The 2023 Residency Match Data Explained... This Is The Worst We've Ever Seen

Every year, I look at the recent CaRMS data from the most recent medical residency match, but this may one of the worst years that I've ever seen for certain specialties. As of 2023, there continues to be a growing problem with newly graduated doctors that I think is slowly setting our healthcare system up for collapse... In this video we'll be going over the most competitive medical specialties, the least competitive medical specialties, how IMG applicants did in the most recent match and why the healthcare system is going to be in a lot of trouble if we don't do something to fix the current issues with interest in family medicine.

Hi everyone, its nice to meet you 🤙 My name Is Gianluca and I'm a second year family medicine resident physician at McMaster University in Ontario, Canada.     I believe as an IMG that the doors are still open for recent graduates to join the system they just need a little guidance not to wast the time and start a making use of the time they have for research, volunteering, electives & high exams marks. Yup the window is very small to join the same year you graduate but still promising from what I’ve seen through the video. All the best to all the MGs out there!

    Hi everyone, I hope you enjoyed today's deep dive and my own thoughts on everything - please share any thoughts that you have about this year's match data and remember to like the video if it helped! Also, I would love to hear everyone's thoughts on the Queen's "family medicine exclusive" medical school - perhaps for insight on a future dedicated video 🤔I certainly didn't know which specialty I enjoyed the most when I first entered medical school and I think that it's unfair to students to lock them into a profession without getting the chance to explore other career options. Also, in addition to my other criticisms that I talk about in the video, I think we're going to create a lot of unhappy doctors by forcing people into positions instead of incentivizing that position and allowing people to choose it on their own. Happy to discuss and see you all in the next one!

    as a three-time omsas applicant i was obviously happy to see that an additional option was being made available for us pre-medical applicants in the form of the queen's family medicine program. however its interesting that (anecdotally at least) ive heard the same, overall critical sentiment voiced against it from actual doctors/residents. let's see how this turns out over the next couple of years... i would love to see data on the rate of attrition (or perhaps, possibly, a lackthereof) in this program and see how many are content with their education.

    Hi Sir , i am IMG , THIS WAS 2ND YEAR i APPLIED IN Ontario's FM program and was not matched even i had interviews last year and this year too , with this years end interation interview went really well with the interviewer saying that 'WE HOPE YOU CHOOSE US " but still did not match. I have graduetaed from a med school in India in 2011 , I have recency of practice as I went to work in INdia for couple of months in 2022 and from Last year I am volunteering in a FM clinic. I even have good scores IN MCQE1 and NAC OSCE and have volunteer experiences in various fields including mental health. Still i am not being matched .Hoping to hear from you .

    Very good recap. Thanks. One comment I would like to make is the US match happens before the Canadian and thus it is highly risky for an IMG to forgo the US match and hoping to match in Canada where the chances are less. It is unfortunate that the bi-lateral agreement on matching obligates one to accept the first match and thus removing your application from the 2nd match which is in Canada. As a Canadian who was an IMG, I only applied in the US and thankfully matched but I did want to come to Canada. One last point, the US match is much more competitive as graduates from around the world want to train in the US so USMLE test scores are very important whereas in Canada the MCEQ1 score is not relevant. Finally, many lucky IMGs that do match in Canada had some sort of contact with the program that helped in being accepted.

    For family medicine in Ontario as an IMG in the first-iteration, the MCCQE1 was used similar to the USMLE step exams where the scores were used as a cutoff for interviews, if one didnt get above a certain score, they didn't get an interview.

    it's gut-wrenching to think there were 100 unfilled spots and I applied to Fam med in 2023 and still did not get in. This IMG life is hell.

    most of those spots were probably in quebec, since those programs need french speaking applicants

    ​@stun140 Actually, surprisingly alot of the unfilled fam med spots were in Alberta this past year. It is unfortunate though becauss the CMG and IMG positions are separate so they are not up for grabs for IMGs.

    Sorry to hear about that @Abdullashora I'd love to hear more about your results if you're open to sharing (I am asked about IMG matching quite a bit) Did you apply broadly, to schools across Canada? Was it your first time applying? Did you meet the cut-offs for the NAC OSCE and MCCQE1 No pressure, but it would be helpful to know. Thank you :)

    @@nxtgenmd I only applied to Ontario programs. I kind of went into the mccqe blindly thinking I just needed to pass and got 240. Got 1407 on NAC, but I think I just screwed up my 2 interviews. Doing the nac again this year hoping for superior performance and applying everywhere this time. I'll see you in a residency program next year! 😎

    MONEY Offer full tuition paid medical training by student loans. This would include the tuition portion paid by governments. And should include a small living allowance. People who enter medical field have their student loan forgiven if they serve for a number of years. Those who choose to practice else where need to pay back their loans. For example, if a medical professional decides to work outside Canada, they would be required to repay their full student loans. It a person choose rural medicine their student loans be forgiven in a shorter time. This is basically what Singapore does.

