Hello, my name is Janet Yarsanaeva, I am a gynaecologist from St. Petersburg. I do office hysteroscopy and often watch your videos on KZread. I am interested in a question, how do you maintain constant pressure in the uterine cavity when working with a grasper and scissors? I have difficulty maintaining pressure in the uterine cavity, which makes it difficult to work. thank you for your hard work

Thanks Dr Janet. If you look closely, you see that the working part of the scissor is broader than it's stem. Also, assuming you use a Bettocchi sheath, the instrument and the inflow share the same channel, and the problem you face is common. The trick is to first distended the uterus and THEN insert the scissor in. Once the instrument is in, keep the broad working end inside the cavity and not at the tip of the sheath, where it could block the inflow of saline and cause a loss in pressure. Hope this helps!
