When exactly in the video does his expression change in realizing it’s the Toymaker? Cause it’s a little hard to tell if he realized this when the Toymaker was tossing balls at him.

It all seems to work with what we know, Cassandra will be the last human and was originally a man, so I thing it could have been her he referenced.

Cassandra was never the last human, the last humans were all the humans at the end of the universe in Utopia. Cassandra calls herself the last human because she's the last "pure" human who has no alien in her bloodline since the other humans "mingled"

The true last humans were the ones who gathered at Utopia at the end of the universe. They were far closer to the original humans than Cassandra, a fact that was handwaved by the Doctor as humanity returning to form, which doesn't make sense. Probably someone found and, and for some reason, replicated an ancient genetic archive of some kind. Scientists have speculated that life might continue indefinitely despite the "heat death" of the universe, if it just evolves the ability to use energy far more slowly as the amount of energy available gets steadily lower. They've estimated that, with the right slowdown schedule, there's no upper limit to the total number of thoughts that living beings could think, despite the fact that they must continually think at a slower speed to avoid using up too much energy. Beings adapted for the Heat Death of the universe would look nothing like current day humans.