    While I'd certainly love if all my student loans were forgiven tomorrow, I do know that a large portion of the Canadian MD education is heavily subsidized by the government (comparing the international fees and Canadian fees for medical school you can see a very large difference). There is a loan forgiveness program in place for people who choose to work rural. The problem with it (as I see it though) is that even if you apply for it, in many cases you'd still be better off in a well paying specialty position in the big city than a rural family doctor with loan forgiveness.

    @nxtgenmd The difference is that education cost is not subsidize. So as result education loans would be much larger. Singapore also does this for Engineering. Canada is NOT retaining its Engineering talent either. This makes it hard to innovate and expand manufacturing. Rather subsidising education and hoping people stay. Subsidise loan repayments of those who do stay. Student loans available for all success applicants. The number of applicants required, drop out rate all needs to be open access. Adjustments need to be based on data. Canadian Democracy crimped by the lack of open information access. Democracy requires and informed public voting in fair elections. No Democracy without Accountability No Accountability without Democracy

    Hey! just had a little note on the new queens program. I attended a seminar on the program and they made it clear that you aren't locked into family medicine, BUT most of the training is really geared towards family medicine.

    Hi Jayden :) Thanks for the insight - I tried to find a recording of that seminar but was not successful unfortunately. their website now does describe the program as such: "This new, concentrated-training model will develop community-focused family doctors who are specialized in offering comprehensive care to a wide variety of patients. This program differs substantially from the standard MD program in design, curricular delivery, purpose of electives, and freedom from the need for postgraduate residency matching. For these reasons, transfers to other programs, including the standard Queen’s MD Program at the Kingston site, will not be feasible." Because Canadian residency programs do not consider exam marks (and medical school is entirely pass/fail these days), clinical electives and narrative building is extremely important. For this reason, even if you are not locked out of CaRMS, in effect it would be the same thing

    I am a med-1 in the class of 2027 and for my cohort onwards, FM residency is going to be 3 years which will cause more people to walk away from FM. To be honest, most of my peers have no intention of pursuing FM because if you are in for 3 years of FM and 1 year of fellowship you could just go for ER or anaestesia for 5 years. FM in Canada will be a job mostly taken by IMGs in 2027+ because all CMGs will prefer taking competitive specialties or move to the states.

You’re the 2nd person to tell me that the 3 year family medicine residency has been made official but when I’ve asked about it recently to some friends in higher places, they didn’t know anything about this recent development.     What is the work load of a family medicine specialist compared to Internal medicine? During Residency, Which is more tiresome, Family medicine or Internal medicine.? Because personally family medicine seems a nice one for me given the 9-5 schedule, no night calls.

    One of the biggest hurdles are the USMLE step exams. Many Canadian schools do not use the traditional NBME testing metrics in clerkship and the material between the 2 countries can be quite different in my experience. In addition, there are a variety of other hurdles as well that makes the process much more difficult, but not impossible for anyone who really wanted to :)

    Perhaps family medicine is seen as not glamorous. They could also be chased away at being owners of a small business and practicing doctor. They may also see the $$$ they may make as important. Sorry, if you got into medicine to make big bucks vs helping people, you picked the wrong career. The new family medicine doc I found works a normal week. The clinic, despite having four doctors, is closed on weekend an open 9am to 7pm. There is a hospital a two block walk away.

    If there were two jobs, both of which allowed you to help people and use your medical knowledge in a gratifying way - I can't blame people for choosing the job that paid more. Making money isn't a bad thing, and between paying off student loans and making up for the last 10 years where you didn't have an income, I don't believe that handicapping pay is the solution to the family doctor crisis. Just my opinion though :)

    Cardiology is an internal medicine sub specialty - it doesn’t show up in the R1 match. I.e you can’t match to cardiology after medical school - you need to complete internal medicine first

    Yes. You have to be a permanent resident to be able to apply.

    around $350 as a base rate, and then another $60 for each program application after 4. I did meet one student in my year with 22 applications, but the average is probably around 10. I only applied to 4. Depends on your situation :)

    My theory is it’s the AAMC which deliberately chooses to keep residency slots limited to keep doctor salaries high
