Mine was the Game Over Screen from the PSX beat' em up called The Legend. I was young and came over to my cousin's house and they were playing it. My aunt called them up to help make lunch and I continued the game foe them. When me and another cousin got to the first boss, it c zoomed in to the guy and that scared me, in continue as my character dies and zooms in to him dying then a game over screen executing my character in first person. Yeah made em scared of video games until I got to play Crash Bandicoot. which stopped it. Oh the father you get the more bosses kill you in first person which scared me even more.

I had a ton of those as a kid, more than i can remember, i was terrified of everything. Literally the bull enemies in Spyro scared me, that's how bad it was, but now im pretty damn immune to fear.

I was pretty much the same way. I couldn't even handle random encounters in Final Fantasy games, at first. By the time I actually started playing horror games in the Fifth Generation, though, I just wasn't getting scared of things in games, anymore.
I’ve been told about sleep paralysis many times, nothing so far
The T Rex from the PS1 demo disc stuck with me as a kid as something that unnerved me alot
I feel like the closest thing was the format disc red screen on the PS2
