Sarcoidosis treatment - can you avoid steroids?

Treatment for sarcoidosis generally starts with corticosteroids (such as Prednisolone / Prednisone). Not every patient with sarcoidosis needs treatment. Corticosteroids are a very effective treatment, but many patients are worried about side effects (such as developing diabetes or high blood pressure). In sarcoidosis, we generally need to treat the condition for a long time (12 months or more).
We can sometimes progressively reduce and stop corticosteroids after this period or replace them with another "steroid-sparing drug", such as methotrexate, azathioprine or others.
Treatment in sarcoidosis is personalized in each case, because it is a complex disease. It is generally started if symptoms are very significant, there is involvement of vital organs such as the heart, or blood calcium levels are very high.

    I’ve never had steroids. By the time it was diagnosed it was starting to die in my lungs, however, no one mentioned that my liver and spleen were involved. I’m not sure now how much damage. I’m beyond frustrated with my lack of treatment plan. My doctor asked me what sort of treatment plan I should have. I’m scared. I can’t change doctors because there’s no one else. We’re in a doctor shortage. Now, I’m suffering from pericarditis. Still no steroids, no medication whatsoever. I’m concerned.

    Hi Dr Stefan, Thanks for sharing valuable information regarding treatment and alternative treatments other than corticosteroid. I can be a case study as I am using corticosteroid - Wysolone 10 mg from 12 years. I don't have any side effects other gettiing weight gain easily. But if incase I skip the medicine for a day or two then I get running nose and 100s of snizs in a single day. Request you to please suggest shall I try steroid-sparing drugs as I tried reducing corticosteroid to 5mg but that didn't help. Thanks in advance.

    I'm apparently stage 4 still no medications my live is enlarged with severe fibrosis in my lungs and no idea what my prognosis is I basically don't know how long I've got left. Any info would help even if it's bad news at this point.

    I've been living with lymphatic sarcoidosis for many years, and I'm uncertain about starting medication. The most significant issue I face is severe fatigue, particularly during flare-ups. I have no ability to work and hace to significantly slow down my pace. At 34, I feel as though life is slipping away because I'm in bed more than not. Should I consider the potential risks of medication?

    I will never take any types of medications, especially prednisone. I take two tablespoons of turmeric every morning and about 4 tablespoons of apple cider vinegar and I had no mucus build ups. You can order turmeric power online. I was diagnosed in 2016 and have not taken any prescribed medications since 2016.

    If you are taking Turmeric, I believe you can significantly increase the health benefits by also ingesting black pepper, I know you can get Turmeric tablets with this added specifically for fighting inflammation.

    Thank you DR for sharing I was diagnosed with it a few years ago the Dr put me on 20mg prednisone then 10 mg I feel bloated stomach ache feel uncomfortable, my rheumatoid arthritis Dr Dr put me on prednisone but my pulmonary Dr said I don’t need medication

    I was just diagnosed with Saracoidosis. I am 47 ur old white male. I have enlarged lymph nodes in the middle of my chest and Granulomas in my lungs. They started me on steroids 30 mg prednisone. A day for 30 days then 20 a day for thirty days. I don’t have a primary so my pulmonologist is it I also have a cardiologist I have had a fib for 5 years I think this may be what has been causing it. Any advice?😊

