Shoulder Dystocia : Risk Factors and Management : Dr. Shonali Chandra
The one emergency situation where an obstetrician does not want to land is shoulder dystocia, a rather unpredictable and unpreventable event. However all labor room personnel need to be aware of the risk factors, the management protocol and the maneuvers to help conduct a safe delivery under these unforeseen circumstances, the "shoulder dystocia drill". These basics are also asked in the postgraduate entrance exams with emphasis on diagnosis, risk factors and the conceptual understanding of the Mc Roberts Maneuver.

Please help! I went through a shoulder dystocia during my second pregnancy. Thank good my baby boy was born healthy, but I was told that he may have a fractured clavicle. After an X-ray this was dismissed. I am grateful to my team of medics who saved us but people have said that if my son did have a fractured clavicle then it is malpractice? I’m confused. These people saved my baby. Is it common for baby to be born with fractured clavicle during this emergency? Is it malpractice?

Hi Lucy, a fractured clavicle may have been a measure taken to decreal the diameter of the baby's shoulder's during the shoulder dystocia. Considering alternative injuries (like your baby asphyxiating because of prolonged cord compression), the OB's must have considered that the benefits of performing the fracture outweighed it's risks. I'm curious how low the shoulder dystocia lasted. I'm also curious about how your baby is doing, since most clavicle fractures heal very nicely with no complications. All the best!
