Testosterone Replacement Therapy (TRT): What Happens When You Stop?

Are you tired of taking testosterone replacement therapy and thinking what happens if I just. Stop taking it. Maybe you just don't want to take it or you don't think it's helping you, but you're worried about side effects or you don't understand what can actually happen if you just stop taking cold Turkey.
So a lot of people are asking the question and want to understand what happens when I stop taking testosterone replacement therapy (TRT). So what I've seen in my practice is that the levels of testosterone will start to rise again, but most people don't wait long enough to see those levels rise.

So instead of returning to their baseline or pret testosterone levels, they retest or restart, the testosterone too early. So let me walk you through a week by week scenario of what a typical person is going to feel and what's going on with their testosterone during those times when they stop taking. What happens in the first week when you stop taking trt is basically nothing.
Your blood levels of testosterone will actually be a lot lower in that first week. But at the tissue level, you're still gonna have plenty of testosterone there, and the typical person is not going to really notice. That drop in the serum in the blood because the tissues are still very saturated with the testosterone.
On week two and three of stopping testosterone replacement therapy, you are gonna start to see a decline in the testosterone levels at the tissue level, and with that, you'll probably notice a decline in your energy and any other benefits that you're getting from the testosterone.
Now, when I say energy, I also mean like motivation, cognitive performance and things like that. Maybe you'll start to see about a 25% decline in week two and three, and it's in week three and four that your levels are going to be probably right about at their lowest. This is the point right before your body starts to kick in and increase its own production of testosterone.
That tissue levels on week three and four are basically bottoming out, and you're gonna be at the height of your withdrawal symptoms and Fatigue symptoms. How soon those symptoms come on. And the severity of those symptoms is really gonna depend on how much you were taking, how high were your levels, how much benefit were you getting out of that testosterone and where you started out to begin with.
In week four and five, your LH and FSH levels are gonna start surging, and because of that, your estradiol levels are also gonna go up. Now, if you don't know, FSH and LH are the hormones from the brain that stimulate the testes to make testosterone on the LH side and sperm on the FSH side.
The FSH happens to be an aromatizer. so it also will stimulate more estrogen production. So even though you're producing more testosterone as a result of the surge in FSH and lh, a lot of it's gonna be bound up because you're making more estrogen. And it's, in that week five and six range is when most people are gonna be back to their, pret, pre testosterone treatment levels, back to their baseline, whatever that started out to be.
Now, the reason I say five to six is because there's always gonna be some variance with individuals. It's gonna depend on where your testosterone levels actually started out at, and then how much you are taking to supplement. So if you're taking a huge amount. It's going to feel a lot different than someone that was taking a very small amount to adjust their levels. Some people are gonna have more severe symptoms and feel very, very bad when they stop taking testosterone.
So it really depends on what that variation is between where you started and how much you're actually taking. And if you don't understand that from the beginning, you may want to look a little bit closer at what your levels look like Now, before you stop taking testosterone, maybe you want to just reduce your dose first for a few weeks or even a few months before you just stop cold Turkey.

    I'm 47 and my testosterone levels are at 398. I'm still jacked and still building muscle even with low T levels. I've been training hard for 25 years and no physical symptoms of low T. Dont lose hope guys!

    I’m 23 with Post SSRI Sexual Dysfunction, 3 years off the medication lol My T shows normal but symptoms don’t match numbers and the numbers don’t explain my symptoms of genital numbness, anorgamsia and difficulty climaxing that’s onset began with lexapro and persisted afterwards… just a reminder to men wanting to treat their depression or PE, be careful with SSRIs and SNRIs

    Been on T since march. Ive gotten jacked af. Wife and i are trying to have a kid, been struggling. Got my sperm Analysis today and ive got like 0. In really nervous about getting off TRT, im worried im gonna lose muscle and gain fat. But ill do it because my wife needs me to. Pray for me brothers

    After two years of 100ml/wk, I wondered how much I really needed it. Cut by half for two months. Spoiler alert: motivation tanked, fatigue increased, etc. Everything degraded enough to remind me why I went on it in the first place. I didn't even bother with new blood work - I just went back to the dose my Dr. prescribed and a few weeks later I felt normal again. I'm 50 and this is typical. I was not surprised, but I wanted to verify. I do not suggest younger people take this stuff unless you have a serious medical condition. TRT is _restoration_ for those of us where nature (aging) shut us down. This is not a magic dose to make you more manly - quite the opposite. Taking it too young can essentially _make_ you dependent on it by lowering your natural production. If you have enough natural production, keep it.

