Regular astigmatism vs irregular astigmatism, corneal curvature - A State of Sight #57

Ғылым және технология

Astigmatism is common and can be classified as regular or irregular astigmatism. Learn the difference between these in this episode of A State of Sight with Isaac Porter, MD.
Most astigmatism is regular where the cornea is curved more in one direction, but irregular astigmatism can also have an effect on vision. This can be more prevalent in patients with irregular corneas from keratoconus, scars, or injuries.

I have regular/ATR astigmatism (as far as I can tell) with a prescription for -0.75 at 90° (L) and -1.00 at 93° (R). My pupillary distance is 68-69mm, but the glasses that came out of the shop show around 59mm inter-center distance when measured on a machine (triple-checked at different places). I was told by one of the opticians that PD is irrelevant for non-spherical cylindrical lenses (i.e., it won't make any difference to my eyesight) and that it was probably ignored and done at 59mm to fit the shape of the frame of my glasses the best (frame is sporty, slightly curved to the sides). Is this credible info? Or should I ask for new glasses with correct(ed) PD?

Hey Doc. . . I was just wandering, can any kind of eye glasses prevent me from having IA. It's been 6 years when I noticed that I was having this severe headache and blurry vision. I even consulted an opto for any possible options to cure or somehow prevent it. It had became worst over the years.

***** Very sorry I deleted your last comment - trying to figure out this new YT comment system. Irregular astigmatism can cause multiple images in the vision but regular astigmatism should not cause this. A poor tear film can cause a rough surface on the surface of the cornea and not let light pass through smoothly. This can also cause multiple images.

Hi, just wondering, can a patient tell if they have regular or irregular astigmatism without tests? For example, I read somewhere that folks with regular astigmatism tend to see "double vision" on the top and bottom of objects or side to side, but those with irregular astigmatism will see all corners of an object as fuzzy. Is this true?

irregular astigmatism cannot be corrected by soft contact lense or glasses lens. regular astigmatism can be treated by those.

I have irregular astigmatism from LASIK. I am told the fold or aberrations are on the surface of the eye. It has been almost 20 years now living with just one good eye. Is it possible now to correct? Your video seems to suggest a custom ablation could do so. My eye is also very dry which prevents a contact lens solution. The underlying vision appears to be sharp ( I can see under the distortion at a very steep angle). It appears the the blunt nose on the microkeratome impacted the corneal surface but I cannot categorize medically what caused the irregularity. I came across this video searching for the use of castor oil as a possible remedy? Someone was telling me it cleared up his cataracts.

Good Morning, I hope I will get an answer for this, I am a fellow of cardiology and I 've noticed Vision ability is getting worse every year and it really affects my life quality, my ophthalmologist said I have an Irregular Astigmatism . Is this treatable with an Interocular lens Implantation? I heard also about a Topography oriented Lasik I hope I will get an answer since i am really worried. Thanx and best regards from Germany

I'm not a doctor but how can you say at the end that irregular astigmatism is less of a problem for vision than near/farsightedness or regular astigmatism? The others can be corrected perfectly with glasses, while irregular astigmatism can cause ghosting / multiple images that can only be corrected with extremely uncomfortable RGP contacts.

Yes I heard that at the end of the video too, and had to back it up to relisten. I have to believe he said that in error. And he meant it is much more of a problem than regular astigmatism.

Tell me what solution you choose. I don't want contacts coz in long term they maybe bad. Did you get LASIK? I get multiple images and specs don't correct them. I was confused why the doctor insisted that they work when they clearly don't. What is the best solution out of it?

@@Atiq_s I tried RGP contacts and they didn't work for me. They made the ghosting tighter, but I still had ghosting and glare. A few months later another doctor did Pentacam imaging and thinks my symptoms are caused mostly by bands of opacities in the lens. He didn't use the word cataracts, but it sounds like he was describing early mild cataracts. I think my corneas are still contributing. I have chronic eye inflammation issues and I've gotten a lot of different explanations: Blepharitis caused by bacteria, caused by allergies, caused by ocular rosacea, or all of the above. I've had repeated occurrences of marginal keratitis, then papillae in the eyelids suggesting allergies. The conjunctiva is starting to grow onto the corneas. I've done eyelid cleansing and warm compresses for years but it don't help as much as they should. I'm trying to figure out if I'm allergic to something. I have no known allergies. Maybe I'm allergic to something in baby shampoo, so I'm going to try ocusoft foam for eyelid cleansing instead. Between each piece of information in this investigation it's like "come back in a year" or 6 months at best. I've never had a medical problem that gets fixed quickly and easily. It's always this slow-burning stuff that no one can help with. Most people I know have some problem, get it fixed and they're done. The good news is that my vision hasn't changed since the first year this started, so I did get used to the way I see. But if I don't stop the eye inflammation it will damage the corneas.

+Tianny Silva Yes i have studied alot about it because i have it myself.. It can come from surgery of the eye, damage/sickess to and of the eye or be there at birth. I must have been born with it but it didnt start to give me problems until i was about 25 years old. It´s not a dangerous condition and everybody has some sort of astigmatism, most just have so little that its not bothering them.
