It can also be a cultural thing. I don't know the culture of the Japanese, but I'm fairly certain they highly respect their elders, and might feel like mispronouncing his name could be disrespectful. All the executive power is vested in the PM, which makes the PM the most powerful person in Japan, but PM is below the Emperor in rank. All the constitutional monarchies have this system. Same thing with countries where the President is the head of state, but doesn't have any or very limited executive power, for example Israel, Ireland, Germany, Greece, Portugal, Hungary, Italy and so on. Off the top of my head, I think Cyprus and Turkey are the only European countries which have no PM and where all the executive power is vested in the head of state - the President. Much like in the United States and the majority of the countries in Central and South America.

Actually those were German police outriders during a state visit by the Queen to Germany. Her police protection in the UK is rather more discreet.