    One should *not* stop TRT. One starts TRT for a reason: low T symptoms. When you stop you go back to the same insufficient state with all the symptoms.

    I agree most people should not but depends on the risks vs benefits. Not everyone benefits from trt ( although the vast majority do). Some people also have side effects that outweigh the benefits. Always talk with the prescribing doctor to make this decision.

    Hey doc just had a question so im going to be completely honest ive been taking test cypinate for about 6 months now and ive recently gotten blood work done annd as of now my. Levels are about 360 range i would like to stop the injections and try to maintain my levels naturally but im afraid of going back down again ive been taking 1cc a week could you give me your thoughts or recommendations on what i should do next should i continue to stay on them or get off the main reason i would like to stop is i dont want long term side effects and i suffer from acne on my face and head thanks 👍

    Hey doc, thank you for this video. So if what you are saying is that if you titrate down at some point you fsh Lh will begin to trigger endogenous production even though you are still injecting a small amt of exogenous test?

    Thanks for the video, I have been on trt for about 2.5 years and am thinking about stopping for various reasons. I will likely ween myself off if I choose to do this, are there any all natural supplements you would recommended to take while coming off of TRT to help manage the hormone changes?

    Great content!!! From what i understand if you take testosterone, your body will eventually return to producing it normally (as it was before treatment) after a month or two... or will it slightly decrease? Question, if i take an average dose of testosterone for lets say 3 months. And once im off the trt.... My bidy eill produce more estrogen. Is thst permanent? Long term? Or just a few weeks short term?

    Increased estrogen production is temporary and directly related to the amount of testosterone increasing. The surge in fsh that comes back on (when u stop TRT) increases the conversion of testosterone to estrogen.

    Hey doc , thanks for the videos ❤. Please help me :) went on clomid for extended period as PCT (6 weeks) , libido was ok while on the clomid for first month but then it went crash, was on trt for 6 months at a heavy dose. Stopped trt at start of April and Clomid towards middle of May, the drop in libido was really head wrecking for me and I went back on the clomid, it may improvements but again after two weeks I crashed so I’m cold Turkey with a month. It’s start of September and I’m still crawling in terms of libido and strength is way down. I went to doctor who said I should wait the three months. My question is should I try HCG injection as I have read in a number of sources that extended use of clomid can cause this ? My partner and I are trying g for a baby and what are the bounce back timeframes in terms of fertility, Thanks in advance

    Dude, YES, try HCG. Probably what you should have done from the beginning. Do some research on it. There's some great info right here on youtube. This guy named Steve who's very vigorous has some great info. The HCG will act as LH and help stimulate the testes to come back from vacation. It's essential to regaining testicular function. Another option would be HMG.

    I’m not sure those are the only factors but could be a big part. If you were not feeling good before trt and you did not make any changes to diet lifestyle you will go back to not feeling well.

    Hi Doc, I was pelleted in mid-April of this year (about 6 months ago). The doctor gave me way too much (12 pellets total, levels went above 2000), and consequently I had a number of adverse effects. I extruded 3 pellets over the last two months (yes, after the incision site had healed- 3 times over), but I am roughly at the 6 month marker and have decided not to re-pellet. I initially did not notice much of an effect, but all of a sudden I am getting super sleepy and fatigued about mid-day (this is not normal for me). My motivation at the gym has decreased slightly, but I am still able to push myself to go (and my lift is close to what it was- I am losing strength). The last pellet extruded was about 2 weeks ago. Any idea if this sounds normal, and what I can expect? The pellets that remain are super small now (the one that came out recently was at least 70% dissolved). As I understand pellets, they are essentially NOT doing anything for me at this point, but there is still a little left in them. Do they have to be completely gone before my system restores to "normal".

