Cataract surgery offers various lens options to replace the cloudy natural lens affected by cataracts and restore clear vision. The most common choice is a monofocal intraocular lens (IOL), which provides clear vision at one specific distance (usually either near or far), requiring patients to use reading glasses or glasses for distance vision, depending on the lens chosen. Alternatively, multifocal and accommodating IOLs are available, allowing for improved vision at multiple distances without glasses, though they may not provide as sharp vision in each range as monofocal lenses. Toric IOLs are designed to correct astigmatism, enhancing vision quality. Ultimately, the choice of cataract surgery lens depends on the patient's vision needs, lifestyle, and their surgeon's recommendation, as each type offers distinct advantages and trade-offs.

In September of 2021 I had laser-assisted cataract lens replacement surgery performed on both eyes at the same time with Panoptix multi-focal lenses. The outcome has been spectacular. I now have 20/20 distance vision, and 20/15 close up. For many years prior I wore soft contact lenses and needed reading glasses when wearing the soft contacts. If I was wearing my regular glasses, in order to read I would take off my regular glasses to see close up. Now, at 68 years old I see extremely clearly at all focal lengths. As an over-the-road truck driver with doing tons of night driving I was concerned about potential star burst patterns, but found them to be of little to no concern. Depending on the brightness and the angle of lights at night I could sometimes see circular rings around bright lights, but not always. At this point two years later, they rarely show up. I did a lot of KZread video research before the surgery, watching many videos on both sides of the potential issues and side effects. I would NEVER suggest anyone have elective eye surgery such as LASIK. However, if one needs cataract surgery that’s a different story. For anyone needing cataract surgery, I would suggest doing lots of their own research and know up front to the best of their ability what potential side effect they personally might or might not find bothersome. No longer carrying around glasses and constantly putting them on and taking them off has easily been the greatest benefit. By the way, I was back at work as an over-the-road truck driver 48 hours after surgery.

Panoptix, as with all multi focal lenses, divide the light. This means the ability to see shades of light or contrast is diminished. Of course this is also what your cataract has been doing! If we give you this type of lense with a non multifocal lens in the fellow eye you would be surprised by how less defined your vision might seem. That being said patients like you “sell” this lens as when it fits your lifestyle and visual sensitivity it provides a powerful solution. I prefer extended depth of focus (EDOF) as they do not affect contrast and can generally meet your near needs in good light.

@stratdoc That's why it's great that we have choice. In my case I'm an over-the-road truck driver and do an extreme amount of night driving, yet even with the multifocal lenses I still have fantastic night vision. I'm not attempting to sell anything, but am just relating my own personal outcome. The PanOptix lenses have been one of the best things that I've done for myself in years. I can see how people might feel bitter post surgery that they made a wrong choice whatever that choice was.

Before the procedure I could not read hardly anything Could not read a decal under the hood of a car (I am a professional mechanic) but now I can read prescription medicine bottles fine print at 12 inches. I can see perfectly at all ranges.. Do not need glasses anymore ever. I can see like a 10 year old kid can see the ice cream man at any distance. The halos sere large for a year but now 75% of the halos have been deleted by my brain. The halos never really bothered me actually. Best money I ever spent on myself.

The problem with multifocals is this: the peripheral portion of this lens is usually blocked by iris under the bright sun light and you have to use reading glasses or sun glasses to read. My daughter has this issue. I also had a cataract removed - first on left eye 10 yrs ago and then I’ve lost IOL in this eye 3 yrs ago in car accident . I had 3 surgeries: first on removal of the old IOL and then implanting a new IOL into the anterior chamber, promptly followed by its removal. After that I’ve completely lost all vision in this eye. 5 yrs ago I had cataract removed on my right eye and now, after 5 years I have IOL in this eye shifted down by about 2 mm. It looks (as much, as I still can see) I am gradually going blind. So, my advice to all of you - don’t rush with your cataract surgery and stretch time as much as you can. Remember it is not such a simple operation, as it looks on this video, and, besides, not all doctors are good doctors, especially, while they are working hard on their own assembly lines to maximise production (mainly money) .

