The mouthpiece is used to inflate the reservoir under his arm. He then squeezes it between his arm and side to produce a tone. That way he can produce a consistent tone without worrying about breath control. The finger-work controls the pitch, just like on a recorder. The pipes coming out of the top are drones, so they play the same note all the time.

Went to see my friend in glasgow was lucky enough to see them perform on the streets a few times best ive ever heard

Watched my 3years old great nephew listening to his poppy play harmonica like a drum kit and he was jumping up and down like ska dancing kicking up his heels and toes and playing bongos on wooden boxes smiling like he was in his best place. Fabulous stuff here. Makes me want to get dancing like I was a young lass again.