    Yes they would have to be completely gone. 6 mo seems like plenty of time for them to be gone u would have to check your levels specifically LH and Fsh. I don’t do pellets so not 100 percent sure.

    Thanks for your content. My T level started at 38. Yes, it was 38! I started TRT in 2020. My last bloodwork was 400. I take .5 mL twice a week. Biggest side effect is my testicals are sore 24/7 and have shrunk to raisins and I cannot get anyone to prescribe HCG. Very frustrating.

    Hey Doc, I was on trt for about 17 weeks. Cypionate. 200ml per week for the first 12 weeks and 250ml per week for the last 5 weeks. I stopped cold turkey almost 3 weeks ago and haven't noticed many negative symptoms besides more fatique (never really got good benefits from the trt anyway), but now im dropping weight like crazy. Im only at a 500 cal deficit so I should only be losong 1lb of body fat per week. In the last 3 weeks since coming off trt I've lost 8 lbs so i suspect 5lbs of that is muscle mass. I gained 7lbs total lbs of muscle mass while on trt for those 4 months (as per the bodpod) and now im worrying big time that I'll lose all my muscle that i was finally able to gain with trt. My acne surged about a week or so ago, indicating the hormonal change and my original natural T level was 478. How long do you think it'll take to get back to normal levels so i can stop losing muscle? (Never did blood work while injecting so no clue what my enhanced T levels were.) TIA Doc.

    That depends entirely on the human, your health, diet, and activities. Which is why you won't get an answer. In general the half-life of most esters is in the 8-9 day range, so you'd think it's an easy piece of math. The truth is the values are dependent on things like adipose tissue amounts (fat %) and probably 20 other endocrine systems. Heck, a few nights of bad sleep will throw your numbers off 100, even when you are in treatment. Assume an experiment where you went off cold, took a daily chart of your descent, then went back on. You repeat the experiment a year later. You are likely to see a completely different descent profile the following year.

    @@justinflint3313 google tadalifil it boost your testosterone… I was on trt 50 mg every 4 to five days for two years. Stopped two weeks ago and using 5 mg tadalifil in the morning and 5 mg in the afternoon I will wait two months get tested and titrate the tadalifil to 5 mg a day . And hope for the best . Good luck.

    hey doc how fast would testosterone propionate as a trt timeline look like compared to enanthate/ cypionate. owuld that still be tissue saturation at week 2 or is it going quicker since blood levels dip quicker? propionate is like half or shorter of the duration half life of enanthate/cyponate

    When you return to baseline after stopping TRT, is it clear that the baseline won’t be even lower than before you began the therapy? I wouldn’t mind taking some test, but if I don’t always have a supply of it, I don’t want to be shooting myself in the foot in the long run.

    Nothing is certain but many of my patients do return to baseline. The longer you are on it the more likely it doesn’t return to baseline

    My total t before stopping was 989 I did a cycle and ran TRT for a while after it. I have currently tapered to 12.5mg test prop daily subq injections and I want to quit it due to problems with hot flashes and some anxiety sensations . I was thinking of now just adding in 250iu or HCG twice per week while tapering the test down over the course of 4-6 weeks and then maybe run enclomid for 4 weeks after . Is the clomid really necessary? I have daily headaches too and don’t want to keep using it anymore . I also don’t want this to effect my body composition as I worked very hard for my current physique .