I have had cataract surgery in my right eye. Shortly after surgery, my doctor went on vacation. Everything seemed fine with sight, but it felt like something was always scratching my eye. One morning about two weeks in, I woke up with no sight. I never recovered my sight as it was just after the surgery. I found out years later that the surgeon had left an edge when she glued the sac and it was causing inflammation. I had also been diagnosed with glaucoma in that eye. I have very little vision left in that eye and am reluctant to have cataract surgery on my left eye for fear of losing more vision.

I was misadvised by my eye doctor regarding which lens to choose. I chose the "middle" lens. I regret this decision and wish I would have chosen the multi-focal lens. Now I have to have readers in eary room and with me constantly. I cannot see close up at all and am subject to memory loss (Alzheimer's). Because of this ill-advised decision there are readers all over my home, on my person, in my car, next to my laptop, etc. Unfortunately I have a tendency to forget to return them to thier proper place. As a result I am always losing them. It's a challenge. So go for the best lens. Yes they are the most expensive but it is so worth it. I got the "better" lens and regret it. Get the best.

Thrilled with the drastic improvement in my vision, but i do now need reading glasses, which is a bummer, but i have hsd to wear glasses or contacts since grade school, so just needing reading glasses is not so bad. I was terrified before both surgeries. It's a routine surgery, but nothing is routine when it is your eye that's being cut on and removing and replacing some vital paet of your vision. Very glad that i had it done though. I just had the basic lens replacement that's covered by insurance and Medicare. I am one month post my first eye and 2 days post my second eye surgery and am seeing well enough to drive without glasses, watch TV, etc. It's great to be able to see at night again! Reading is good but not close up small print like On a jar I massively struggle. I also don’t like glasses makes me anxious wearing them as they slide up and down and because it’s bifocal they move slightly and the vision is out of contrast. I then need laptop glasses as well. So multifocal are meant to be my best option. What do you think Doc? 🙌🏼😇

The procedure was quick outpatient and it worked. I understand not everyone gets this protein build up but some do. Your vision starts getting clouded again. Not to worry just get the Yag.

  • @bdonkersgoed

    @bdonkersgoed

    5 ай бұрын

I don't think it was protein on your lens, but rather on the your natural capsule that the artificial lens is placed inside. This condition is called "Posterior Capsule Opacification" (aka "Secondary Cataract") and it's pretty common after cataract surgery. The YAG laser essentially blasts a hole into the capsule so that light can get through it again. I'll likely have to get the YAG procedure done as well.

The first two doctors recommended cataract surgery ASAP, but I was not experiencing most of the commonly described symptoms of cataracts, which left me confused. Luckily, before this latest visit I found your web site and learned much more about cataracts so that I could ask the right questions of today's eye doctor. Between your very good videos and the good fortune of finding a doctor who had the patience to explain my situation, I learned that cataract surgery will one day be a necessity, but is not now urgent. Thank you for the detailed info - it really helped! My advice to other patients is to beware of surgery mills (who I suspect recommend cataract surgery to just about anyone who walks through their doors), and ask lots of questions before you submit to surgery!

Your videos are so helpful and informative. Thank you for your willingness to share your knowledge and evaluations. An idea for a future video...Can you explore contact lens options for people with very high myopia? I have used monivision successfully for a very long time but now that extra years are making presbyopia more of a problem and with the addition of dry eyes, I am finding that my old lenses are not working as well and there are not many alternative options for people like me. I'm getting frustrated with the need to overcorrect everything! I live in a small community so I do a lot of online exploration trying to find newer, better options that I could ask my doctor to look into.

