Awesome lecture Jaz. Do we need to remove the cords before impression ?

thanks! for impressions usually yes - you want a nice fat sulcus recorded by the imp that will cast predictably. For digital - depends if the scanner can capture the margin or not.

Rough cut the prep slightly supra-gingival. Don't touch the gums. Pack 2 cords. Now do the decay/old filling removal and build-up while the cords are sitting. Finish the prep. Turn the water off at the end to "smooth" the prep, especially margin. You will rarely get bleeding if you pack your cords early.

Should I prep my tooth to the 000 retraction cord? Make it slightly subgingival

Hi Amit! It depends. If I am working anteriorly, and it's a dark tooth and I WANT the crown to be a little subgingival, then yes, after the gross prep equigingival I will add cord. By adding the cord, the prep is now SUPRAGINGIVAL. I then prep this to make this equigingival to the cord - which in turn means the crown will sit subgingival. Will also allow you to grab more ferrule. Only time I would do this posteriorly would be when trying to grab more ferrule. Does that help?

I’m playing around with this technique to ensure predictable gingival margins with my veneers and crowns? Do you get issues with biological width violation? Inflammation/ recession etc

  • @protrusive

@@amitpatel5 yes for anterior veneers I do use this technique to help hide the veneer join sometimes - as long as I'm still on enamel ideally. I think with the amount of retraction you get with 000 you would be displacing the sulcus and not really the junctional epithelium or connective tissue, so I don't worry about breaching the BW this way. I am happy to be corrected by a Periodontist who thinks otherwise! Smooth margins/no excess cement is super important