    I suggest you find a medical practice that helps people get off PEDs. There is some serious chemistry involved and the fact you "tapered" to what is almost the max FDA weekly dose suggests you did a lot more. Plus the steroids. HCG is hit or miss (I regret trying it because it caused serious facial rosacea). From your comments I assume your composition is not natural, and that you are younger. Please don't take this the wrong way, but you might want to look into some therapy to accept yourself with a more natural form. You won't become a walking stick figure, but people coming of PEDs often _feel_ that way. This is a sign. Cycles have lifetime consequences. I have met few 50 year olds who were happy they cycled PEDs in their 20s and 30s (I'm over 50). I do not want to sound condescending (internet comments kinda suck for "talking" with another person), so please accept this as some guy just offering some ideas to a fellow man. Good luck. Hey: quick edit. At my age and a small replacement dose of Test (I am in the mid normal range), I still have broad shoulders, narrow waist and low body fat. Good definition, and if I work it I can get the six pack. Even with kids. Friends say I look "jacked", but nobody would mistake me for a competition builder. I don't measure biceps. My wife says I have the "old school GQ man build, like Beckham in the 90s before every man got 'roided up or went trans." She's a doll (I'm no Beckham). Point being: men like us - who do the work and watch our diets - are the top 5-10% of the western world - _without cycling gear_ . Take the win, my brother.

    Sorry, this is not what I'm experiencing at 58 years old, a decade of TRT, starting level of 216 @ 406 lbs, after 5 months off my number dropped to 68 @ 185 lbs. I'm now on HCG fighting my way back to normal. Its been a huge roller coster ride, with waves of "morning sicken" days, and days with no "push" to run pass a few miles. If I could go back in time and tell my younger self not to do TRT, I would. Instead, I would've told myself to fix my body and learn about real food nutrition instead of modern day, marketing distractions. I will win this fight and be 100% free of any medication for the first time in my life. TRT is no longer for me!

    @@espada9 I eat fatty pork and beef, tons of fish, shellfish, seaweed (not the Costco type for rice) all types of vegetables. I also have two supplemental Rich drinks one in the morning or evening. The morning drink has a whole lemon, apple cider vinegar with electrolytes, pumpkin seeds and Brazil nuts I'll sweeten with monk fruit extract. The late afternoon drink is collagen, rich with four scoops of bovine, two scoops of marine, six tablets of type two, and sweetened with Monk fruit extract. I also sleep with my mouth taped as it helps with my daily running. I normally run between five and 13 miles daily daily. I have never ran in my life until three years ago a group of retirees adopted me and taught me how to run. I am one of the younger runners at 58 years old and the old is Runner is 78 years old. I have never played sports and I could never run one time around the football field without stopping. The Running Group has been so understanding with my morning sickness, as well as my hot flashes as the leader of this group is a Grandma of four grandchildren with Moore on the way because she has six adult children. She is a 5 foot two petite lady of Asian dissent and she is tougher than men twice her size. My goal is just to be normal for once in my life before it's too late. As I want to experience simple things that I have missed because of my size. I am located in Hawaii and I cannot find any doctors that have practical experience with someone in my situation. All the doctors here take educated guesses as what might happen, so I'm really on my own as far as my medical needs are. Sometime in September I'll take another testosterone test however, the way that I feel now I think my numbers are going to be above 800 just on hCG, because for the first time in over a year, I can run every single day and not run out of "push!" my short term goal is to run a Honolulu marathon in under five hours this coming December. As last year, which was my second time, my finishing time was 6 1/2 hours all because I got off of the TRT and my body was crashing, it took me in 90 minutes longer to finish my second year.

    @@espada9 I eat lots of fatty pork and beef, in addition to fish and shellfish, with vegetables and all types of seaweed. I only use lard, tallow, coconut oil, sesame seed oil, macadamia nut oil, and avocado oil. I supplement with zinc, copper electrolytes, taurine, creatine. Apple cider vinegar, pumpkin seeds, Brazil nuts, non fortified nutritional yeast, ginger, turmeric and whole lemons. And I only eat one meal a day. Side benefit has been most of my white hair has fallen off as I have a full head of 99% black hair.

    Keep it up brother! i am 4 months off TRT. I also cycled anabolics from age 20-30 blast and cruise style. I came off and really feel good, My libido isnt as great... but ill take it. Better off for me versus relying on a medication for the rest of my life. My sleep is awesome, daily headaches are gone, acne is gone, and my training is finally getting better.

    @swestintegrative -- will there be a long term or short term dip in libido? The biggest difference I noticed was that drive returning. I haven't really had any negative side effects since starting over a year ago aside from remembering to take it weekly (went to Testy Tuesday to remember). Thoughts?