New subscriber, just got consult scheduled 04/2024 for cataract surgery. I had just gotten new trifocals that I could not adjust too, double vision in right eye predominantly. I’ve worn tri’s for many years, never could get comfortable with progressive lenses. I’m a retired LPN (graduated in 1971 - when patients returned to the unit having to lie flat, & heavy sandbags each side of head). Boy has this surgery changed in the last 50 years! My OD was hoping we could get by before having the cataracts addressed, right is worse than left, but this last Rx change I could not adapt to. Having neurological changes following a horrible case of bacterium sepsis in 2018 has left me with instability in walking & balance issues & the new glasses made things even worse. I’m fascinated by the technology changes in all of medicine, so it should be interesting to see how things will change for me in the next few months. Looks like I’ll be doing some binge watching to get educated prior to that consult. Living in Rochester, NY, home of Bausch & Lomb, seems they have played a role in many of the changes we are seeing today. Thank you for sharing your knowledge and expertise, it is very much appreciated, I immediately pulled out my gel drops and will make sure to utilize them regularly prior to my appt.!

Very informative video. I had my first eye done on October 3rd this year. Day two after surgery I was amazed at how clear I could see. Second eye done the 7th of this month. Today is the 11th and I’m pleased with the results. Can still feel there’s more healing to be done on the most recent eye and this time there was some blood residue in the eye. But so far very pleased. I chose the lens that makes night vision better. I will still need glasses to read. I can read pretty well but eyes get very tired with much reading. So far I’m very happy with the results. Thank for your video.

Glad I found this video. I have been going to this one guy for about 7, 8, years. He is 5 minutes from my house, for the past 2, 3 years I tell him, I kinda see double, and my new glasses have not been right. I decided to try an eye doctor, who found I needed a prism, and after a few weeks of test prisms they gave me a script that should have cleared things up. However, they did not take my eyeglass insurance, so I took the script to the guy next to me. (This was last year) when I received my new glasses Nov 2022 they did not seem right, the guy said it may take a few days to get uses to them. Well this Nov, I went back to the eye doctor and when they checked my glasses, they said it was not the script they gave me, no prism, and straight was wrong, and I was lucky to see/read anything. The guy near me put in some BS script and when he checked them, he said they were right. Needless to say, I nor my wife use him anymore, and I tell everyone. However, the Cataract is an entirely different story, the sergeant is trying to dig deep $$$$. If you have time, I can comment on it.

Recently I had cataract surgery on both eyes with one week between each surgery. I was used to wearing mono vision contacts for years so went with the mono vision lens. I also had an astigmatism in one eye and my surgeon was able to correct this. I was amazed how well my vision was after the first lens replacement. I have one lens for distance and one lens for close and no longer need any glasses. I was reluctant to have cataract surgery at first but am so glad I can finally see perfectly and do not need glasses.

My QUESTION for you is: Had Lasik surgery abt 20 yrs. ago. Have been using glasses for close and midrange (computer). Don't have any (or significant) Cataract yet (67 Yrs. old) But recently after eye exam my optometrist advised considering IOL option. Have to see a surgeon in a month. Am I a good candidate for LAL or better avoid any surgery while still have almost perfect far vision and just need glasses for near and midrange & some astigmatism????

When I have my lenses fitted I plan to have a mono lens for far vision as I think things become over-complicated if they try to correct my astigmatism, myopia and presbyopia at the same time. If my astigmatism changes it will be far easier to change spectacles than to adjust an IOL.

Thanks so much for mentioning PCO. I'm pretty sure I am one of the lucky ones to develop this. I had cataract surgery in July of 2019. It was not a good experience. Sometime between 2-3 years my right eye developed a haze. I can see through it, but it makes it difficult to read. I can drive on side streets, but not on highways that require quick reaction. I know I should have gone back to the opthomologist, but I had such a bad experience that I'm afraid of it being worse. I don't even know if they do any type of laser surgery. Now that I know it's an easy procedure, I feel somewhat better about checking into it. I really miss being able to see clearly!!

@karenmcgrady6349 Correcting the PCO is a piece of cake! I had both eyes done (at the same time) almost five years after my cataract surgery. Don't delay -- your quality of life will be improved exponentially -- and you will walk out of the Dr.'s office seeing perfectly again. Good luck! It is painless and no "recovery" or special/additional eye drops are necessary, either before or after the procedure.

