The "Pestilence" is basically sin. In the bible, Pestilence is mentioned exactly 49 times referring to SCP-049. God used Pestilence as a punishment for sinners. He is speaking of the great dying which is mentioned in the bible. Also he killed Dr.H because at first he wasn't doing anything to interfere with 049. But later he started restricting his amount of deceased animals. That means he started sinning because he got in the way of gods will. God then suspended him in pestilence which 049 sensed. (Note: this was not in my own words, full credit goes to Marat Bodoev)

Who said he was mad, he may have another sense from us or a much stronger one of our senses that allows him to recognize it. As he’s said in interviews not everyone was gifted with the ability to see the pestilence like he can. And like he said himself his current cure for the pestilence is flawed, and definitely not as good as it could be. So really we won’t know if the pestilence is real or not until he makes another breakthrough on the cure to the pestilence, and see what the “cured” look like after the more perfected version of the cure is formed. Perhaps they’ll look even more “zombie” like, or perhaps there appearance will be that of the original and they won’t be brainless and they can explain exactly what not having the pestilence feels like. Like I said we don’t know if it’s true that there actually is a pestilence until SCP-049 makes another breakthrough in the “cure” of it.

Yes he does have a very smooth voice, he has been alive for at least over 3 centuries I think it was, regardless he’s spent a lot longer in this world then us, gaining wisdom and enhancing how fast he can learn and understand things, so it only makes sense through his many interactions with people he knows how to make his voice sound smooth.
