Depends on what you are planning to do at the gym. In an adult hand frcatures take approximately 3 months to heal, and, assuming all is going well with your bone healing, I would recommended no heavy lifting for 3months after your surgery. However cardio is fine. For any individual case however, it is always best to seek advice from your doctor or hand therapist. Hope this helps.

Thank you. Yes, as long as the fractures were stable (i.e. were not moving around when you moved your fingers), or stabilised by operation, and are still stable. For any individual case however, it is always best to seek advice from your doctor or hand therapist. Hope this helps.

I'm seeing the hand doc. today. Hopefully all goes well. thank you for your time and knowledge to share in the public as to how we can strengthen our hand, in my case the 4th and 5th metacarpal. Thus far, after 5 weeks with a splint, no strong pains. That could be a good sign. However, I do feel some pain and swelling with soreness. Which I'm pretty sure to be excepted. I will continue to apply the hand therapy you have suggested. I already seen some strengthening improvements and better 4th and 5th metacarpal movements. Thanks again for your valuable time! Kind regards, Danny

This is not negative, but really sad that I can find a better Way to go simple exercises without editing a month for an appointment. Especially when, personally, you feel like you haven't gotten the proper help. Thank you Soo much for these exercises. I broke my knuckle in my pinky finger, "boxer fracture,," as well as shattered the middle of my middle finger, and due to a full on disagreement with the Dr. at the time I have been left without then pins in the final 3 weeks,( personal decision, on me fully) but was left with no info at all on what to do once/,if it healed properly. No matter how late I am, Thank you for this video!!!

I am having my stiches removed today, and I am starting with excersise, I had 3 pins in palm, and my last (smalest finger) cant bend or straight all the way - is this because of th einjury, or because I was not moving it for two months I had pins and partial cascet on? Also, did you say to repeat those excersises every hour? What is the average time person return most of his finger mobility? My bone has healed "like a stone" , meaning great, but now I need to handle my reduced mobility - how long peopl eusually take to recover (I know every case is unique, but we are speaking in general). Hi i broke my right 5th metacarpal 4 years ago. Had surgery and got a metal implant because the bone wasn't staying in the correct position. I think it was because the muscles were to tense there, and pushing the bone back out. Today i have full movement of my hand, but it does ache when i do so. I was wondering if getting the metal implant removed would help reduce the ache i have in my hand.

Hello Dr. Please i have a question , a fracture in the 5th metacarpal with a 35~40 degrees angulation of fracture needs a surgury to install the pins or it doesnt have a surgury, Why i’m asking this because i had this fracture 5 weeks ago , now i removed the splint and doing these exercise but i noticed that the knuckle is not visible.. will it come back again and heal ? Or i should’ve done the surgury?

Fractured midshaft of my 5th metacarpal on April15, with orthopedic practice I was considered done healing June 1st (approx six weeks) since then I’ve regained control of everything except my ring and pinky finger. Am I supposed to be pushing my hand with these exercises or am I better off waiting the full 12 weeks then starting to work those fingers.

The exercises shown in the video should be commenced immediately after it is established that hand fractures are stable (i.e. shall not move out of place when the digits are moved). This may, or may not, require an operation. These exercises are designed to prevent joint stiffness and ensure optimal movement of the digits. These exercises are NOT designed to strengthen the hand. The exercises should be performed as many times as possible each day for at least 3months. During at least the three months after the injury, or until the fracture is healed, one should neither lift heavy things nor do any strengthening exercises. One should not commence strengthening exercises until advised to do so by surgeon or physiotherapist. Hope this helps.

Hi there, thanks for the video. I had a 4th Metacarpal complete break 3 weeks ago (surgery was 2 weeks ago; metal plate and screws were put in to re-align the bone and keep in position whilst the bone heals; ORIF procedure). I've been doing these exact exercises since around 5 days after surgery, but the thing i've picked up straight away since watching your video is that I've been doing these with way too much force (really trying to squeeze, especially with the last full fist exercise, thinking that I'd recover faster if I did so). My main concern is that I've pushed so hard that i've bent or moved the plate (as it feels funny when i move my fingers at the moment). I just wondered if you've ever come across this and if i'm right to be concerned? Any help or advice would be much appreciated. Rik

How long after K pins are removed can you start doing exercises. I had a closed PP fracture in my pinky. It happened at work, so worker’s compensation has to approve therapy first. So when can I start doing exercises? Or should I give it a few days before trying anything?

This depends somewhat on how well stablised your fracture was after pinning (K-wires). However, generally speaking one should really start a gentle, graduated, physiotherapist supervised excercise regime within a week or two of having the fracture stabilised. My underpinning principle for management of hand fractures is “ Stabilise and Mobilise”. Otherwise digits/hands can develop stiffness, which may result in permanent decreased movement of digits and impaired hand function. Hope this has answered your question. Best of luck!