    I have some advice; I am a 46yo male, and I was diagnosed with sarcoidosis last year after light breathing difficulties. X-Rays showed a large amount of “candy flossing” in my lungs, in both, but more so in my left, it was horrifying. I was put on a 6 month course of prednisone, but unfortunately this didn’t clear it up. 3 weeks after finishing the course I was restarted on a second course, and now I’m down to 10mg a day, and will be until the Sarcoid Team at my Hospital have found a drug to take it’s place. As the first course was ending I noticed, often when relaxing on an evening, that I felt anxious, and that when this was the case my heart felt like it was racing occasionally. I was given an MRI on my Heart, and they found granulomas there as well. The granuloma can infiltrate the heart in a way that interrupts the bodies electrical signals to it, and the heartbeat can become irregular, this can cause it to play catch up and beat erratically from time to time. If what I’ve read is true, apparently 25% of people who are diagnosed with sarcoidosis granuloma in their lungs will also get them in their heart. Often this isn’t diagnosed until post-mortem, and is often the reason why seemingly healthy middle aged men have a heart attack. If you have it in your lungs push to have them check your heart, sarcoidosis is still relatively new ground, if you do have it you’ll be glad that they find it earlier rather than later, and if they don’t find it then good for you.     I was diagnosed with Neurosarcoidosis in June of 2021 . I suggest you change your diet cut out meat eat lots of organic seeded fruits and plenty of organic green vegetables. Drink lots of spring water and coconut water google anti inflammatory foods. Herbs are going to be your best friend. I am suggesting what’s working for me because Neurosarcoidosis is the most extremely painful form of Sarcoidosis. I just received my two year follow up and my test results were excellent but I am committed to a excellent nutritional lifestyle and I have been 30 years prior. I wish you all the best and if you are able to exercise whatever you can do please exercise whatever you are capable. And lastly if you drink smoke etc stop that for life.     ⁠@@kakolipradhan771 I would recommend cutting out Alcohol, the Steroids will damage the Liver, so promoting better health by cutting out other things that will damage it makes sense, chances are as things progress beta-blockers may be prescribed as well and Alcohol nullifies the effects, so best to knock it in the head now. If long term management is required then they will want to stop the steroids, and there will be an overlapping period with the drug they want to use moving forwards (in my case this is Methotrexate aka MTX, and this overlapping period is causing some liver toxicity making me feel lousy). I’m glad I stopped drinking alcohol when I started the steroids as it has made things easier, doesn’t take long until you don’t miss it, and there are lots of positives, steroids cloud the mind a little, glad I’ve never had a hang over on them. Sugar needs to be limited, Sarcoid granulomas are located within scar tissue (created by the immune system attacking the body), within inflammation, and inflammation thrives on sugary fluid, so lower the sugar and stop the inflammation from growing. With the Heart the inflammation can interrupt the electric signals and cause arrhythmia (in my case it’s SVTs, which is sudden rapid heart rate for very short durations-often I’m unaware). Look at how to improve the diet and limit inflammation, Turmeric is good, but it needs to be combined with black pepper to be better activated in the body for the best results. I don’t agree with cutting out red meat, it’s vilified when it shouldn’t be; packed with loads of nutrition especially in the fat, in my opinion carbs are the real enemy, they are sugars, and need to be limited to a low amount (I’m trying to move over to a more Mediterranean diet, with more fish, and on every plate the meat is about half of the meal), this will have a very positive effect. Eat lots of veg, and vary the colour as much as possible. If overweight then start to limit portion sizes, this should be done gradually and will get easier, lowering the sugar intake immediately has the best affect on weight, but will make you feel lethargic until you become more accustomed. This is why the fat level of the fish and meat is important, energy will be taken from this in place of all the sugars. Vitamin D3, in large amounts (I’m on 1000 UI/day) will promote the regeneration of the heart. Magnesium is good for the heart (and lots more), but needs to be taken with Zinc to be fully utilised by the body. I’m not taking this as of yet, going to get the OK from my Doc to proceed. Sleep, I need more sleep than I used to, not sure if it’s the Sarcoid or the drugs, but getting a good nights sleep is imperative to managing good health. When I put my head down I worry, I think bad thoughts that will only lead to anxiety or depression. I’ve found reading, if only for five or ten minutes, stops me from brooding on such thought. I read my Kindle, in the dark on the lowest light setting and it makes me naturally more restful. All the drugs can break up the sleep, steroids will make you need to get up in the night to have wee. If I get back into bed and can’t go straight back to sleep I read again, and 10-20mins later I’m back to quality sleep. Steroids are incredible, and after time the sides effects become more manageable generally, I rarely wake anymore and sleep through as I used to before the medication. I’m sorry to hear of your Dad’s struggle with Cardiac Sarcoidosis. Stay positive, as positive minds tend to lead to more positive outcomes, my Wife is incredible and has made it all far more tolerable and easier to cope with, loved ones can have a huge impact on recovery, but he has to try himself to change things for the better, when he needs telling off be firm and make it clear that it’s effecting everyone. The Heart regenerates very very slowly, and most of the damage caused by the Sarcoid to the Heart is near permanent (in my case the inflammation in my heart is non-active, it’s left damage by scarring, but at least it’s in decline). Damage to the lungs by scarring is permanent (I have multi-system Sarcoidosis so it is also attacking other areas, lungs, liver, pancreas, bones, saliva glands), but other areas of the body can recover. Sarcoid seems to be in waves for me personally, I’ll feel OK for days and then rubbish again for days. Get sensible levels of exercise on the good days, and take it easy on the bad ones, listen to how your body feels. I hope things work out for your Dad, most people, mainly men, don’t know they have cardiac sarcoidosis, and sadly the family find out in post mortem, so it’s a very positive thing that you are aware he has it now, as you are able to intentionally make changes where others haven’t been so fortunate. I wish you and your family all the very best. *Edit: Magnesium shouldn’t be taken with Methotrexate, and Zinc shouldn’t be taken with Prednisolone (corticosteroid), or any drug being taken to suppress the immune system …so I won’t be taking these supplements.

    Had sarcoidosis since age 24 im now 60 took early retirement at age 50 because it became lung fibrosis and pulmonary hypertension (48.2mm ) resting oxygen of 93 ,88 when mobilising the fatigue is awful apart from inhalers i get no treatment can anybody tell me at what stage i should get ambulatory oxygen

    Thanks for this video. A dear friend of mine has been diagnosed lately and is suffering from fatigue and a huge weight loss over 7 months. She has had all the scans and biopsies, the result is a so-called mild version of sarcoidosis, no lung problems, luckily, so the doctors won't prescribe meds. But what can be done about the chronic fatigue? 🤔

I haven't had a biopsy done but all my test results came back and everything was normal and good the doctor also told me I might have early case of this but it hasn't been confirmed.
