It's not breaking the 4th wall, it's alerting the other members of the team that Joker needs help. Futaba does that too and usually you would have Sumire with Akechi and Joker too

Akechi talking about his ailments is hilarious, extra points to the brainwashed ones

Akechi never had any sanity. Everything we saw before his boss fight was all an act in order to set up the Phantom Thieves. He never was an Ace Detective and he never was as innocent as he looked, what we see here is normal Akechi