    There will be short term drop for sure but if it is helping your libido that probably will go back to your original baseline over the long term

Before starting I was in the 150-300 range I had a lot of symptoms with fatigue etc but I didn’t struggle with libido at all. Now that I’ve started trt i have increased energy, feel great, but the one side effect I’m having is low libido/borderline ED. Do you have any suggestions on what could possibly help with that ? I’m supposed to start going to 200mg once a week soon.

    I've been on trt for about 10 or 12 years Im now 69, I stopped because the injectable wasn't lasting the 3 months it was suppose to anymore as a result I was starting to feel like sh*t about 5 or 6 weeks before my next shot and my doc wouldn't give me the shot till the 3 months was up then the Covid bs started and I stopped seeing the doc and taking the trt therapy. Its been one year now I feel great though I have lost some muscle but feel better my balls have grown bigger again which is a plus. But I still wonder if I should get back on it. I really hate taking medications of any sort. Currently am on no medication what so ever. Feels real good that.

    Hi. What’s wrong when a man’s testosterone level is high from TRT but libido is still very low? This happens to my husband from Testosterone Pellets which is at 0.09900ng/mL Free and 10.6ng/mL Total. Progesterone is 0.483ng/mL Estrogen 49pg/mL.

    His progesterone is high and estrogen is high-normal. His ratio of free to total test is off - his free levels are fairly low. That combined with the higher E and P ...my guess is he is overweight? Adipose (fat) tissue has higher aromatase, which metabolizes T into E. Script mills will tell him to take an aromatase inhibitor, but that can cause low libido (and makes people feel like garbage). The real answer might be getting diet, exercise and sleep in order. If he has high body fat, he needs to get that down. Testosterone is not a magic elixir - it only enhances what you have. So the answer might be, "do it the hard way". If his weight in in check, his next stop might be a urologist. Also ask if he's into porn. That stuff is massively destructive to "real" libido - meaning interest in actual humans. Don't judge or get angry. But maybe work with him. edit: I forgot to mention alcohol. Wow. That stuff is bad, especially when we get over 40. Try going sober two weeks and see if that makes things better. If two weeks without drink is too hard...that's the sign he should plan to stop drinking _forever_ . Help is out there.

    I had my shot of TRT last week 400 milligrams I believe The doctor doesn’t want me coming back until every three weeks basically at 12 week Treatment And then retest this is normal plus how long does for libido to kick in? Thank you, DOCTOR

    It’s normal for some doctors to do it every 3 weeks but but will be all out of your system in 7-14 days. So most do it every week at much much smaller doses

    I have a TBI and my T is low without TRT. I had a great high healthy score 2 months ago and a couple weeks after this crazy NP lady just took me off 200mg IM injections every 2 weeks. No wean down, it's been hellish especially since I have some psych issues that have just gone wild and am quite depressed. I found a new dr and had another test it has dropped over 400% and is low again. Results came back this weekend so waiting for tomorrow to get back on it.

    Kind of, im on week three of not taking anything and I feel great as of now. Looking forward to the journey. My drive is still there, my nipples are finally back to normal, I can still lift heavy and exercise, I have no issues with focus, I dont crave caffiene, I feel better than when I started it

    @amateurbiohackingbelgium I'd say about 15 months. Now while I was on it, I got spinal decompression done. The lower part of your spine is connected to your reproductive organs. I'm not sure if that has something to do with me being revitalized but I believe if I went that route first I would never have gone on it. I didn't quit cold turkey I slowly tapered down and started lifting heavy during the tapering. Kept a high protein intake and minimal carbs.

    So guys, I know I’m gonna get a lot of hate and stuff but I’m 22 almost 23 in February. But I’ve been working out for 2 years and have always done research about peds. I was stupid at one point and did rad 140. Did not know you had to do test with it. Test was bottomed out when I came off and didn’t go back to normal for months. This was like a year ago but I’ve jus the other day decided to start a cycle of test. I did my first shot yesterday and I’m kinda having regret.