I went with multifocal toric lenses. I can focus clearly from far away up to about 14 inches. With the addition of readers, I pretty much can see everything clearly. In fact I got some nice progressive readers at Zenni for around $70 that I wear all the time since I was used to wearing glasses. The nice thing is that I don’t require them for driving and so I can use off the shelf sunglasses.

I'm 50 and I have a presbyopia (+2.7/3.0 for reading) and cataract at its starting level (yet). Thinking about multifocal IOLs I'm wondering if I can kind of simulate how I would see with them after surgery using multifocal contact lens now (for example, Bausch & Lomb Oneday for Presbyopia).

I had bad vision anyway but still opted to remain myopic. I wish I had a toric lens as my astigmatism has continued to increase. My right eye could not be corrected to 20/20 sadly but more like 20/40. I did also need YAG laser treatment in both eyes.

Good info, wish it had been available when I had my surgery at a county hospital. Having uncontrollable Glaucoma, the doctor recommended cataract surgery as a last resort. He promised I'd be seeing better afterward, so I opted for it. It changed my vision from 20/25 without glasses to 20/70 and needing to rely on glasses. It improved my near sight, but I was never before bothered by having to wear 99c glasses to read. The glasses I have to buy now are expensive, and I'm struggling to see through them to drive with all the scratches. Apparently, the surgery didn't eliminate my lens. It seems to have given me a cataract I didn't have before. The doctor told me he just pushed the lens out into the eye, and eventually it would sink down out of view and not be a problem. During the surgery I woke up and looked at the doctor's hands and instruments, aware of having intense pain in the eye. Post surgery the pain was unbearable, and the doctor told me I shouldn't be having any pain, that he had done a great job installing the implant, and he prescribed me some Tylenol. In a few hours I was sent home still in pain. When I went back for follow up, I told the doctor I was not happy with my vision, I felt blind with the new lens. He said the surgery had successfully lowered the pressure in my eye, which was the whole point to doing it in the first place. He said the lens they used was the only lens the county provides, and everyone who gets cataract surgery at that hospital gets that lens. Wow, I thought, to save my sight, the doctor destroyed my vision while promising me better vision. I told him I wanted my old lens back, and he said I'd have to get financial approval for that. While I waited for approval, the county terminated my financial aid and kicked me out of their system. It has been about 17 years since then, my eye has floaters, and my glasses are so scratched I can drive only when the sun is shining brightly. In order to get new glasses, I have to have another optometric eval, and the doctor doesn't accept Medicare, my only insurance now. It seems strange that with a lens implant, I'd still need a vision check. Wouldn't the vision be fixed since the lens isn't going to change over time? I am thankful for my sustained sight, but I can't seem to get past the grief of compromising my distance vision to save my sight, because it has severely limited my ability to function as I used to.

I had Lasik back in 1998. I then had cataract surgery around 2015 in both eyes and chose Crystal Lenses because they offered the ability to focus using your eye muscles. I am seeing better than 20/20 near vision in both eyes, plus close to 20/20 in both eyes at distance. Crystal lenses required quite a bit of eye exercises for many months after surgery to get the best results, which I took seriously. I am quite happy with my vision as you can imagine. One question I have is, what category of those you presented, does Crystal Lenses fall into?

I had Crystallenses put in both eyes in August. One I was set for distance, and one was set for near vision. I can see pretty well across all visual ranges, but I have ghosting and streaks with my right eye when I look at lights at night. I had a different kind of visual glare phenomena in my left eye that was very disturbing, but it seems to have gotten a little better recently. I'm wondering if you have had any haloing, glare, or other strange visual phenomena at night with your Crystalenses. The doctors tell me I will neuro-adapt to these phenomena, but they are significant enough that I really don't believe I will ever be able to ignore or be OK with them. Like you, I am using the vision restoration charts, and my vision is improving slowly, but I didn't realize it was going to be such a long ordeal to be able to see clearly at all ranges. i'm really wondering if I should have opted for another choice. I would love to hear more about your experience, if you would care to say more, please. do you have mono vision are blended vision, ordered both eyes get the same lens? Thanks.