Thanks for your email. The pates and screws used for hand fractures are quite small and thin. Whilst I have not come across the plate bending due to exercises, it is not inconceivable. If you are concerned, I would seek a medical, opinion, and probably beat to have your fracture/plate re-xrayed, to be sure. Best of luck. S Majumder (Consultant Hand Surgeon)

I broke my 4th metacarpal 4 weeks ago & had the plaster removed yesterday. I was given no exercises or referral for hand therapy just told no driving for 2 weeks (which I don’t understand) & no lifting for 4 weeks so I’m feeling very much alone & unsure as I don’t want to do more damage. I have a lot of wrist pain & stiffness to my fingers which I think is due to poor casting. I did lots of gentle massage yesterday & am already getting a bit more movement. Are there any any normal activities that could do further damage? Thank you for this great video which is so easy to understand I am starting these exercises today.

I broke my 4th metacarpal, pinky finger and radius in a MVC and I have been taking therapy for my hand. The 4th metacarpal is the most that hurts. You definately need heat to get circulation going and excersises 3-5 times a day. I deal with some stiffness daily but all this helps. Keep it moving and try to get the therapy at least 2 times a week.

In adults bones take, and on average, 12weeks to heal; also when broken hand bone are treated with a cast, there is not uncommonly, some stiffness of the hand/wrist joints initially when the cast is remove. So, do not over-stress your fracture, and be patient with your joint movements; persist with regular physiotherapy, but do not overdo it.

From the little information you have provided I would suggest that there may be scar around your tendons and/or joints restricting the movement. So long after the injury/operation it is unlikely that physio would help any further. You may need further surgery to release the scars around the tendons/joints. I would recommend consulting a hand surgeon.

Difficult to comment accurately with such little information, but generally speaking, assuming optimal management and physiotherapy, one should expect good movements after a few weeks; regaining grip strength may take somewhat longer. I would advise that you have an x-ray to confirm that the fracture fixation is satisfactory, and do your physio every day, as often as you can. Best of luck!

Good question. If you mean how long should you continue your exercises for, the answer is at least 3months; some patients may benefit from carrying on for longer, until the improvement plateau, or normal hand function is achieved (good movement of digits, with little/no stiffness). Hope this helps.

Anyone here broken their 3rd metacarpal bone??? I did just last month was told it was on a 20 degree angle had a cast put on and was sent to another hospital because apparently i needed surgery according to the first hospital... well when i went to the 2nd hospital i was referred to by the first hospital that dont have plastic surgeons the one i was referred to cut my cast off done a xray and when results came in i was told i didnt need surgery which i was very concerned about it considering the first hospital told me i needed it long story short they put a splint on and i have to have it on for 2 weeks then go back to the 2nd hospital to possibly have ot taken off and do hand exercises which is in a weeks time but when ive taken the splint off for showers ive tried to do similar exercises to see what would happen... for example i clinch a fist not fully tight but just lightly and my index finger and middle finger are very tight... is this normal? Just concerns me as i do muay thai kick boxing and i dont want it causing me any problems in the future im not aloud to punch with it for 3 months of course but even after the 3 months im concerned its going to give me grief... even knowing my index finger and middle feel tight while lightly clinching a fist is that just normal and does it go away? I hope so... was also told ill always have a lump in the middle of my hand which i dont like to look at lol... thanks.

I am assuming the fracture has healed, in an acceptable position? If this the case then you have now work at getting your stiff joints moving, all 3 joints associated with each finger. Physiotherapy works better the more often you do it; little and often, rather than too much at each episode of therapy. Decreasing the swelling in your fingers using massage and hand elevation helps the physiotherapy to improve finger movement, and increased finger helps reduce finger swelling, so a positive loop! Would strongly suggest expert advice and supervision by a hand physiotherapist. Hope this helped. Best of luck!

I have open fracture of second third and fourth metacarpal bones. They put screws in the second in the fourth one the third one two k wires which have since been removed. I got caught up in medical paperwork so I've been unable to get into therapy for my hand I'm doing what I can by watching these videos. But so far they've only focused on breaking one metacarpal bone. I've broken three. Should I still be doing the same exercises ?

I fractured my 5th metacarpal . Its been a week since the cast came off. I am not able to make a fist. The pinky fingers seems as if it's really stiff. How long will it take for me to make a fist ? I am worried.

Try dipping in COLD ICY water for 15 sec then immediately into HOT water for 10-15 secs... It will heal really fast.

Hello sir, i had an small accident and 5th metacarpal was fractured, and it's just in position, doctor said, no need for k-wire , just split is enough for 6weeks. 2weeks back split is removed, now 2months completed, but fracture line still visible, how long it will take to not shown crack line on x-ray, and I'm afraid of do physiotherapy, because when I'm doing physiotherapy some pain on fracture site. Give me some suggestions, please, thanks in advance....

The fracture takes, on average, 3-months to heal completely, but from 4-weeks it is stable enough for aggressive physio( but NOT stable enough to lift heavy things). THE PHYSIOTHERAPY IS VERY IMPORTANT.

Basically that’s my orthopedic told me. I broke mine last week and I don’t need surgery. They said we could cast it, flexible splint or wear the splint I have already with the wrap. But he doesn’t want me to really want me using it all day. He wants me to practice making a fist. It’s confusing because I had a clean break so idk how I’m expected to make a fist when my hand feels tense. I can say I can kind of make one but my pinky is stubborn.

Sorry to hear this. Certainly do your physio, but somewhat worrying that at 50days you still have a wound. I would advise contacting your surgeon to ensure all is okay.
