The MCAT is a terrible and useless test. Here's why.

All aspiring doctors have to take the MCAT to get into medical school. Too bad this standardized exam is the most asinine thing to have ever been created. A waste of time, a waste of energy and a waste of money. Oh and spoiler alert, it has nothing to do with real medicine.

I disagree with this take for the following reasons. And just to preface im a physics premed Firstly, the mcat isnt about bulk memorization and regurgitation. Alot of the mcat is more about understaning how to read a passage and extract useful information. Many of the passages disguise themselves as difficult, when in reality all the information you need to solve the passage could be hidden in one sentence. It is up to the examinee to look past the jargan and be able to understand what is being asked and where to go to find it. Second, the mcat does take a long time to study for, however you are forgetting that most people who plan to take the mcat are taking their bachelors courses, which are all information that is tested on the mcat. As a physicist and someone good in chemistry, i spent the least amount of time studying for the phys/chem section and yet it was still my best section due to all the preexisting knowledge i has. Third, for med school admissions, it is really hard to asses one's intelect from gpa alone. Which do you think is harder to get? A 4.0 in physics spending 4 years at a prestegious school or A 4.0 in health management spending 2 years at a cc and 2 at some random school. The mcat serves as a standard that everyone is subject to regardless of their origin, and is effectivley a way to quantify intelect. And before you call me a shill I dont work for the AAMC or anything else but i did take a prep course with princeton review.

First of all thank you for saying the things all of us premeds believe about the MCAT! It’s definitely not the best measure of what makes a good physician. As an aspiring pathologist, this makes me very happy 😁 that said, if we were to replace the MCAT with another test that more accurately represents the skills physicians use in practice like you mentioned, what concepts would you test? And would you make a different entrance exam for each specialty? I’m just curious because as I said before, I want to be a pathologist, but the skillset of pathologists is very different from the skillset of physicians of other specialties. I’m sure you’d agree that most physicians don’t need to examine peripheral blood smears under the microscope very often, and I don’t think pathologists will need to practice suturing often either. So I guess my question is, assuming that getting rid of an entrance exam isn’t an option (even though I like that idea a lot) in lieu of the MCAT itself, what would you replace it with? Thanks again for this video! I look forward to more of your content!

Thanks for the comment! That's a very interesting idea actually -- having different entrance exams based on what specialty you want to go into; it does give each person the ability to showcase their individual strengths and backgrounds. My initial preference was to test concepts based on the following ideas though of course, nothing is perfect. (1) being as practical and as general as possible (applicable to as many specialities as possible; eg basics of taking histories and physicals) (2) being as useful to society as possible (eg. all premeds are functionally equivalent to trained paramedics and as practically-inclined, eg are the kind of people that those in an apocalypse would want with them) or (3) fulfills a gap in medicine that exists today (example. primary care) or (4) fulfills an important knowledge gap that is not taught in med school and yet, very important to understand (eg. business and finance of medicine) It's pretty unfortunate that docs are steadily losing control over their own fates to people who have no idea how medicine is practiced.

I'm not in the medical field but studied MCAT for the knowledge, was not that bad, I mostly used free material like the course on Khan Academy and KZread videos (Ninja Nerd).

For the MCAT, why not just base it off of med school courses? That way it's as relevant as med school, and you don't have to be there as long, so you have less debt, and they make less money! It feels like school is more of an adversarial place than a place that helps students reach their goals faster. On how I like my healthcare, I like listening to actual ideas on health as well as health debates, then share the resources that I think do a great job explaining a point of view or defending it.

As a physician I agree the MCAT isn’t a good test for reflecting what people need to know to become practicing doctors. But the problem is that it’s difficult to come up with a better test to weed out the huge applicant pool. At least the science MCAT topics are foundational to medicine as opposed to randomly memorizing sports stats. Testing basic first aid or BLS would be more practical but also too simple to distinguish anyone. As for how to select for compassionate doctors instead of cut throat test takers - the problems are two fold. As you know top institutions like Harvard aren’t interested in churning out compassionate doctors. They are interested in turning out top researchers and future leaders in medicine. And as top medical schools set the trend, the other schools follow. The second issue is the prestigious, high paying salaries which attracts a lot of people who are just in it for the prestige and high salary. It’s easy to see with the lack of primary care physicians which are relatively low paying but the competitive nature of getting into high paying specialties. Sadly the system isn’t getting fixed without a major overhaul. MCAT is just a small component of this.