I had cataract surgery in late February and mid March what a life changing event. I had to wear thick heavy glasses since I was 3 years old and I am now 66 years old. I after talking with the doctor and surgeuy team decided with just going with the standard mono-focal IOL. The primary reason was I did not care if I still needed glasses post surgery. I was wearing +6.00 in the right eye and +6.25 in the left eye and a very mild astigmatism in both eyes and +3.00 for my bifocal. Well after the surgery I need no correction for distance in the right eye and a +0.25 in the left eye, basically 20/20 in the right and 20/25 in the left mid to long range and the bifocal power was determined to be +2.25. Just totally mind blowing for me as not needing glasses all the time but I still just grab my new light weight glasses first thing when I get up and take them off at night. If I am driving I can clearly see the dashboard and at distance with no problem and I will take the glasses off and slap on a pair of sunglasses, I also use the sunglasses when I am out doing my lawn mowing I had not glare issues at night driving just so totally different for me as I am able to see so much better then I ever did

? Hi Doc! re: Cataracts? Back in the ‘79-‘80 timeframe, I had the old fashioned Radial Keratotomy with 16 incisions in each eye? The results were fantastic and had 20/15 vision throughout my 30 yr military career including flying. I’m currently 68yrs old. What are the implications if I end up with cataracts? I think both of my parents had cataracts and my brother has had cataract surgery. I haven’t been told that I have developing cataracts or anything. I still get routinely checked up be Optometrist and Ophthalmologist Docs from my Dad’s side of the family. (Dad was a Pediatrician)

Fascinating! I wish I had heard this prior to my cataract surgeries 13 and 14 months ago. At 56 years old, my vision was going quickly. Within 6 months I was having cataract surgery. My surgeon’s office steered me towards multifocal IOL. Worst decision ever! Prior to the surgery, I had zero glare. Post surgery I have triple halos and glare on every single light. I simply cannot drive anymore at dusk or dawn. And also the overhead lights in stores and recessed lights in homes have halos and bright glare. I had YAG surgery 5 weeks after the first eye and need it on the other eye. I also wear glasses at all times. After one year nothing has improved. Is there anything I can do? Can this lens be removed and a different one out in?

I appreciate your channel and all the good information on eye hygiene and health, and I have benefitted so much since my cataract surgery last year from your videos on dry eye. But suggesting the YAG laser for PCO is an easy, simple effective fix without providing information on possible complications from YAG treatment (chronic floaters) is a disservice. I was led to believe in the same glowing terms that cataract surgery would be a easy fix for my age-related changes in vision, as I was requiring, for the first time in my life, prescription glasses -- rather than being informed that cataract surgery is a serious surgery with potential permanent downsides. Yes, they mention risks, but as you have done in describing YAG for PCO, it's presented as not a big deal. But it is a big deal to experience the downsides. I got a multifocal causing negative dysphotopsia from day one (which I was not informed about prior to surgery though it is a common issue with this lens) and 'wrinkles" in the capsular bag. I see halos driving at night in that eye. I have noticeably less contrast and clarity in that eye. I am having difficulty focusing the two eyes, which my optometrist says is because the lenses now are different systems - but no way will I relinquish my natural lens until/unless it becomes absolutely necessary and risk the loss of quality it gives me compared to the IOL. If I could have known the potential effects of the IOL I would have chosen the inconvenience of prescription glasses. In my estimation, surgeons treat lens implants like it's the default. People who have had excellent vision all their lives and never needed prescription glasses should not be led to believe the IOL will be equal to their natural lens and should be informed of the likelihood of complications, and know that treatment of PCO, which surgeons will admit AFTER THE FACT is pretty much expected, may result in additonal complications (floaters) that are not always resolved.

Greetings Dr Elisabeth, another informative video Thank You . If I may offer you a tidbit of advice ... Do yourself a favor and don't get old , nothing good comes from getting old. ! 😊 For the past 3 weeks I have been dealing with symptoms of a vitreous detachment, so far no sign of a retinal tear and go back in another 2 weeks for check up. Prior to this I never knew or expected that even healthy people can have all sorts of things happen to their eyes ! Stay Well.
