    Thank you for the incredibly important and organized presentation. You said that pediatrics fellowships are easily found on H1B. Is it still harder than finding a fellowship on J1? Also, you said that waiver jobs are harder for pediatrics subspecialties. Does this vary by speciality?

    Peds fellowships are easily available on H1b as mentioned in most sub specialties. The problem in J fellowships in Peds is you may end up in primary care waiver job.

    Hi, please clarify my doubt. I heard from someone that previously Indians were exempted to do the 3 j1 waiver job, (an alternate to the 2 year home return requirement) and that recently only the rules have been changed to force Indian IMGs to do the waiver job. Is that true? Because I've heard from other people that people have always had to do the 3 year waiver job, regardless of whether they were Indian or not

    Categorical residency does not last 7 years but if you add fellowships etc then you either need special approval to extend or if not extended then do waiver/go out and clock starts again

    At 6 minute mark you say " many people on h1b work for 2-3 years before applying for a competitive fellowship again". Please clarify during those 2-3 years do we need to earn a GC also? I'm Indian so that is pretty much impossible to do in such a short time. Or do we just need to work and build our CV and then apply for fellowship again under h1b only?

    @@notonfire7318 No....coz according to him those 3 yrs spent doing job in US will be used to get u PERM labor certification and u can apply for green card within that period and once u get ur green card, u can apply for fellowship after that.....BUT and there is a big BUT The backlog of Green card waiting list for Indians in USA is 150 YEARS....u will probably die before u get ur green card.

    You can come back if you get a job offer on h1b. Most dont leave and keep extending h1b. Employer has to extend

If I apply for h1b during my residency and after then I get a job and apply for labor certification, I have 3 years left but the green card backlog is of 150 years in US for Indians. How will I get my green card within 3 years unless I marry a US green card holder?

    You will get a work permit which is fine. It is called H1 EAD

    Total on h1b is 6 years. If you are from india then in 3 years of job you cant get green card. Other countries can. So they get back to fellowship after GC.

    There are very few H1b programs in FM. Waiver is easier though given the primary care needs.

    Hi Sarthi, My status: H1-B approved, I-94 and I-797 validity: Aug 2025. I-140 approved. The following details are for my spouse: 1. If we get a J1 visa stamped from India(home country) and start pursuing MD Residency in USA, should we visit India consulate each year for J1 visa renewal or only if we visit India, then we have to get a j1 visa stamped? 2. Can h4 to J1 visa change of status be done while we are in USA. would there be any issues converting from H4 to J1 near visa stamping.

    Step 3 is a requirement if you want the program to sponsor h1b for you. They need that result immidiately after the match to give them enough time to file paperwork for your H1b.

    @@suryateja197 h4Ead to h1 may be tough during residency but doable. Many of our students have done it. Programs keep uodating their requirements. So we update our internal program lists for students each year about visa. Otherwise you can check frieda

    @@usmlesarthi2024 Thank you so much for answering. 1. does h1 offered by the program need to go through the lottery system for h1 or is it cap-exempt. I guess they don’t need to go through a lottery system 2. Is it very common for programs to offer J1 or H1-B sponsorship? 3. My fiance is doing her final year DNB and would want to take USMLE and eventually take do a residency. I have my I-140 approved and she would be eligible for H4 EAD. Can she apply for some kind of visa in India before she comes up here to the US and prepares for USMLE? The reason I ask is that I feel that it is good for her to have her own visa instead of a dependent visa.
