I'm writing this, and What's going to happen has never hit home as much as this year. Every time I hear it, I cry (yay post-stroke depression and oversensitivity)

    a run down lived in hospital like sacred heart was probably still using 16 or 32 slice scanners back then, so even a head scan would take a couple minutes. i applaud providers that knock off options before choosing ionizing radiation. there is no safe minimum dose.

    Weirdly, This episode hits close to home as I experienced "Brain on Fire" - Anti NMDA -R Encephelitis that wasn't caught until my 3rd hospital . Before that I was being treated for mental diagnosis's such as Bipolar, Schitsophrenia, Manic due to the how my TB presented itself. And while I don't' know what kind of tests they ran or didn't. They didn't catch it until another doctor at the ER of the third hospital recognized the symptoms and had them run a test where they found a Terra-toma that had caused the inflammation of my brain. Kind of crazy to think that this show wouldn't do tests right away and write it off as "Being Crazy" but in a way thats exactlty what happened to me.

    Love your insights so much. I feel like these type of videos are extremely valuable when there’s an expert dissecting them. It’s informative in a way you can’t usually experience. Not sure if you’re accepting suggestions since I imagine you get so many, but I’d love if you took a look at the episode “my last words”. Another Doctor named Dr Syl watched it earlier this year and fell in love with that one. Thank you for the content and hope 2023’s going well 👍

    I wanted to react to Guy Love, but that's just a great song. I've 3 eye surgeries since 2020, It's been with the same doctor that I know really well, but I don't know the pre-op staff. I've been so impressed with how pre-op anxiety. My doctor can't be there, but the pre-op nurses keep me calm.

    People talk about Doctor Cox like he's amazing but episodes like this prove he's not. His bias shows here. He's human and flawed as a doctor too. I have fainted before and hit the ground so hard my tooth shattered and clearly got a concussion. Vomiting the whole 9 yards. At the time we had no idea why. The doctor ran no CT scan. No blood work. Just sent me home. Turns out I had PKD that had effected my renal and cysts that appear in my brain. My blood also doesn't clot well now. Took two years to discover it due to this. I have a terminal illness with seizures consistent fainting and I'm skinny, work out, don't smoke, am known complacent with doctors, refuse pain killers, avoid ibuprophen and tylenol to reduce damage in the liver. On top of that I'm allergic to every antibiotic I have taken. All five. And I still have to fight for treatment. What makes scrubs fantastic is how accurate the patient side is too. I watch ER and just can't see the accuracy in it. CT scans cost money. Even in cases they have to use it they avoid it. This was unfortunately realistic for a patient with no pre-existing history. I still love Doctor Cox the way his tone changed when he found it was accurate to good doctors. But people miss the point glorifying him. All doctors can have prejudice and mess up. And the show is designed to remind both physicians and patients it's way more common then doctors think. I love it.

    I'm not saying this is the reason, but maybe imaging was intentionally delayed as this was their excuse to have a "musical" episode. If you solve the condition too soon, you lose your underlying reason for the episode to be a musical and it becomes just like another episode.

    Next time you see a new patient, you should just walk into the room and immediately burst into song. It'll let them know that you are multi-talented because you can help treat them, and also sound good. :)

    So, as far as I know, no, a CT head would not detect any brain changes related to depression/anxiety. However, I did find an article in JAMA that discussed MRI findings that correlate with depression (some decreased gray matter volume in specific parts of the brain, like the cingulate gyrus).     If you can afford it you need to have one parent stay with the child from zero to 5 (or whenever you turf them to school) because those first five years are critical in early childhood development. If both parents are at work who is raising the child, because they're no supposed to raise themselves.

    I agree that those are important formative years, but I disagree with the blanket statement that one parents needs to be a stay at home parent to raise the child. There are lots of options and other caregivers that can also help and be a part of your village when raising a child…

    Just spent two visits in a hospital after years of nothing. You may find this interesting. So I had classic Stable Angina... It was unpleasant, in the end no stint recommended. And I'm doing the usual standard stuff for it. But then things got weird. I was laying out pieces to a toilet tank gasket kit- leaning over and I started feeling weird. Chest pain started and upper body pain in the standard patterns. I wonder if it was unstable angina or if I was having a major heart attack. Admitted myself about an hour later as the pain increased and my BP was in stroke territory. But it wasn't a heart attack (blood test showed no raised levels). It wasn't a pulmonary embolism (cat scan). But the morphine they gave me did *nothing* for the pain. First time I ever had the stuff. So what was it? A dislocated left rib right over the heart. It was out almost 1/8" and it was putting all upper torso muscles into full spasm. I was nauseatingly sore like from some crazy workout for 4 days. I fixed it myself by leaning backwards over a chair with a short sturdy back. I had a history of a right rib going out that way (but only level 4 pain from that). Anyway... inexplicable pain could be from a dislocated bone.

    @@JessTheMD Thank you! I was really surprised that a dislocated rib could duplicate the pain symptoms and pattern of a major heart attack. And to your comments in this video, I got blood tests within 60 minutes and a CAT scan after about 3 hours when the troponin blood work came back with normal range. And I agree the delay on her testing seemed odd. Could be insurance, could be editing, could be we were given the impression more time was passing by the editing than we thought.

