Hi Jaz , I just want to say that I've watched this episode before and once more today ! ( I've watched many of your episodes more than once btw ) May his soul rest in peace 🙏🏼 was such an amazing dentist. To drill is such a big decision, I always think not only once but 10 times before I cut a tooth . Yesterday I had 2 early teens patients with leasions that are " borderline!" and before I submit the treatment plan , I needed that reassurance that I am doing the right thing. It is very appreciated to have such a valuable bank of information from the best in the field to make our daily dental life easier. Thank you again 😊 On a different note, I am interested in the RBB lecture / notes you've mentioned at the start of the episode, kindly let me know how to access it , if still available. Thank you 😊

Hi Jaz! A question. Whenever I do full mouth rehabs, I usually order a set of pmma crowns and adjust the occlusion on these and then the patient wears them for a trial period. If everything is great I just scan these and send to the lab so they can copy the occlusal design when making the permanent crowns. Now to my question. Should I expect them to be able to accurately copy the scan? In my experience I've always had to make some adjustments (and it bothers me). Is it expected or is there something wrong with the dental lab work?
@protrusive14 күн бұрын
I also adopt this approach - usually there is a little adjustment to do, but not major. There will always be lab inaccuracies (often beyond control) to replicate the restorations exactly. Also the articulation on their articulator will not exactly mimic the patient, so there is some error here. I think a big issue is bite registration of prep vs prep - when the lab mount these, very often there is a discrepancy - even for a single crown, when the lab mount the models, they must be stringent to check that the wear facets match up - very often the wear facets will not be matching and some model work is required to get them to match (and hence OVD is now correct). I hope that a lab technician can give more insight in to the challenges and nuances of this approach

I have a question about pain/sensitivity. Dentists ask questions such as is it a sharp pain or dull, do you feel pain upon biting, is it with cold/warm food, etc. Can you explain what information the dentist is looking for and how it helps them diagnose?
@protrusive19 күн бұрын
I wish my channel had capacity to serve patients too - this would require a small essay. Wishing you the best
@WayneTrain4517 күн бұрын
Dentist here. The purpose of those questions is to help us determine what kind of issue you may be having with the tooth or confirm something we may be seeing on an x ray or other type of image. For instance, if the pain is sharp in response to cold or sugary foods, it could simply be a cavity that needs a filling. If the cavity has reached the nerve and caused irreversible inflammation, you’ll often find that the patient is sensitive to heat and needs a root canal usually followed by a crown in order to save the tooth before it becomes necrotic because it won’t heal on its own. If the tooth is necrotic (dead), you may have a lingering achey sensitivity to pressure when you bite down but absolutely no ability to feel cold, whereas if the tooth is alive but sensitive to both cold and pressure when biting down, it could be due to harmful grinding habits. If the tooth is alive and sensitive to pressure that worsens when you *release* the bite or when biting on a little instrument we use to check each cusp of the tooth, you may have a cracked tooth that needs a crown or a root canal and a crown or possibly extraction depending on the extent of the crack. Those are just a few examples of how those questions help us figure out the best type of treatment for your specific issue or what further tests we need to run to narrow it down. Hope it helps!

For what’s it’s worth and without starting WW3 I hate air abrasion as the powder just goes everywhere. Honestly I would have to showery to get rid of it all Instead I like ton 1. Disclose 2. Use a micro brush dipped in sodium hypochlorite and scrub off biofilm - no evidence as such but you can clearly see it coming off 😊 3. Then I use a bristle brush with a ALo3 pumice to “abrade the tooth” Works well for me