    I just stopped TRT after 20 months on @ only 100ml cyp dosage every 8-12 days. It's been 5 weeks since last shot and it was a bumpy ride up until 4 days ago. My sleep was bad, waking up every night at 2-3AM not being able to get solid rest for rest of night. My energy was terrible, depression, anxiety and worry. Loss of muscle mass. I just recently began to feel somewhat better however my libido is almost nothing! I was crazy great with libido prior to stopping. My question is if I take another pct dosage of HCG with clomid/nova, will this help restore my normal function sexually? I understand I will not be as crazy with libido desires as on TRT (that is one reason I wanted to stop TRT, just too horny CONSTANTLY) My age is 63 and there are other reasons I wish to try and stay off TRT and be natural if possible. Thx

    The testosterone ester also plays a huge part in how much time it takes for your body to start producing testosterone again once you stop TRT. I have been on TRT for about 1 year and a few months between 2020 and 2021 using testosterone undecanoate (Nebido) doing the recommended 4ml (1000mg test) every 3 months and my body started producing testosterone again after 8 months. It was the most horrible experience of my life. I stopped taking nebido in June 2021 and only in January 2022 my testosterone started to rise and actually increase past what I was having before TRT. I started TRT because my blood test reported ~3-4 nmol/l of total testosterone and almost 0 free testosterone. While I was off TRT, between June and January, I was having almost 0 total testosterone left in my body and once January hit, my blood work reported ~13-14 nmol/l of total test and ~10-11 pg/ml. At that time, I was 21 years old and unfortunately, I had to start TRT again last week because my levels dropped before minimum again, being 7.67 nmol/l of total test and 3.99 pg/ml of free test, but this time I am using testosterone enanthate because I don't want to ever go through almost 1 year of having literally no test agian.

    @@iamthefiremanjj Currently I am on TRT again because my levels dropped below "normal" levels (although the range is between min and max is pretty large if you ask me) and I started with Testosterone Enanthate (bought from an UGL lab as nothing other than Undecanoate and gels are sold here from pharma grade) and started with 175mg/week and later dialed back to 125mg/week as my Estradiol was pretty high (112pg/ml). I am currently on Androgel (testosterone gel) as I want to see if my E2 self regulates with daily testosterone because even with 100mg/week of Enanthate and using 2mg of Anastrozole, my E2 would not go below 80pg/ml. Currently I am trying to find my best Androgel daily dose is as with the recommended starting dose, I am at 7nmol/l of total testosterone.

    Start first with cleaning up your diet, organic high quality meat, food, no junk food and processed crap, go to the gym lift weight, squat, sprint as well, slow down on porn. You’re young your levels should naturally go up in few weeks/months buddy

    No it won’t. The main thing is it going back to where u started before going on testosterone. It won’t increase beyond that wether you are young or old. In some cases it will be lower than when u started

    I started at 47yrs old, my levels were 504 on 1 test & lower on another. My Doc said hes never seen someone that low.. So I started 2000mg/10ml (1ml injection per week) Im 53 now & still take TRT Injections however, I've decreased & started kind of micro dosing i suppose. I quit for 4 weeks, because my levels rose to 1200. *Natural - I noticed energy increase like no other a week after starting TRT with penis / testicle growth. I would get hard just driving down the road or mowing the grass. Slightly painful erections & I went from 6" to 7.5", also larger in Girth, so Momma is happy. Plus I'm on Blood thinners, give blood & I also take Anastrozole for the back pimples & other side effects. Hairy shoulders bother me but I shave that. I'm 6'1" 175lb and I dont work out or interested in getting "Jacked". I have a dresser full of Testosterone boxes unopened, yet. I take depending on how I'm feeling now. I don't notice anything when i stop taking for a month or 2

    Why did you stop for 4 weeks when the level was 1200? That is actually perfecto. They say 750-1200 is what you want on TRT. That is the youthful level. I got to 2650 because they had me doses too frequently. I did not even notice the difference between how it feels with 1200 versus 2600. So less anything above 1000 is just more of the same.