Someone else here mentioned that it might have been more medically accurate and still go with the flow of the musical to do the CT first, and do all these musical numbers while they waited for results 😝 They might have had to re-write a bit of it, like the psych turfing part, but still could have made it work and be *more* medically accurate 😊

    Well to be fair, it all took place in like 2 hours or so. So to say it took a long time to get images? Patients get bored and really restless waiting for all the tests to take place and result after result trickling in must have been frustrating when she was anxious and listening to all these conversations (or songs) about the doctors personal lives while waiting to find out what was wrong with her. We see it from her POV and yeah, she wasn't at her most coherent. So while they waited for the imaging machines to be available, they did other tests and Cox expressed his doubts they'd find much which to her musically minded mind came out as he was convinced she was nuts.

    I like this episode but yeah, I wish that like, maybe they did the CAT scan when she first got there and then all the other stuff while they awaited the results? It seems very out of character that Cox would be outright dismissing the test that probably would show the most relevant information to the issue, just for the structure of the drama.

    Ok, the head scan was late. But most of your "problems" were about what happened in the delusions. Not really fair to take that as if it happened like that ^^ btw, just meant as an argument. Not a complaint, because I really love your reactions. Sort of in a bit of a binge ;p

    I strongly disgree with the asessment, that it would have never taken that long in the real world to get an MRI. I experienced this. My symptoms were not visible, the doctors had to go from what i told them. That is a very bad position to bee in, doctors don't like that. They like blood tests and broken bones, something you can see and measure. I went to 3 different doctors, finally got to a good one who sent me to the university clinic. Got almost got sent back home in the ER with a brain tumor the size of a golfball. If i did not have a family member fight for me, this stupid picture would not have been taken, certainly not in time - and i would be dead. Stuff like this happens all the time.

    omg! I am SO sorry that this was the experience you had. I think in my heart, I want to believe this $h!t never happens. I know there are definitely places around the country that it does, for whatever reason, be it "bad medicine" or just lack of resources. It happens and I wish it didn't. I would love to be able to do something to help this NOT be a thing, and I'll do whatever I can to work toward that goal ❤️

    I like the story but this episode was never my favorite because I just have a natural bias against musicals. I mean, I like how this is presented. Normally I say if you're going to tell me something then say it, don't sing it because I have such a hard time understanding words that are sung to me for whatever reason. I miss lines, they kind of slur together sometimes, sometimes important lines I miss. You know when you hear a song on the radio and some lyrics sound like other words that make no sense? That's the way musicals are to me, so I don't like musicals, but I do at least understand in this show why everybody is singing, and the singing is necessary for us to understand the problem with the patient, still not my favorite episode. That being said, I mean, I understand this is exaggerated for the show, I'm sure nobody imagines people singing in a big musical style flourish like this, but I mean do people actually experience something like this in real life? Is there realism to this show at all or is it just a cheap excuse for the show to do a musical?

As you say it may be exaggerated for the show but yeah they wanted to do a musical and needed to find a medical reason for why someone might be hallucinating all the singing and dancing and researched it. The mind is a powerful thing. I've done musical theatre for reals but I have also dreamt I was performing in musicals that were completely made up and random and I was hearing songs seemingly unknown to me so I don't see it as too much a leap that someone experiencing hallucinations might see that mixed with the reality around them. I dunno if we'll ever fully understand the imagination and potential of the human brain. :O Have a nice day. :)

    @@JessTheMD Sometimes when I see a Scrubs episode such as this one I'm reminded of a Simpsons episode that I can't quote from very well but something happens to somebody, there is some weird medical explanation, Bart then (kind of breaking the fourth wall) asks Lisa what we're all thinking "Is that a thing" and Lisa replies "Yes" and follows up with an explanation. I'm always asking in my head during a Scrubs episode "Is that a thing." Glad you're here to set me straight on if that's a thing or not!

    @@JessTheMD i'm a pretty big fan of musical theater, but this type of thing is just pointless. it brings nothing additional to storytelling. it doesn't make any of it any deeper or more meaningful. just an opportunity for the people who make tv shows to pat themselves on the back for doing something some other people have already done... i just don't get it.

    There are definitely a lot of standalone episodes in various shows (Scrubs included) that don’t bring a lot to the storytelling overall, but are just standalone. This was one of them… it’s okay not to like all of those episodes. I’m usually not the biggest fan of musical episodes either. I did enjoy the musical qualities of this, but that may also be because I just love scrubs 😝

    ​@@TheRedStateBlueI don't like musicals when they use popular songs. But I do like it when they make their own songs. It's just a bit of variation, I don't wanna say it's my favourite episode, but I love the songs. For me, it's definitely added value. Which shows, different people have different likes. Now, flashback clips episodes (in scrubs: my night to remember), those are generally lesser value. In the end, a show that doesn't really tell one story (tight, story based ones) can benefit from just variety.
